1.Update in the imaging assessment of the severity of mitral regurgitation

Adequate assessment of the severity of mitral regurgitation is fundamental to decisions on the timing and the patterns of surgical treatment.Nowadays,two-dimensional echocardiography continues to be the first choice of assessment,but it's considerable limitations preclude its being a real gold standard.With the newly emerging techniques,such as three-dimensional color Doppler ultrasonography,multi-detector CT and MRI,the parameters promoted may have the potential to overcome some of the pitfalls of the two-dimensional methods.Advances in imaging assessment of the severity of mitral regurgitation were reviewed in the article.
2.Segmentation of heart valves in DS CT image based on Contourlet transform

Objecthe Children's heart valves by Dual-Source Compater Tomography(DS CT)images were enhanced and segmented in order to reconstruct 3D construction of heart valves and make motion analysis based on this model.Method,Adaptive seed-filling algorithm is proposed in this paper to segment interested region of heart based on DS CT images.Contoudet transform is used to enhance and segment heart valve based on the part-segmented image.Result With these methods,DS CT images were SUCCESS fully pro-cessed and segmented in Matlab 7.0 and clear images of mitral valves were got.Condusions The adaptive seed-filling algorithm proposed in this paper and Contourlet transform are very useful for segmentation in dif-ferent situation.The experimental result in this paper proides useful reference for doctors in their research on the motion of mitral valves and planning on surrgical operation of mitTal valves.
3.Investigation on the relationship between dietary factors and higher prevalence of gastric cancer in residents of islands

Objective It is thought that environmental agents, especially the diet, play an important role in the processes of gastric cancer. The study was designed to investigate the relationship between dietary factors and the risk of gastric cancer in residents of islands so as to offer the evidence for effective measures of prevention and control. Methods A frequency matched design of case control study was used. 103 cases of gastric cancer newly diagnosed in 2001 and 133 controls were randomly selected among the residents of islands. And dietary factors were further investigated. Adjusted odds ratios (OR) and 95% confidence intervals (CI) of dietary factors on the processes of gastric cancer were estimated using unconditional logistic regression models. Results After the correction of the effects of some non diet factors, increased risks of gastric cancer were associated with hot diet (OR=1.48,95%CI:1.03 2.14) and pickled food (OR=2.63,95%CI:1.74 3.98). An inversed association with the risk of gastric cancer was seen in sauting (OR=0.33,95%CI:0.13 0.83 ), food eaten at regular time (OR=0.29,95%CI:0.19 0.44), allium vegetables (OR=0.43,95%CI:0.22 0.82) and carrot (OR=0.10,95%CI:0.02 0.41). Furthermore, PARc% of hot diet and pickled food were 11.8% and 57.8% respectively, which suggested that the carcinogenic effect of hot diet and pickled food on stomach in the population reached about 70% after the correction of the effect of other factors.Conclusions Pickled food and hot diet as the habits of residents in Zhoushan islands are important risk factors for gastric cancer, while decreased risk effect is found in sauting food, having regular dietary time and frequently eating allium vegetables and carrot.

5.Point-through-point acupuncture for treatment of peripheral facial paralysis.

OBJECTIVETo search for a more effective penetration needling method for peripheral facial paralysis.
METHODSThe patients were divided into a treatment group and a control group according to visiting sequence. The treatment group of 50 cases were treated with Yangbai (GB 14)-through-Touwei (ST 8), Yangbai (a little above the original point)-through-Shangxing (GV 23), Sizhukong (TE 23)-through-Yuyao (EX-HN 4), Qianzheng-through-Yingxiang (LI 20), Dicang (ST 4)-through-Jiache (ST 6) and Jiache-through-Yingxiang, at an angle about 45 degrees between the penetration needling direction and local paralysis muscle bundle, and the control group (n=45) with Yangbai-through-Yuyao, Zanzhu (BL 2)-through-Yuyao, Dicang-through-Jiache, Jiache-through-Dicang, Qianzheng-through-Dicang, Sibai (ST 2)-through-Yingxiang.
RESULTSThe cured rate and the cured and markedly effective rate were 76.0% and 92.0% in the treatment group, and 55.6% and 75.6% in the control group respectively, with significant differences between the two groups in the cured rate, the cured and markedly effective rate and the therapeutic times for cure.
CONCLUSIONThe penetration needling method at an angle about 45 degrees between the penetration needling and paralysis muscle bundle is more beneficial to recovery of facial paralysis.

7.Establishment and evaluation of methods for determinating cystic fibrosis transmembrane conductance regulator quantitatively.

