1.A Case of Fibrous Histiocytoma in Cornea and Corneosclerallimbus.

Fibrous histiocytoma are complex group of tumors that feature cells resembling fibroblasts and histiocytes.In a case of fibrous histiocytoma involving the cornea and corneolimbus, we evaluated the finding of H-E, Masson-trichrome, reticulin, CD68, alpha-1-antitrypsin antibody and smooth muscle actin antibody stain after the localized excision in 26 years old male patient. In H-E stain, tumor cell included many multinucleated giant cells having abundant eosinophilic and foamy cytoplasm infiltreated by numerous lymphocyte, CD68 & alpha-1-antit-rypsin antibody staining resulted in diffuusely positive finding. Therefore examination. Good result was noted after complete excision at 6 month follow-up.

2.A Case of Choroidal Malignant Melanoma.

The patient, 54-year-old female, visited our department of ophthalmology because of decreased visual acuity of left eye. The light brown to gray colored mass was visible through clear ocular media, and extensive serous detaclunent of adjacent retina was noticed. Ultrasonograpy showed large smooth smooth ecogenic density extending into vitreous cavity and occupying most of vitreous cavity. Enucleation was performed. The histopatholgical examination revealed malignant choroidal melanoma composed of spindle B type cells and epithelioid cells. Extrascleral invasion was not found.

3.Detection of Antibodies against Shope Fibroma Virus and Formation of Fibroma in the Korean Domestic Rabbits.

In our preliminary study to find antiviral or antitumor agents from Korean natural products, we found that the Shope fibroma virus (SFV) induced fibromas reaching maximum size at 5~6 days with spontaneous disappearance at 15~20 days after SFV intracutaneous inoculation into Korean domestic rabbits. However, the sizes of fibromas of rabbits at day 5 after virus inoculation were significantly different individually. Assuming that the variation of tumor size was due to either susceptibility or the preexisting antibodies against SFV in the Korean domestic rabbits, the rabbits were checked for the antibodies against SFV by IFAT using SFV infected RKl3 cells. The antibody positive rate of normal Korean domestic rabbits was 32.8% and the sizes of the fibromas of the positive rabbits were significantly smaller than those of negative rabbits (p<0.0001). The fibroma sizes were dependent on the antibody titers of rabbits to SFV. The sizes of fibromas after inoculation of SFV into immunized rabbits were about one tenth of those by the first inoculation into normal rabbits. This is the first report on the antibody prevalence against SFV among normal Korean domestic rabbits and it suggest the existence of a wild fibroma virus or related virus in Korea.

4.A Case of Intraspinal Epidermoid Tumor Iatrogenically Occurred Following Previous Lumbar Punctures.

Epidermoid tumors rarely occur in the central nervous system. Their incidence among intraspinal canal tumors is even less than 1% in most large series. Epidermoid tumors are generally understood as embryogenic or congenital tumor, however the etiologic importance of previous lumbar punctures in the formation of epidermoids has been also suggested by some papers, and almost confirmed by an extensive literature review and an animal experiment. Recently we have experienced a case of intraspinal epidermoid, which occurred at the same site as a lumbar puncture had been performed to carry out lumbar myelogram. To our knowledge, such a case has never been reported in a scientific journal in Korea yet. In this report we emphasize the importance of using a spinal puncture needle with a stylet in spinal taps, and of considering a lesion in the thoraco-lumbar junction on performing lumbar spine CT scan even in a patient only with lumbar disc symptoms.

5.Three Cases of Dieulafoy's Disease.

Dieulafoys lesion consists of abnormally large gastric submucosal artery which ruptures into the stomach causing massive or recurrent intragikstric bleeding. The lesion is very small and easily overlooked even at laparatomy and aan only be correctly diagnosed by endoscopy or arteriography if the patient is actively bleeding. Three patients who were admitted with bleeding of upper gastrointestinal tract and eventually diagnosed as having Dieulafoys lesions were analysed. All were men with age range of 44 to 55 years. All patient were asymptomatic before presenting with hematemesis. Two of the three patients had had history of upper Gl bleeding. One patient used analgesics daily for ureteral colic and two patient drank alcohol excessively. Gastroscopy was performed during the bleeding episode in all three patients. Dieulafoy's lesion was seen in all three cases and in the second case, there was concomittent diffuse petechia in the whole stomach. The lesion was situated on the posterior wall of upper body in one, on anterior wall of upper body in another, lesser curvature side of gastric fundus in the other case. All three patient underwent laparotomy for persistent bleeding and the lesion was suture ligated only in two patients while in one patient vagotomy and pyloroplasty was added. Resection biopsy was performed in two cases and both revealed only normal gastric mucosa. All patients discharged after complete recover.

6.Changes of Intraocular Pressure after Implantation of the Posterior Chamber Lenses.

