From May, 1971 to June, 1984, we performed extensive saucerization on the chronic osteomyelitis of long bones eradicating all pathologic foci; not only sequestra but sclerotic involocurum and necrotic original cortex embeded by new bone, which was different from the conventional methods such as sequestrectomy or guttering in its extensiveness. Clinical analysis on the base of radiologic evaluation after saucerization was done on the 16 patients; for average 5 years of follow-up. The results obtained were as follows; 1. The angulatory deformity of the long bones after pathologic fracture complicated by the chronic osteomyelitis was not corrected satisfactorily due to the bony sclerosis or hyperostosis at the fracture site, especially in cases of posterior or medial angulation. 2. The recurrence was closely related to the persisting non-sequestered original cortical lesion after incomplete saucerization, which was embeded by the new bone. So to prevent the recurrence the non-sequestered original cortical lesion must be removed completely. 3. The cortical defect after saucerization was restored completely in patients under the age of 14, but which was not the way in patients over the age of 16.

2.Experience of Transvaginal Bladder Neck Suspension(Raz Procedure) for Stress Urinary Incontinence: 10 Cases.

Transvaginal bladder neck suspension by Raz is an acceptable procedure for the treatment of stress urinary incontinence with high cure rate, operative simplicity, short hospitalization and little morbidity. Recently we experienced 8 cases of stress urinary incontinence treated with Raz's procedure and 2 cases with coexisting cystocele were treated with 4-corner operation. All cases have been assessed prior to and following operation and the results were satisfactory. Herein, we report 10 cases of stress urinary incontinence treated by Raz's procedure and 4-corner operation with review of literatures.

3.Clinical Observation on the Gonorrhea and Non-specific Urethritis.

During the period from Jan. 1978 to June 1978, clinical and laboratory observations were done on the 46 cases of gonorrhea and 40 cases of non-specific urethritis at random selection in the Department of Urology, Korea University Hospital. The following results were obtained: 1. The average age of the gonorrheal patients were 24 years old and 30 years in non-specific urethritis patients. 2. The main clinical symptoms of gonorrheal patients were profuse yellowish urethral discharge found in 91.3 % of cases and painful urination in 37 %. The symptoms of non-specific urethritis patients showed scanty urethral discharge in 525 % of cases and urethral discomfort in 25%. 3. The incubation period of gonorrheal cases was 3 to 7 days while in one-fourth of non-specific urethritis the period was shorter than 10 days. 4. In the past history 43.5 % of gonorrheal patients had V. D. and 62.5 % of non-specific urethritis patients had them. 5. In 20 cases ( 50%) of non-specific urethritis had positive shred culture with no evidence of positive findings in urinalysis or in Gram stain of urine sediment. 6. In 31 cases(77.5%) of positive shred cultures, staphy lococcus albus were grown in 12 cases, diphtheroid in 10 cases, mixed growth of staphylococcus albus and diphtheroid in 5 cases. Antimicrobial sensitivity test with Gram(+) cocci obtained from 19 cases of nonspecific urethritis showed highly sensitive resultes to hentamicin, lincomycin, carbenicillin, novobiocin and highly resistant reactions to oxytetracycline. 7. 82.6% of cases of gonococcal infection were cultured on the Thayer-Martin medium and antimicrobial sensitivity test was done with these colonies by disc diffusion method. Penicillin and its derivatives were highly sensitive, other antibiotics such as erythromycin, oxytetracycline, chloramphenicol and gentamicin were also sensitive.

4.THE EFFECT OF COLLAGEN SUBSTRATE IN CULTURE MEDIUM ON DNA AND PROTEIN SYNTHESIS OF DERMAL FIBROBLASTS.

Collagen is the major component of scar tissue. Considerable progress of fibroblast growth kinetics and of collagen synthesis has been achieved in the past decade. We have been interested in fibroblasts activities as they are expressed by cells cultured in collagen substrate. This study is to examine the effects of collagen substrate and peptide growth factors In culture medium on DNA and protein synthesis of human dermal fibroblasts. Collagen, interleukin-1(IL-1) and transforming growth factor-beta(TGF-beta) were added to fibroblast culture media according to the designed experiment model and DNA and protein synthesis were measured by [3H]-thymidine, [3H]-leucine, and [3H]-proline incorporation method. The morphological features of fibroblasts were observed by light microscope. The results were as follows ; 1) There were significant decreases of DNA and protein synthesis of cultured fibroblasts in the presence of collagen substrate compared with those in Control groups(p<0.01). 2) DNA and protein synthesis were decreased as dose dependant manner of collagen density in culture media. 3) Morphological features of fibroblasts became less stellate and flat, more spindle-like in the presence of collagen. 4) In responsiveness to IL-1, collagen non-treated groups responded to IL-1 but collagen treated groups were unresponsive to IL-1 (P<0.05). 5) Cells In collagen non-treated groups responded to TGF-beta as dose-related manner(P<0.01). Collagen treated groups desponded to TGF-beta but did not show TGF-beta dose-dependant relationship. In Conclusion, collagen substrate in the culture medium could lower the DNA and protein synthesis of fibroblasts. Cells in collagen substrate were unresponsive or less responsive to peptide growth factors than those in non-collagen substrate.

