1.A Case of Lichen Myxedematosus.

The patient was a 50 year old woman complaining of generalized multiple pruritic erythematous waxy papules on the face, chest, abdomen, both dorsum of hands and both knee joints areas. There was marked accentuation i>f the skin folds, and the thickened skin was freely movable. Histopathologic findings showed extensive proliferation of fibroblasts throughout the dermis associated with irregularly arranged bundles of collagen, In many areas, the colagen bundles were split up into individual fibers by much, which was confirmed by special stains. She showed clinical response to melphalan therapy, which had to be discontinued because of severe bone marrow depression after 7 weeks.

2.Regional Morphological Study on the Development of the Human Embryonic and Fetal periderm.

BACKGROUND: Morphologic characteristics of the human embryonic and fetal periderm according to body region are not very clearly defined. OBJECTIVE: We have tried to Clarify the sequential development of periderm regionally. METHOD: Skin samples were obtained from 12 human embryos and fetuses ranging from 4 to 23 estimated gestational ages(EGA) and divided regionally into scalp, face, back. abdomen, thigh and sale. Specimens were observed by scainning with an electron microscope. RESULTS: Human embryonic end fetal periderm show distinct morphologic changes as follows along its own sequence of development flattened surface, elevated surface, fiormation of incomplete bleb. single bleb stage, multiple-complex bleb stage, formation of regressinjg bieb. Then towards the end of the second trimester, most of thieperidermal cells are completely regressed. Regional variation in peridermal development is not evide!nt during the embryonic period, but earlier development is apparent in the sole, face and scalp especially in the sole compared to other areas during the late first snd second trimester after EGA 9 weeks. CONCLUSION: The periderm, which can be seen only in embryonic and fetal epidermis, shows distinct sequential developmental changeis with regional variation.

3.Subcutaneous Angiolymphoid Hyperplasia of the Inguinal Region and Anteromedial Side of the Proximal Thigh: A case report

Subcutaneous angiolymphoid hyperplasia is an uncommon disease characterized by eosinophilia, proliferation of the capillary vessels and lymphoid tissue and infiltration of the inflammatory cell, especially eosinophils. The etiology of the disease are obscure, but probably trauma, lower grade infection, nervous factor & hormonal status etc. The predilection sites are face, ears, scalf & neck, but rarely reported in the extremities. We experienced a case of subcutaneous angiolymphoid hyperplasia of the inguinal region and anteromedial side of the proximal thigh in a 10 year-old man who complained pain and tenderness on the above region. The patient was treated by excision with satisfactory result.

4.A Malignant Lymphoma of the Ilium: One Case Report

Malignant lymphoma of the bone is a rare, but involves the shaft or metaphysis of the bone, producing destruction, frequently pelvis, spine, skull and femur. Most of the patients are over thirty years old and equal sex distribution. The authors experienced one case of lymphoma involving the left ilium.

5.Scintimetric Evaluation of Femoral Neck Fractures by Tc-99m-MDP: Experimental Study in the Mature Rabbits

In femorsl neck fracture, viability of the femoral head depends on the revascularization of the femoral head. The purpose of this study is to observe the effect of early fixation and anatomical reduction by measuring the Tc-99m-MDP uptake ratio of the femoral head in experimental animsls. Femoral neck region of 20 mature rabbits were osteotomized completely and divided into 2 groups ; each group consisted of 10 rsbbits. Group 1 had two Kirschner wire fixation after osteotomy and anatomic reduction. Group 2 had no fixation after osteotomy. The uptake ratio of Tc-99m-MDP of the femoral head in each group were measured st the intervals of 24 hours, 48 hours, 1 week, 2 weeks, 3 weeks, and 4 weeks, respectively. The results obtained were as follows ; 1. In experimental group 1, the mean uptake ratio of the femoral head was 0.72±0.13 at 1 day after operation, increased to a level of 1.44±0.14 at 1 week, increased to a maximal value of 2. 90±0.36 at 3 weeks and decreased to a level of 1.10+0.12 at 4 weeks. 2. In experimental group 2, the mean uptake ratio of the femoral head was 0.70±0.10 at 1 day, 0.64±0.13 at 1 week, decreased to a maximal value of 0.33±0.05 at 3 weeks and 0.47±0.05 at 4 weeks, showing decreased level below 1.0. The results suggest that early anatomicsl reduction and rigid fixation of femoral neck fracture is helpful for revascularization of the femoral head and prevention of non union.