Preoperatively normotensive 211 cases of extracapsular cataract extraction (ECCE) and posterior chamber lens implantation in the capsular bag had intraocular pressure (IOP) measurements recorded 3 months postoperatively. Analysing the data of IOP level, surgical technique (planned extracasular cataract extraction or Kelman phacoemulsification), age, sex and corneal curvature, we could find the following results. 1. The mean change in intraocular pressure (postoperative IOP minus preoperative pressure) was -0.8 mmHg. 2. In 8 cases(3.8%) of 211 eyes postoperative IOPs were higher than 21 mmHg. 3. The higher preoperative IOP level was, the more the decrease of postoperative IOP was. 4. The changes in intraocular pressure was more significant in females than males. 5. There was no significant change in IOP level according to the age, surgical technique and corneal curvature.

7.Clinical Study of the Postperative Pain Management with Silver Spike Point Electro-therapy.

Silver Spike PintElectro-Therapy(SSPET) means passing electric current through the skin surface with the triangle spike silver coated metal electrode. Transcutaneous Electrical Nerve Stimulation(TENS) for chronic pain management was first reported by Shealy in 1972. Since 1975, Vanderark and Mograth reported that TENS has had a analgesic effect for the relief of acute postoperative pain. SSPET was reported by Hyoto and Kitade at Edinburg, Scotland. 3rd World Congress on Pain in 1981; It has a number of advantages over traditional narootic medication in postoperative pain management. It does not depress the cardiovascular, respiratory of metabolic system and has no apparent effect on the sensorium. Furthermore it is noninvasive, nontoxic, simple in its application and can be used continuously or intermittently. This study was undertaken to investigate recent reports concerning the use of Silver Spike Point Electro-therapy for the relief of postoperative pain. Sisyt patients undergoing elective Cesarean section were subjected to a standard perianesthetic proctocol by the Department of Anesthesiology at Hanyang University Hospital from March, 1st to August, 31, 1984. The patients were divided into three groups of twenty, designed control, SSP and sham SSP. Postoperative analgesic requirements for each group were compared. The results are as follows; 1) The age distribution ranged from 21 years to 36 yearts. Mean age was 25 years and the body weight ranged from 45kg to 68kg. The mean body weight was 52kg. 2) The incisional area of the Cesarean section was 23 cases (38.3%) of low midline incision and 37 cases(81.7%) of Pfannenstiel incision. 3) There were no remarkable changes in the blood pressure, pulse rate or respiratory rate during before and after SSP administration. 4) The total number of intramuscular doses of Demerol given to the patients in the ward was: control group 94 doses, SSP group 31 doses and sham SSP group 89 doses. The SSP group was statistical different from the other 3 groups when compared by the Student's T-test. 5) In the subjective assessment of each group, their statements were markedly varied. Nevertherless SSP has shown that the analgesic requirement was considerably less than that of the other group. This result means that SSP can be used for the relief of acute postoperative pain. Despite a number of diffidulties encountered during this study, we were primarily concerned with the action mechanism, subjective measurement of pain and standards in the requirment of Demerol. We considered that further investigation should be done in the use of SSP for postoperative analgesia and pain. SSP for postoperative pain management is a useful method that is simple, non-invasive, non-toxic and obtains safe analgesia.

9.Comparison of Nutritional Status According to Serum C-reactive Protein Concentration in Hemodialysis Patients.

Serum C-reactive protein(sCRP) is an acute-phase reactant that exhibiting negative correlation with serum albumin concentration. It was reported that sCRP is an independent predictor of survival in both hemodialysis and peritoneal dialysis patients, and an acute phase inflammation could be preceded by protein catabolism, hypoalbuminemia, anorexia and even atherosclerotic cardiovascular disease. We have evaluated serum biochemical parameters including albumin and prealbumin, Kt/Vurea, nPCR, SGA score, anthropometric parameters and diet history in 30 ESRD patients maintained on chronic hemodialysis subdivided by sCRP concentration. Upon comparing the two subgroups[high CRP group(sCRP >or= 0.4mg/dL), n=15 vs. normal CRP group (sCRP<0.4mg/dL), n=15], high CRP group showed significantly lower levels of hemoglobin(9.3+/- 0.7 vs. 9.8+/-0.6g/L, p<0.05), hematocrit(28.3+/-2.3 vs 29.8+/-1.696, p<0.05), creatinine(9.6+/-3.1 vs. 12.2+/-2.5mg/dL, p<0.05), prealbumin(20.9+/-5.0 vs. 25.8+/-6.4mg/dL, p< 0.05), SGA score(5.0+/-1.2 vs. 5.9+/-0.7, p<0.05), and percent of patients who have higher nPCR than protein intake(85.7 vs. 28.6%, p<0.05). Ferritin was significantly higher in high CRP group(503.1+/-205.7 vs. 323.3+/-186.6, p<0.05). There were no differences in age, sex, duration of hemodialysis, prevalence of diabetic nephropathy and cardiovascular disease, Kt/Vurea, nPCR, residual renal function, amount of protein intake and other nutritional parameters. In conclusion, there was higher probability of malnutrition, anemia and protein catabolism in hemo-dialysis patients with elevated sCRP concentration.
