1.Comparison of different methods for detecting mycoplasma pneumoniae

Objective To investigate the sensitivity and specificity of different methods for the detection of mycoplasma pneu‐moniae .Methods The serum and throat swab specimens were collected from 199 pneumonia patients .ELISA and real‐time fluores‐cence quantitative polymerase chain reaction (FQ‐PCR) assays were used to detect the serum mycoplasma pneumoniae IgM antibod‐y and DNA .Results Among 199 pneumonia patients ,61 cases were diagnosed as mycoplasma pneumoniae pneumonia according to the diagnosis standard .The sensitivity and specificity of ELISA and FQ‐PCR for detecting mycoplasma pneumoniae were 86 .9%and 96 .7% ,and 78 .3% and 97 .1% ,respectively .Conclusion The sensitivity and specificity of FQ‐PCR for detecting mycoplasma pneumoniae are higher than those of ELISA .
2.Risk Factors of Nosocomial Infections Caused by Vancomycin-resistant Enterococcus

OBJECTIVE To investigate the risk factors for vancomycin-resistant Enterococcus(VRE) infections and their effective isolation measures.METHODS The data of 21 cases of VRE nosocomial infections were analyzed from Jul 2003 to Dec 2005 in Beijing Chaoyang Hospital;28 cases of antibiotic-sensitive Enterococcus infection were randomized as control.T test,chi-square test and Logistic regression analysis were used for statistics.Strict measures were taken to all of the VRE infected patients.RESULTS According to univariate analysis,the factors associated with the development of VRE nosocomial infection were age,in ICU,accepted invasive operation,Acute Physiology and Chronic Health Evaluation Ⅱ(APACHEⅡ),live in hospital more than 30 days,co-infection with other pathogens,and fluoroquinolone and vancomycin/norvancomycin use 15 days before isolation of VRE.Multivariate Logistic regression analysis identified two independent factors: accepted invasive operation and previous vancomycin/norvancomycin use.Spreading of VRE had not occurred.CONCLUSIONS Accepted invasive operation and previous vancomycin/norvancomycin use are independent risk factors for VRE infection.Effective measures can prevent the spread of VRE.
3.A natural pancreatic 3D bioscaffold platform for in vitro study on exocrine pancreas

Objective To evaluate the biochemical function of rat nature three-dimensional acellular pancreatic bioscaffold (3D-APB) in promoting cell proliferation and differentiation in vitro.Methods The fresh pancreas from 10 rats were perfused to prepare 3D-APB.The biocompatibility of 3D-APB was eva luated.The experiment was divided into 4 groups based on 4 kinds of three-dimensional media for AR42J culture,including blank control group,ECM group,PLGA group and 3D-APB group.We compared the proliferation and differentiation of AR42J pancreatic acinar cell at 3 days,5 days,7 days and 10 days after seeding among 4 groups.Results The 3D-APB could represent a biocompatible scaffold capable of integrating within host tissue.The proliferation rate of AR42J cells by MTT in 3D-APB was higher,while the apoptosis rate by flow cytometry was lower than those in other 3 media,which were all significantly different (P < 0.05),respectively.The protein expression of PDX-1 and PTF-1 by western blot in 3D-APB group was greatly higher than those in other 3 groups (both P < 0.05).The mRNA expression of PDX-1 and PTF-1 through qRT-PCR was significantly higher than those in other 3 groups (both P < 0.05).Conclusion Compared with the commonly used chemical and natural scaffold at present,3D-APB could promote cell proliferation and differentiation,which was more appropriate for regenerative medicine.
4.Effects of endogenous sulfur dioxide on the oxidative stress induced by cobalt chloride in the rat pulmonary artery smooth muscle cells