OBJECTIVETo establish and evaluate a BA-ELISA method for the quantitative detection of cystic fibrosis transmembrane conductance regulator (CFTR) protein.
METHODSWe deliberately selected three tables of CFTR and made the synthetic peptide be expressed in E. coli, then used the antigen to immunize rabbits to obtain the anti-CFTR polyclonal serum. After that, 96 well plates were coated with the purified antibody against CFTR. The antigen CFTR which was extracted from human sperm was detected by anti-CFTR antibody labeled with biotin, horseradish peroxidase conjugated avidin, and the substrate. The concentrations of two kinds of antibodies and the experiment parameters were optimized. Thereby, the double antibody sandwich BA-ELISA method for the quantitative detection of CFTR protein was established. Furthermore, the reproducibility, specificity and so on were evaluated by clinical specimens of sperm.
RESULTSThe optimal concentration of coated anti-CFTR IgG was 4 µg/ml, while the biotin labeled anti-CFTR IgG was 10 µg/ml; the optimal blocking buffer was 1% BSA-PBST, the optimal time of the reaction between antigen and antibody was 60 min, the optimal chromogenic time was 15 min, the intra-assay and inter-assay coefficient were 2.16%-9.23% and 2.29%-11.71% respectively; The lowest detectable limit was 0.15 ng/ml; the standard curve had a good linear correlation of R2 = 0.962.
CONCLUSIONThe BA-ELISA method for the quantitative detection of CTFR protein is successfully established, and it is demonstrated that the method has strong specificity, high sensitivity and good reproducibility. It provides the basis and evidence of the further application of the method.

8.A new labdane diterpenoid glycoside from Nervilia fordii.

To study the chemical constituents of Nervilia fordii (Hance) Schltr., various chromatographic methods were used, including D101 macroporous resin, silica gel, ODS and preparative HPLC chromatographic techniques. A new labdane diterpenoid glycoside named as nervilifordoside A was isolated from 60% EtOH extract of Nerviliafordii. The structure of compound 1 was elucidated as 12, 17-epoxy-3-hydroxy-17-methoxy-labdan-13-en-16, 15-olide 3-O-alpha-rhamnopyranosyl-(1 --> 2)-O-beta-glucopyranoside on the basis of HR-MS, 1D and 2D NMR spectroscopic data as well as chemical methods.

9.Clinical significance of serum myocardial enzymes and CSF LDH detection in adults with intracranial infection

Objective To investigate the changes and clinical significance of serum lactate dehydrogenase (LDH),creatine kinase (CK),glutamate pyruvate transaminase (AST),and cerebrospinal fluid lactate dehydrogenase (CSF LDH) in adult patients with acute central nervous system infection.Methods The levels of myocardial enzymes (AST,LDH,and CK) in serum of 96 adult patients with acute intracranial infection in 7days and 39 healthy people were measured by Beckman automatic biochemical analyzer and enzyme rate assay,and CSF LDH level in 96 patients were measured simultaneously.Results (1) The serum myocardial enzymes (LDH,CK,and AST) of intracranial infection group (47 cases with viral encephalitis,30 cases with tuberculous meningitis,and 19 cases with purulent encephalitis) were significantly higher than those of normal control group (P <0.01).(2)The myocardial enzymes (LDH,and AST ) of patients with cerebral functional disorder were significantly higher than those of patients with normal cerebral function (P <0.05).(3)The levels of serum AST,LDH,and CK in the virus encephalitis group,serum AST and LDH in the purulent encephalitis group,and serum LDH in the tuberculous meningitis group were significantly higher than those in the control group (P < 0.01).The CSF LDH level in the viral meningitis group was prominently lower than that in the tuberculous encephalitis group and purulent encephalitis group,respectively (P <0.01).(4) No correlations were found between CSF LDH and serum myocardial enzymes (P >0.05).Conclusions (1)There is significant change in the levels of serum LDH,CK,AST,and CSF LDH of adult patients with acute intracranial infection,especially in infected patients with cerebral functional disorder,and the change of LDH is the most obvious.(2)The levels of serum myocardial enzymes and CSF LDH are helpful to the differential diagnosis of intracranial infection in early stage,and judging the severity of the illness.
10.Study on the sterols of Hericium erinaceus extracts

Purpose The aim is to compare the sterols in Hericium crinaceus ethanol extract and water extract from solid fermented hyphae and to study the pharmaceutical chemical basis of the different medicinal effects. Methods The nonsaponifiable lipids were isolated by saponification.The sterols were then detected by TLC and RP-HPLC.Results The content of sterols in ethanol extract was found to be higher than that in water extract.And one type of sterols from Hericium erinaceus hyphae was identified as ergosterol.Conclusion Due to ergosterol′s multifunction in biological activities,it may well be one of the active components of hyphae.And higher content of lipids, especially sterols may be one of the reasons for the better medicinal effect of ethanol extract than water extract.
