3.Gluteus Maximus Fibrosis: Report of 3 Cases

The fibrosis involved in gluteus maximus causing limitation of flexion and adduction of the hip has become a recognized clinical entity since the first report by Fernandez de Valderrma in 1969. Its most constant and characteristic histologic feature was substitution of the fibrous tissue in the definitive etiology was unknown but presumed to be multiple intramuscular injections. Authors present three cases of the fibrosis involving gluteus maximus. In two cases Z-lengthening was performed on the thickened fibrous bands with good results.

4.Cross-reactivity between sera from dogs experimentally infected with Dirofilaria immitis and crude extract of Toxocara canis.

This study was performed to investigate whethere there is cross-reactivity between Dirofilaria immitis and three intestinal nematodes of dogs. In ELISA, D. immitis-infected dog sera obtained at the 4th molting stage (9-11 weeks) and microfilaremic stage (25-30 weeks) were shown to be highly reactive with crude extract of T. canis. In immunoblotting, some antigenic fractions, 44, 57, 88, 100 kDa of crude extract of T. canis, were found to be positive reaction with sera of dogs infected with D. immitis. However, little or no cross-reaction were observed between sera of D. immitis-infected dogs and crude extract antigen of T. vulpis or A. caninum. These result suggest that there are partial cross reaction between sera of D. immitis-infected dogs and the antigen of T. canis.

5.Vesical Endometriosis : A Review and 1 Case Report.

The term endometriosis refers to ectopic endometriume. It most often involves the pelvic organs, including ovaries, broad and round ligaments, fallopian tubes, cervix, vagina and the pouch of Douglas. We report a case of vesical endometriosis in a 4- year old female patient and review literature. Segmental resection of this mass with involved bladder resulted in cure and this lesion was confirmed histologically as endometriosis of the bladder.

7.A Clinical Study of Hand Tumors

Hand tumorsare many and varied, although benign tumors are common. The pathology and histology of these tumors are not unlike these characteristics of the tumor when it occurs elsewhere. Early diagnosis and proper treatment of all hand tumors have been emphasized since the growth of tumors and pain can cause disturbance of intrinsic function of the hand. For the period of 10 years from 1977 to 1986, 105 cases of hand tumor were treated surgically and the results of clinical observation were as follows ; l. Among 105 cases, benign tumor was 101 cases(96.2%) Bone origin tumor was 20 cases, in which enchondroma and enchondromatosis were 17 cases(70%). Soft tissue origin tumor was 85 cases, in which ganglion was 54 cases(63.5%). 2. Among 105 cases, 42 cases were male and 63 cases were female. In age distribution, it showed even distribution. 3. In the duration of wymptom, 51 cases were below 1 year(48.6%). 4. The tumor occured 85 cases in soft tissue, 14 in phalanx, 7 in metacarpal and 1 in carpal bone. 5. The tumors were treated by excision or curettage and bone graft with good improvement, but postoperatively recurred in 2 cases of Hemangioma. Ampuation was performed in 3 cases of malignant tumors without recurrence.

8.Percutaneous Nephrolithotripsy.

Recently, the modern surgical practice for the renal stone is being increasingly supplemented through the development of new technique. For example, hypothermic renal surgery, partial nephrectomy and extended pyelolithotomy were carried out by the ""therapy with the knife"". The percutaneous Nephrolithotripsy is one of the minimal invasive procedure to the renal stone surgery all over the world recently. This method is known as a safe, simple and reliable procedure for renal stone surgery, and they realize this technique as enormous advantage for patients; short hospital stay, minimal morbidity and rapid return to work etc. Three cases of renal stone through percutaneous nephrolithotripsy were carried out with much favour the first procedure in Korea. The first and third case were excellent result, but the second case was perforated on the uretero-pelvic junction of the kidney. There after, on the fifth postoperative day pyeloureteroplasty was performed with the splint of double J catheter. All the Patients were performed as the one session puncture, dilatation with telescopic dilator and lithotripsy with ultrasound lithotriptor. The two patients had 6th and 7th hospital day postoperatively.

9.Primary Leiomyosarcoma of the Ureter.

An 82-year-old woman was admitted to our hospital with a palpable mass in her right lower abdomen. The mass was enlarged and tender for the previous 3 months. Abdomen-pelvic computed tomographic scan demonstrated severe right hydroureteronephrosis and about an 8x8x9cm sized huge irregular shaped pelvic mass. Retrograde pyelography showed complete obstruction of the right middle ureter. En bloc resection of the mass and segmental resection of the ureter was performed. Pathologically, it was diagnosed primary leiomyosarcoma of the ureter. At 3 month postoperatively, there has been no recurrence or metastasis.

10.The Role of Gastric Resection for Advanced Gastric Cancer with Peritoneal Seeding.

PURPOSE: Peritoneal seeding is the most common type of metastasis and recurrence in gastric cancer. Recently, some studies have reported the benefits of a noncurative gastrectomy for greatly advanced gastric cancer; nevertheless, there are many controversies. This study was performed to investigate the survival benefit of a noncurative gastrectomy for patients with greatly advanced gastric cancer with peritoneal seeding. METHODS: We retrospectively analyzed 286 gastric-cancer patients who had received operations and who had been proven to have peritoneal seeding without liver metastasis or other hematogenous distant metastasis between January 1990 and December 1997 at the Department of General Surgery, College of Medicine, University of Ulsan. RESULTS: The distribution of the degree of peritoneal seeding was P1 in 84 cases (29.4%), P2 in 56 cases (19.6%), and P3 in 146 cases (51.0%). The duration of median follow-up was 9 months (range: 0.4-83.9 months). A noncurative gastrectomy was performed in 121 cases (42.3%); out of them, a total gastrectomy was performed in 49 cases (40.5%), a distal gastrectomy in 70 cases (57.9%), and a proximal gastrectomy in 2 cases (1.6%). A noncurative gastrec tomy was done 51 of the P1 cases (60.7%), 23 of the P2 cases (41.1%), and 47 of the P3 cases (32.2%). D2 lymph-node dissection was performed in 168 cases (87.6%). Postoperative complications developed in 5 cases with a noncurative gastrectomy, and there was no operative mortality. The median survival times were 11.3 months in P1 cases, 10.5 months in P2 cases, and 6.6 months in P3 cases. The median survival times of noncurative gastrectomy, bypass, and expoloratory laparotomy cases were 11.5 months, 6.6 months, and 6.3 months, respectively; according to the degree of peritoneal seeding, they were 14.8 months, 7.1 months, and 5.3 months in P1 cases, 15.3 months, 8.2 months, and 12.5 months in P2 cases, and 7.6 months, 6.4 months, and 5.7 months in P3 cases, respectively. The difference in survival time between the resection and the nonresection groups had statistical significance regardless of the degree ofperitoneal seeding (p<0.05). In the multivariate analysis, the degree of peritoneal seeding (RR: 1.33) and gastric resection (RR: 1.52) were proven to be significant prognostic factors. CONCLUSION: A noncurative gastrectomy might lengthen the survival time in advanced gastric-cancer patients with peritoneal seeding.