Objective To investigate the effects of endogenous sulfur dioxide (SO2) on the oxidative stress induced by cobalt chloride (CoCl2) in the rat pulmonary artery smooth muscle cells (PASMCs).Methods Rat PASMCs were treated with 200 μ mol/L CoCl2 to mimic the hypoxia insult.Endogenous SO2 generating enzyme aspartate aminotransferase 1 (AAT1) expression was upregulated or downregulated (AAT1 sh) by transfection with lentivirus.Rat PASMCs were randomly divided into 8 groups:vehicle group,vehicle + CoCl2 group,AAT1 group,AAT1 + CoCl2 group,scramble group,scramble + SO2 group,AAT1 sh group and AAT1 sh + SO2 group.SO2 donor Na2 SO3/NaHSO3 at concentration of 100 μ mol/L were added in scramble + SO2 group and AAT1sh + SO2 group.The expressions of AAT1,superoxide dismutase 1 (SOD1) and SOD2 in PASMCs were detected by Western blot method.In situ SO2 content in PASMCs was detected by fluorescent probe.The superoxide anions in PASMCs were labeled by dihydroethidium (DHE) probe under fluorescent microscope.Results Compared with the vehicle group,the levels of SO2 and the expressions of AAT1 (0.221 ± 0.002 vs.0.446 ± 0.004),SOD1 (0.076 ± 0.028 vs.0.171 ± 0.019) and SOD2 (0.080 ± 0.031 vs.0.196 ± 0.018) significantly decreased (all P < 0.01),and superoxide anion increased in rat PASMCs of vehicle + CoCl2 group.Meanwhile,compared with vehicle + CoCl2 group,the levels of SO2 and the expressions of AAT1 (0.839 ± 0.056 vs.0.221 ± 0.002),SOD1 (0.177 ± 0.020 vs.0.076 ± 0.028) and SOD2 (0.195 ±0.018 vs.0.080-± 0.031) markedly increased (all P < 0.01),and superoxide anion decreased in rat PASMCs of AAT1 + CoCl2 group.On the contrary,compared with the scramble group,the levels of SO2 and the expressions of AAT1 (0.062 ±0.017 vs.0.354 ±0.034),SOD1 (0.054 ±0.029 vs.0.157 ±0.023) and SOD2(0.180 ±0.100 vs.0.586 ± 0.176)significantly decreased (all P < 0.01),and superoxide anion increased in rat PASMCs of AAT1sh group.Furthermore,compared with the AAT1 sh group,the levels of SO2 and the expressions of SOD1 (0.155 ± 0.022vs.0.054 ± 0.029) and SOD2 (0.578 ± 0.200 vs.0.180 ± 0.100) significantly increased (all P < 0.01),and superoxide anion decreased in rats PASMCs of AAT1sh + SO2 group.Conclusion Endogenous SO2/AAT1 inhibits CoCl2-induced oxidative stress in rat PASMCs.

7.Isolation and differentiation of stem cells derived from human placenta into cardiomyocytes

AIM: To isolate the stem cells from human placenta and induce them into cardiomyocytes. METHODS: Stem cells were isolated from human placenta and characterized by morphologic analysis. The “hanging drop” methods were used to inducte stem cells into cardiomyocytes. The expressions of atrial natriuretic factor(Anf) and ?-myosin heavy chain(?-MHC) genes were analyzed by RT-PCR. RESULTS: Stem cells derived from human placenta were self-renewed and differentiated into cardiomyocytes in vitro. RT-PCR showed that Anf and ?-MHC genes were expressed in the “beating cells”.CONCLUSION: Human placenta is an abundant source of stem cells.
8.Laparoscopic Roux-en-Y gastric bypass surgery improves quality of life for overweight type 2 diabetes mellitus patients

