3.A Study on Relation betwen Red Cell Electrolyte Concentractions and Serum Digoxin Level.

For the purpose of elucidating relation between red cell electrolyte concentrations and serum digoxin level, measurement of red cell electrolyte concentrations and seum digoxin level by radioimmunoassay were done in 46 normal controls and 63 patients of CHF including 8 patients with digitalis intoxication. The results obtained were as follows: 1. Red cell sodium concentration and ratio of red cell sodium to red cell potassium concentration in normal males were significantly higher than those of females, and red cell potassium concentration in normal males was significantly lower than that of normal females. 2. Red cell potassium concentration in patients of CHF was significantly higher than that of normal controls, but there was no difference in the red cell sodium concentration between the groups. 3. There was no relation between red cell electrolyte concentrations and serum digoxin level, and red cell electrolyte concentration did not exactly estimate serum digoxin level. 4. Red cell electrolyte concentrations in the patient of CHF were valuable in detecting digitalis toxicity.

4.Lacrimal canalicular reconstruction with various materials.

This retrospective clinical study consists of 27 patients with lacrimal canalicular injury caused by various accidents and which were reconstructed with various materials in the department of plastic surgery at Catholic University Medical College from December 1987 to July 1996. These 27 patients were followed up at least 6 months after the canalicular reconstruction and their medical records were reviewed and analysed retrospectively in order to obtain the clinical pattern and understand the therapeutic results. The statistical items were the age and sex distribution, the causes of injury, the prevalent site of injury, the reconstruction materials, the duration of intubation and the results of treatment. The following results are obtained: 1. Males were more dominant than females by 4.4 : 1. 2. The patient age was from 6 to 73 years old and the prevalent age groups were the third and fourth decades(55.5%). 3. The most common cause of canalicular injury was the violence(29.6%) and was followed by traffic accident. 4. The left canaliculi, especially inferior canaliculi, was the prevalent injury site. 5. Regardless of the materials(P.V.C. tube, nylon and silicone tube) used in canalicular reconstruction, the longer duration of intubation was obtained the better result of treatment. 6. So the silicone tube with stainless steel(C-line canaliculus intubation set) was relatively inert and less complicating, it could be maintained more longer and had better results than other materials such as P.V.C. tube and nylon.

8.Rectangular Rod Instrumentation in Spondylolisthesis

Since the rectangular rod had been used by Eduardo R. Luque in 1979, many authors reported good results by this technique in low back arthrodesis. The advantages of this device are that the technique is relatively easy, the cost is cheaper than the other devices, and no special instruments are needed for application of this device. On the other hand, it has the disadvantage of possible neurologic damage while passing sublaminar wires. The rectangular rod functions fundamentally as a tension band that although it is strong in flexion, it is relatively weak in lateral bending or axial loading on the spine. From June, 1984 to June, 1988, the authors experienced 41 cases of rectangular rod instrumentation with sublaminar wiring in various low back conditions. And 30 cases of spondylolisthesis which could be followed for more than one year were analyzed. And the results obtained were as follows. l. Of 30 patients of spondylolisthesis, 7 cases(23.3%) were male and 23 cases(76.7%) were female. 2. 21 cases(70.0%) were isthmic type and 9 cases(30.0%) degenerative type. 3. 23 cases(76.7%) were in Grade I, 6 cases(20.0%) in Grade II and 1 cases(3.3%) in Grade III, according to Meyerding's classification. 4. At follow up examination, 18 cases(60.0%) were in excellent group, 8 cases(26.6%) in good, 2 cases(6.7%) in fair, according to Gill's criteria. Two cases(6.7%) of pseudoarthrosis were noted and these cases were classified in unsatisfactory group. 5. Rectangular rod with sublaminar wiring was not a good device for reduction of spondylolisthesis

9.Clinical Application of Hepatic Arterial Embolization

Transcatheter arterial embolization was applied in 7 cases of hepatic disease including hemobilia of various causes and malignant tumors at Department of Radiology, Seoul National University Hospital during recent several years. The embolic materials were autologous blood clot and Gelfoam particle. Successful control of bleeding or devascularization of tumor vessel was made in 6 of the 7 patients. There was no serious complications; however, post embolization syndrome was found including pain, fever, nausea and vomiting in all cases and subsided after a few days. Transcatheter hepatic arterial embolization is suggested to be a safe and effective treatment in control of hemobilia and devascularization of malignant hepatic tumors.

10.Radiological evaluation of traumatic spinal fracture in computerized tomography

We had a retrospective study for taumatic fracture of spines with simple X-ray and CT. During the period of 2years from June 1983, the radiological and clinical evaluation had been made on 36 patients suffered from traumatic fracture of spines which were confiremd by the radilgocial examination. The results were as follows; 1.Among 36 patients, single spinal injury was 26 cases(72.2%), multiple level injuries was 10 cases(27.7%). 2. Levelof spinal injuries were as follows; Most frequent site was thoracolumbar junction (T11-L2) in 19 cases(52.7%), andlower cervical spine(C3-C7) in 9 cases(25%). 3. Simple fracture was in 15 cases(41.6%), burst fractures was in 21cases(58%). Neurologic symptoms were appeared in 11 cases(52%) in 21 cases of burst fractures. 4. Manifestationsof spinal canal narrowing: 2 of 15 cases in simple fracture, all of 21 cases of burst fractures,and we were foundexactly posterior element fractures with CT, which were difficult to detect with simple X-ray film. 5. Multiplanar reconstruction images were more favorable to detect the distraction of facet joint, fracture through the vertebralendplate, subluxation and kyphosis, which were difficult to detect with axial CT images.