5.The Urine Cytology on the Genitourinary Tumors.

Urinary cytology is rather simple diagnostic procedure for the detection of the urinary tract tumors, but has not yet been fully accepted by urologists. Since publication of the work in urinary cytology by Papanicolaou and Marshall in 1945, many studies with urine sediment smears have been reported. To evaluate the results of cytologic examination of the genitourinary tumors, we have compared with the results of biopsies which were performed at the Department of Urology, Korea University Hospital, during the period from April 1972 to September 1979. One hundred fifty patients were subjected to this examination, of which 81 cases were diagnosed as the genitourinary tumors histopathologically, while 69 cases were negative histologically. In 81 cases, 50 cases (61.7%) were negative in urinary, 9 cases (11.1%) doubtful and 22 cases(27.2%) positive.

6.Analysis of angiographic findings in cerebral arteriovenous malformations: Correlation with hemorrhage.

Intracerebral hemorrhage is the most serious complication of cerebral arteriovenous malformations (AVM). To identify angiographic characteristics of AVM which correlate with a history of hemorrhage, we retrospectively analyzed angiographic findings of 25 patients with AVM. Nine characteristics were evaluated; these include nidus size, location, arterial aneurysm, intranidal aneurysm, angiomatous change, venous drainage pattern, venous stenosis, delayed drainage and venous ectasia. These characteristics were correlated with hemorrhage, which was seen in 18 (72%) patients of CT or MR images. Venous stenosis (P<.05) and delaved venous drainage (P<.05) well correlated with a history of hemorrhage. Arterial aneurysm and intranidal aneurysm also had a tendency hemorrhage although they did not prove to be statistically significant. Detailed analysis of angiographic finding of AVM is important for recognition of characteristics which are related to hemorrhage and may contribute to establishing a prognosis and treatment planning.

7.A Convenient Device of Circumcision.

The beginning of the circumcision was originated from the rite of the ancient Israel historically. The circumcision has performed for the purpose of penile hygiene and for the treatment of phimosis, paraphimosis, balancposthitis and so on. In the way of the circumcision the conventional circumcision was mostly used, but we used Gomco clamp and the incision was made 3mm away from the metal edge of the instrument. And the merits in the use of the instruments were as follows: 1) the short time operation, 2) safety and cosmetically satisfactory performance without need of assistant for the procedure, 3) no bleeding on operation.

8.The Effect of Topical Rebamipide 2% in Managing Dry Eye Following Cataract Surgery

Purpose:
To evaluate the potential advantages of using topical rebamipide 2% to manage dry eye following cataract surgery.
Methods:
Patients who underwent cataract surgery (phacoemulsification and intraocular lens insertion) were evaluated for dry eye 1 week postoperatively. Randomly selected patients were divided into the rebamipide group, who received 2% rebamipide eye drops, and the control group, who did not receive rebamipide. Various dry eye-related tests, including Schirmer’s test, the tear break-up time (TBUT), ocular surface staining, tear film osmolarity, tear lipid layer thickness, and the ocular surface disease index (OSDI) were performed in both groups 1 week, and 1 and 2 months postoperatively.
Results:
In the rebamipide group, we observed significant improvement in TBUT, the ocular surface staining score, and OSDI at 2 months postoperatively, as compared to 1 week (p = 0.002, 0.014, and 0.013, respectively). Schirmer’s test, tear film osmolarity, and tear lipid layer thickness remained unchanged in the rebamipide group. Throughout the entire follow-up period, the rebamipide group had a significantly prolonged TBUT (1 month: p = 0.007, 2 months: p = 0.000), and lower ocular surface staining score (1 month: p = 0.000, 2 months: p = 0.000), as compared to the control.
Conclusions
Following cataract surgery, the use of rebamipide eye drops produced improvements in TBUT, ocular surface staining scores, and OSDI. This suggests that rebamipide offers benefits in managing dry eye that can occur after cataract surgery.