Objective To investigate the change in quality of life (QoL) following laparoscopic Roux-en-Y gastric bypass (LRYGB) for T2DM with obesity.Methods A total of 46 overweight T2DM cases (mean baseline BMI (32.7 ± 3.9) kg/m2) undergoing LRYGB were followed by QoL questionnaires (SF-36) before and after 6 and 12 months.Results The preoperative physical QoL scales (physical component summary,PCS) was significantly different from the general population(GP) scales (P < 0.01),and the improvement in QoL was significant at 6 months postoperative and continued to consistently increase up to 12 months (P < 0.01).For mental component summary (MCS),the preoperative scales was not significantly different from the GP scales,however,the 6 and 12 postoperative scales improved significantly (P <0.01).As for physical function,role physical,role emotion,the 6 (78 ± 8,76 ± 17,70 ± 23) and 12 months' (85 ± 7,82 ± 18,77 ± 18) postoperative scales were markedly higher than those in the preoperative controls (P < 0.01),and there were no differences between the 12 month's postoperative scales and GP scales.The preoperative bodily pain,general health and social function (66 ± 14,55 ± 12,63 ±15) was different when compared with (76 ± 12,75 ±6,75 ± 13) at 6 months and (82 ±9,79 ±6,94 ±9) at 12 months (P <0.01).The preopertive mental health (71 ± 12) was not significantly different from GP scales,nor there were significant difference when compared with (71 ± 10) at 6 months and (73 ±8) at 12 months.Conclusions There were improvements in physical and mental QoL in T2DM patients with obesity after LRYGB.
9.The interventional treatment for shunt stenosis or occlusion after mesocaval shunts in portal hypertension

Objective To evaluate interventional therapy in the treatment of interposition graft stenosis or occlusion after mesocaval shunts.Methods The clinical data of 19 cases of artificial vessel stenosis or occlusion after mesocaval shunts for portal hypertension at our department from march 2009 to march 2012 were retrospectively analyzed.Results In the 19 cases with artificial vessels stenosis or occlusion developed after mesocaval interposition shunts for portal hypertension,there were 5 cases in which acute thrombosis occurred within a week after the surgery.Catheter directed thrombolysis was successfully conducted.In 6 cases in which shunt stenosis developing 1 to 8 years after surgery were managed by balloon dilatation or stent angioplasty successfully.The shunt graft occlusion occurred in 8 cases after 1 to 4 years of surgery was successfully managed and the shunt was reopened by balloon dilatation or stent angioplasty in 6 cases,and in 2 the procedure was failed for the guide wire can't go through the anastomotic site of artificial vessel-superior mesenteric vein.In 11 cases embolization of the esophagogastric varices was successfully carried out for postoperative standard anticoagulation.During a period of 3 months to 3 years follow-up,stenosis recurred 1 year after balloon dilatation in one case,and stenosis was managed by angioplasty successfully.Conclusions Interventional radiological techniques by percutaneous puncture through femoral vein-inferior vena cava-artificial vessel-portal vein (including catheter directed thrombolysis,balloon dilatation,stent placement,etc) are less traumatic,highly successful in the treatment of shunt stenosis or occlusion after mesocaval shunts in portal hypertension.
10.Correlations between HIV-1 subtypes and changes in CD4+T cell count over time

Objective To understand the correlations between HIV-1 subtypes and changes in CD4+T cell count over time in patients with different subtypes of HIV-1 infection.Methods A total of 94 patients who were diagnosed with HIV-1 infection in Nanjing and received at least twice CD4+T cell counting test before antiretroviral therapy (ART) were recruited in this study.Descriptive analysis was used to present the rates of CD4+T cell decline for different HIV-1 subtypes.Logistic regression analysis and nonparametric test were conducted to investigate the factors responsible for CD4+T cell decline and to analyze the correlations between the rates of CD4+T cell decline and HIV-1 subtypes.Results The median monthly rate of CD4+T cell decline was-2.20 [interquartile range (IQR):-11.36-2.13] cell/μl.Of all patients,25.5% (24/94) had a significant decline (≥30%) in CD4+T cell count.Compared with the patients infected with CRF01_AE,those infected with CRF07_BC (OR=0.28,95%CI: 1.7-6.5) or other subtypes (OR=0.16,95%CI: 1.0-2.9) had a lower risk of significant decline in CD4+T cell count.In addition,results of the nonparametric test showed that the patients infected with CRF01_AE (M=-21.54,IQR:-30.97——11.92 cell/μl) had a faster CD4+T cell loss than those infected with CRF07_BC (M=-11.26,IQR:-14.06——5.63 cell/μl) (P=0.033).Conclusion HIV-1 subtype is associated with the rate of CD4+T cell decline.It is important to monitor the changes in CD4+T cell count in patients infected with CRF01_AE and to carry out timely ART.