6.A Clinical Study of Femoral Neck Fracture

We analysed forty-six cases of femoral neck fracture treated with internal fixation and followed for more than one year at Korea University Hospital from January 1979 to June 1986 and the following results were obtained. 1. Sex distribution was 23 cases(50%) male and 23 cases(50%) female, and average age was sixty years old (range from 21 to 91 years). 2. The most common cause of injury was slip down(76%). 3. The most common type of fracture was Garden's type III(46%) and Pauwels' type III (67%). 4. In cases of internal fixation, the most commonly used method was closed reduction and multiple pinning under the local aneshtesia(83%). 5. Bony union was established within 5.9 months in average. 6. Among forty six cases, 7 cases(15%) of avascular necrosis, 4 cases(9%) of malunion, and 2 cases(4%) of nonunion was occurred. 7. The clinical results were excellent in twenty cases(43%), good in sixteen cases(35%), fair in seven cases(15%), and poor in three cases(7%) according to Lunceford's assessment.

7.Total Hip Replacement of Pathologic Fracture in Fibrous Dysplasia involving Proximal Part of the Femur

The fibrous dysplasia, divided into 3 categories of the monostotic, polyostotic and extraskeletal forms, is a relatively rare disease which was first reported by Well in 1922 and shows a characteristic findings replacing the medullary cavity of noe or more bones by the pathologically proliferating fibrous connective tissues. In a consequence, the structural integrity of the diseased bone becomes weakened and suffers frequent episodes of pathologic fracture and deformities of the bone, which are most common around the hip. Various operative methods and instruments have been tried to manage the pathologic fracture around the hip recently. The authors report a satisfactory resnlt of total hip replacement for the pathologic fracture in a patient of the fibrous dysplasia involving the proximal half of the right femur.

8.Cancer Screening Rate and Related Factors in Rural Area.

OBJECTIVES: Cancer is the second most frequent cause of death in Korea. Cancer screening tests can save lives through early detection. Enhancing the cancer screening rate is an important strategy for reducing cancer mortality. The purpose of our study was to evaluate the screening rate and related factors in a rural area. The study investigated relationships between sociodemographic characteristics, several preventive behaviors, and the experience of several cancer screening behaviors. MATERIALS AND METHODS: The study population was recruited voluntarily from the three rural areas(Myen) in Chungju city. The participants completed structured questionnaire from July 21, 1998 to July 26, 1998. RESULTS: The proportions of the study population who had previously received stomach, liver, breast, or cervix cancer screening tests were 24.5%, 18.5%, 27.0%, 59.2% respectively. The 1-year screening rates of stomach, liver, breast, and cervix cancer were 7.4%, 6.8%, 8.6%, 15.6% respectively. In multivariate logistic analysis, some sociodemographic variables, preventive behaviors, or psychological variables were significantly associated with several cancer screening tests. Those who had previously received a stomach cancer screening test were significantly associated with the presence of chronic disease, physician? recommendation, use of alcohol family history of cancer, or previous liver cancer screening test. Those who had previously received a liver cancer screening test were associated with education level, physician? recommendation and previous stomach cancer screening test. Those who had received a cervix cancer screening test were significantly associated with education level, presence of a transportation vehicle, physician? recommendation use of alcohol and previous breast cancer screening test. And those who had received a previous breast cancer screening test were significantly associated with age, marital status, and earlier cervix cancer screening test. CONCLUSION: Based on the results of this study a strategy to promote cancer screening and health objectives at the district level can be made.

9.A Case of Congenital Diverticulum of Ventricle Associated with Pulmonary Atresia.

The congenital diverticulum of ventricle is a rare cardiac malformation arising from the left or right ventricle, the former being more common. Ventricular diverticulum is usually associated with other anomalies including intracardiac, midline thoracic, diaphragmatic and abdominal wall defect. The authors experienced a case of congenital diverticulum of left ventricle in nine month-old female infant. Left Blolock-Taussig shunt operation was done and total correction will be done later. Thereafter we presented a case with brief review of the related literatures.

10.A Case of Generalized Pustular Psoriasis in an Infant Trated with Etretinate.

We present herein a case of generalized pustular psoriasis in a 17-month-old female infant, who showed good response to etretinate therapy. The baby showed generalized pustular eruptions with high fever and marked leukocytosis preceded by annular scaly erythematous and annular pustular lesions. Etretinate was given in a dose of 10mg(0.75mg/kg/day) for 10 days, and then tapered to 7mg, 5mg, and 4mg in every 10 to 13 days. Clinical improvement was noted after 7 days' treatment and almost all of the skin lesions including erythroderma were cleared after 6 weeks' treatment. She is now maintained with 3mg of etretinate without recurrence.
