1.A Clinical Observation of Corneal Lacerations Treated with Therapeutic Soft Contact Lenses in Corneal Lacerations.

26 Eyes(34.1%) among 85 eyes of corneal lacerations who visited the Department of Ophthalmology of Masan Korye General hospital from 1991 to 1992 were fitted with therapeutic soft contact lenses instead of primary suture. The other 59 corneal lacerations had various causes that ruled out treatment with contact lens. 29 Eyes(51.8%) were associated with traumatic cataract, 23 eyes(41.1%) had associated iris prolapse and 7 eyes(12.5%) had intraocular bodies. Maximum size of corneal lacerations was about 5mm, and average duration of fitting with contact lens was 4.9 days. Complications included corneal opacity(24.0%), cataract(12.0%), retinal degeneration(4.0%), vitreous opacity(4.0%). We have found that therapeutic contact lens can be used successfully to treat corneal lacerations of less than 5mm, good apposition and alignment of wound edges, and abscence of associated injury to produce good restoration of the anterior chamber and visual acuity.

2.Recession of 4 Rectus Muscles for Neovascular Glaucoma or Absolute Glaucoma.

We have performed recession of all 4 rectus muscle in 8 eyes with neovascular or absolute glaucoma. The visual acuity of these eyes ranged fram no light perception to recognition of hand motion. As the purpose of surgery was to destroy all anterior ciliary arteries from the rectus muscles, after performing 2mm recessions on all 4 rectus muscles we applied cryotherapy on the oclera from he original rite to the mear insertion site. The cases were followed up for periods ranging from 3 to 19 months, with an average of 10.3 months. At the end of follow-up periods, we observed a statistically significant drop (p<0.05) in the intraocular pressure level, from the preoperative leveI of 49.8 +/- 7.9 mmHg (mean +/-SD) to 36.6 +/- 18.1 mmHg without any additional medical therapy. However, the decreases were not notable. In the treatment of neovascular glaucoma for absolute glaucoma, we have found that simultaneous disruption of all anterior ciliary arteries through anterior segment ischemia will not result in a satisfactory decrease in postoperative intraocular pressure.

3.A Case of Acute Posterior Multifocal Placoid Pigment Epitheliopathy, Associated with Bilateral Serous Retinal Detachment.

Acute posterior multifocal placoid pigment epitheliopathy(APMPPE) was first described by Gass in 1968. This syndrome is characterized by sudden loss of central vision secondary to multifocal yellowish-white placoid lesion of posterior pole at the level of the pigment epithelium and significant visual improvement after spontaneous resolution within a few weeks. In the early arterial and arteriovenous phase of fluorescein angiography, choroidal fluorescence is blocked by the placoid lesion but becomes hyperfluorescent in the late venous phase due to staining of the lesion. We experienced and treated a case of acute posterior multifocal placoid pigment epitheliopathy associated with bilateral serous retinal detachment in 29 years old male patient, who had a good visual improvement.

5.A Case of Hellermann-Streiff Syndrome.

Hallermann-Streiff Syndrome is a rare congenital anomaly characterized by dyscephalia, dental anomalies, hypotrichosis, congenital cataract, bilateral microphthalmia and has characteristic feature of bird face due to marked mandibular hypoplasia and beaked nose. The authors have experienced a case of Hallermann-Streiff Syndrome combined with bilateral aphakia and glaucoma in a 34 years old woman and we report this case with the review of literatures.

Sturge-Weber syndrome is a congenital disorder characterized by facial nevus flammeus along the distribution of the trigeminal nerve, ipsilatal leptomeningioma, buphthalmos, and choroidal angioma. The Authors experienced a case of Sturge-Weber syndrome in 9 year old female patient and glaucoma was controlled by trabecuectomy without any complications.

7.A Clinical Study of Pediatric Ocular Injuries through Emergency Room.

The characteristics of ocular injury is that the extent of visual disturbance is variable and serious in spite of inconsiderable trauma. Moreover, the prognosis of ocular injury in infancy and childhood is poorer than that in adulthood because the visual funjction of infants and children has not completely matured yet. And the visual disturbance will influence unfavorably upon the selection of occupation in future. Therefore, further investigation of pediatric ocular injuries is thought to be necessary for the establishment of measures of treatment and prevention. The authors clinically analysed 161 eyes of 152 pediatric ocular injurise under the age of 15 year-old who visited emergency room of Masan Koryo General Hospital during the 3 years from January 1, 1986 to December 31, 1988. The ambulation rate of pediatric ocular injuries was 7.5% of total eye patients through emergency room and the incidence was higher in male. The most common disease of all pediatric ocular injuries was lid laceration, followed by subconjunctival hemorrhage and traumatic hyphema. It was revealed that the age group from 4 to 6 was the most hazardous stage for pediatric ocular injuries and a pierced wound by sharp pointed materials was the most harmful and the most dangerous mode of injury.
