2.Epidemiologic Study on Diabetes Mellitus in Primary School Children.

An epidemiologic study on diabetes mellius in 13,152 primary school children(6,378 boys 6,765 girls, 9,302 Kwanju-city pupils 3,850 farming area pupils) had been performed in the areas of Kwangju-city, Hwasun-gun from December, 1976 to June, 1977. At first, Diastix was used for detecting glucosuria in one to two hourpostprandial urine and pupils who showed even 0.1% sugar in urine were considered for glucosuria. Children with glucosuria were given the oral glucose tolerance tests using 1.75gm of glucose per kg body weight. Chemical diabetes was diagnosed the cases whose blood sugar values 1 hour after glucose loading were over 170mg/dl and 2 hours after glucose loading over 120mg/dl. Cases which met only one criteria were applied to cortisone-primed oral glucose tolerance test according to the method of Fajan and Conn. If the bolld sugar value 1 hour after glucose administration was over 180mg/dl and the sugar balue 2 hours after glucose administration was over 140mg/dl, suspected diabetes was diagnosed. The results were obtained as follows : 1) The overall positive rate of glucosuria was 0.167%(22 of 13,152 cases). Male pupils showed 0.188%(12 of 6,387 cases) and female pupils 0.148(10 of 6,965 cases), and pupils in Kwangju-cith 0.161%(15 of 9,302 cases) and those in farming area 0.182%(7 of 3,850 cases). 2) 13 of total 22 pupils with glucosuria revealed abnormality in glucose tolerance(59.1%). 3) Of 13 children with abnormal glucose tolerance, 3 were overt diabetes(23.1%), 6 were chemical diabetes(46.2%) and 4 were suspected diabetes(30.8%). 4) The overall pervalence rate of diabetes mellitus was 0.099%, consisting of 0.125% in boy and 0.074% in girl, and consiting of 0.086% in school school children of Kwangju-city and 0.130% in those of farming area. 5) Of 13 children with abnormal glucose tolerance, 12 children were in the age group of 10 years and over 10 years.

3.A Case of Disfiguring Xanthelasma Palpebrarum.

We report a case of xanthelasma palpebrarum (XP) with bilateral extensive and disfiguring involvement of all four eyelids. The patient was a 28-year-old woman and noticed this condition eight years ago. Physical examination showed yellowish flat topped bilateral extensive plaques around the eyes. Laboratory result was remarkable for a decrease of a serum high density lipoprotein cholesterol (HDL-C). Other laboratory findings were within normal limits. Histopathologic findings showed diffuse infiltrations of foamy cells, histiocytes and foreign body giant cells in the dermis. Electronmicroscopic examination showed that the dermal infiltrate was composed of lipid laden histoncytes showing multiple villi which indicated an activated state of histioncyte. Bilateral and extensive XP is very rare. Only two cases have been reported in the literatures as far as we know.

6.Evaluation of Treatment Effect of Primary Hyperhidrosis using Skin Surface Hydrometer.

BACKGROUND: Primary hyperhidrosis is the disease of production of exessive sweat which is mainly localized in palm, sole, and axilla, treatment of the disease is usually accomplished with topical 20% aluminum chloride solution, an iontophoretic device, and surgery(sympathectomy) being reserved for recalcitrant cases. OBJECTIVE: We have compared the therapeutic effects with each treatments for hyperhidrosis using a skin surface hydrometer. METHODS: Thirty six patients(11 ; male, 25 ; femlale) with primary hyperhidrosis were evaluated in this study. Nineteen patients used topical 20% aluminum chloride solution, 10 patients used iontophoretic device, and 7 patients in recalcitrant cases. had sympathectomy. We have measured the conductance on the stratum corneum of the palm and sole suing skin surface hydrometer before and after treatment every weeks for a month. the control group were composed of healthy 10 males and 2 females. RESULTS: 1) In the patients of primary hyperhidroisis the conductances of palm, sloe and axilla were higher than that of control group(P<0.01). 2. There was a marked reduction of conductance after the treatment for one week by topical 20% aluminum chloride solution, an lontophoretic device, and sympathectomy(P<0.01). 3. After 2-4 week of treatment by the change obsetved after treatment for one week. 4. By iontophoretic device, there was a slight increasement of conductance of the palm after 2-3weeks of treatment. 5. We obseved a sustained decrease in the conductance by sympathectomy into 4 week. CONCLUSION: Topical 20% aluminum chloride solution, iontophoretic device, and sympathectomy were effective for treatment of hyperhirosis. Among hem sympathectomy showed the best effect. We observed that the skin surface hydrometer is useful to evaluate of the efficacy of the treatment.

7.Three Cases of Functioning Adrenocortical Tumor.

Neoplasms of the adrenal cortex are rare in children. Some of them are functioning malignant neoplasms. Two girls with variable degree of virilization and Cushing's sydnrome and a boy with only Cushing's syndrome, which were due to functioning adrenocortical tumors, were presented. All iesions were found in the left adrenal gland. Two of them were histologically adrenocortical calcinoma. There was no histological evidence of malignancy in the oter one in spite of considerable increasing of 24hrs urinary 17 kerosteroid excretion. A review of literature was also made.

9.A Clinical Study of Ipsilateral Fracture of Femur and Tibia

Twenty-six cases of fracture of the femur and tibia on the same leg were treated in Masan Koryo General Hospital during the period 1981 1986. We studied all of these patients, divided by four groups according to the method of treatment, retrospectively with analysis of treatment and end results. The results were as follows : 1. Twenty-two patients were male and four patients were female. Of twenty-six patients, eighteen patients were at their third and fourth decades. 2. The main cause of injuries was traffic accident; 23 patients(88%). 3. The most common concomitant injury was fracture and dislocation in other site(14 cases). The fat embolism developed in one case and one patient had died due to head injury. 4. The average healing time of fracture was 17 weeks in femur and 20 weeks in tibia in group three, that was most rapid of all groups. 5. A good or excellent functional result was achieved in 87% of the patients treated with internal fixation of both fracture, 75% of the patients with internal fixstion for the femur fracture and external fixation for the tibial fracture, and 25% of the patients trested conservatively.

10.Clinical Study of Tibial Fracture: Comparison of 3 Methods

There is much controversy concerning the method of treatment on the tibial fracture because of frequent complications, such as non-union, delayed union and infection. Authors reviewed and analyzed 110 tibial fractures which had been treated at the Department of Orthopedic Surgery, Sung Ae General Hospital from June 1983 to March 1987 by the groups, conservative treatment, open reduction and internal fixation with compression plate and closed flexible intramedullary nailing. The results were as follows :1. In regard to the fracture location, there was no difference in the healing time among the treatment methods. The I.M. nailing group healed 1.5 weeks earlier than other groups in each location. 2. In minor and moderate severity by Ellis, the I.M. nailing group revealed better result, and in major one, the compression plate group showed better result. More complications occured in major severity. 3. Union time among the trestment methods was not affected by the fibular fractures, but most of the complications occured in the concomitant fibular fracture case. 4. In open fracture, the healing time showed no difference among the groups, and was prolonged about 2 weeks than closed fracture. In closed case, the I.M. nailing group showed good result. 5. The cases, which were reduced earlier in the conservative group, and treated within 3 days in the I.M. nailing group revesled better result. 6. The radiological union time from the definite treatment was 15.80 weeks in the conservative treatment group, 15.18 weeks in the compression plate group and 13.92 weeks in the I.M. nailing group.
