1.A Clinical Analysis of the Ankle Fractures

The ankle is a complex, weight bearing joint and its injury result in a severe functional disturbance of complex anatomical characteristics. It is very important to understand the mechanism of trauma in order to make definite diagnosis and proper treatment. The authors analysed 52 cases(52 patients) of the ankle fractures which were admitted and treated in Orthopaedic department, Korea University Hospital from Sept. 1980 to Dec. 1986. The results obtained were as follows : l. Of the 52 cases, male was 32 cases(61.5%), female was 20 cases(38.5%) and average age of the patients was 32.9 years. 2. The causes of the injury were slipping down, traffie accident, falling down and sports injuries, in orders. 3. Open fractures were 8 cases(15.4%) and closed fractures were 44 cases(84.6%). The most common type of the ankle fracture was supination-extenal rotation type(18 cases, 34.1%) and next was pronation-external rotation type(15 cases, 30%), according to the classification of Lauge-Hansen. 4. 38 cases(73.1%) were treated by open reduction and 14 cases(26.9%) by closed reduction, and average duration of immobilization was 7.7 weeks in open reduction and 10.4 weeks in closed reduction. 5. The associated fractures other than ankle was 15 cases(28.8%). 6. The better results were obtained from open reduction and the worst results obtained from pronation-dorsiflexion type, according to the criteria of Meyer. 7. The complication was 7 cases(13.5%), traumatic arthritis 4 cases, malunion 2 cases and nonunion 1 case. 8. Accurate reduction and rigid internal fixation of the fractured lateral malleolus, distal fibular and distal tibiofibular diastasis was important in treatment of the ankle fracture.

3.Anesthetic Management of a Patient with Toxic Epidermal Necrolysis: A case report.

Toxic epidermal necrolysis (TEN) is one of the most dramatic, life-threatening cutaneous drug reaction. TEN is characterized by extensive detachment of the epidermis due to full-thickness necrosis. Because of extensive epidermal loss, these patients are vulnerable to infection, fluid loss, electrolyte imbalance, and heat loss. Anesthesiologist should attempt to prevent further skin damage and reduce complications such as pulmonary edema, renal failure, and sepsis. A 30-year-old male patient was admitted with fever and myalgia. 1 day after admission, scale, erosion, ulceration, eruption on skin and mucosa were developed. Symptoms progressed to TEN after 5 days of admission. 6 days after admission, he developed panperitonitis due to duodenal ulcer perforation, and underwent laparotomy under general anesthesia. We report a case of a patient with TEN who received general anesthesia for laparotomy due to panperitonitis.

Among 80 types of yeast isolated from wild flowers in Daejeon, Korea, two species that have not yet been identified by phylogenetic analysis of the internal transcribed spacer-2 (ITS2) genes and 26S rDNA sequences were identified as Candida sp. 44-C-1 and Cryptococcus sp. 9-D-1. Neither of the newly identified species formed ascospores, while Candida sp. 44-C-1 formed pseudomycelium and Cryptococcus sp. 9-D-1 did not.

5.A Case Study for the Reconstruction of Crime Scene by Computer Simulation.

Especially in design of automobile, there are many professional softwares based finite element or multi-body like Madymo, Pam-Crash, Ansys. These program have been released almost every car maker and university. Also in accident reconstruction fields, there are some softwares like PC-Crash, HVE was developed. Unfortunately these programs for accident reconstruction are so general and its input data are so limited, it is very difficult or even impossible to analyze some special cases. In this study, we reconstruct two crime scenes using Madymo program. Although this program was developed for design of car, we apply to crime scene reconstruction. First case is regarding traffic accident. A car was fell under 5m from the road and turn over and then move about 40m. Finally the driver eject from the car. Second case is regarding suicide by hanging. When the police investigate the crime scene at first time, they assume that this crime is a murder case but we carried out autopsy and investigated the crime scene again and analyzed by computer simulation and finally find out this crime is suicide case not murder.
