2.Carcinoma expleomorphic adenoma of lacrimal gland.

Based on the histopathologic findings of the lacrimal gland tumor, the lesion is classified as either epithelial or nonepithelial tumor. 22% of lacrimal gland lesions were primary epithelial neoplasm. 4%of epithelial tumor of lacrimal glands were primary malignant tumor. Squamous cell carcinoma arising from pleomorphic adenoma of lacrimal gland is a very rare(0.4%). Inspite of surgical intervention, postoperative radiation and chemotherapy, the mortality and recurrence rate of the carcinoma ex pleomorphic adenoma of lacrimal gland is very high. We report a case of squamous cell carcinoma ex pleomorphic adenoma of lacrimal gland, which was completely removed via lateral orbitotomy with bicoronal incision and had no major complication or recurrence.

3.A Case of Angioleiomyoma of the Nasal Dorsum.

Angiomyoma is a benign myogenic tumor which can be found wherever smooth muscle is present. They are most commonly found in the alimentary tract, female genito-urinary tract, skin and subcutaneous tissues, but are rare in the area of the head and neck due to the scarcity of smooth muscle tissue. Because of their rarity, these tumors are often clinically misdiagnosed and identified only after histological examination. We present a case of angioleiomyoma which was found in the nasal dorsum and was excised by open rhinoplasty approach.

4.Messenger RNA and Protein Expression of Eotaxin and RANTES by Th1 and Th2 Type Cytokines in Human Nasal Fibroblasts.

BACKGROUND AND OBJECTIVES: Fibroblasts interact with eosinophils and play a key role in the pathogenesis of airway diseases. The aims of this study were to investigate whether Th1 or Th2 type cytokines can induce mRNA and protein expression of eotaxin and RANTES in human nasal fibroblast (HNF) and to verify the correlation between the stimulation of different cytokines and chemokines in HNF. Materials and Methods: Cultured HNF were stimulated by IL-13, TNF-alpha, IFN-gamma, IL-13 with TNF-alpha, IL-13 with IFN-gamma, TNF-alpha with IFN-gamma for 6, 24 and 48 hours. In addition, HNF were stimulated by different concentration of IL-13 (0.2, 2, 20, 200 ng/ml). MRNA expression of eotaxin and RANTES were revealed by RT-PCR and protein of eotaxin and RANTES were revealed by ELISA. RESULTS: TNF-alpha and IFN-gammar induced mRNA and protein expression of RANTES in HNF and they synergistically induced protein expression of RANTES. RANTES expression increased in a time dependent manner. IL-13 induced mRNA and protein expression of eotaxin in HNF and it synergistically reacted with TNF-alpha or IFN-gamma. The effects of IL-13 on mRNA and protein expression of eotaxin increased in a concentration dependent manner. CONCLUSION: Th1 or Th2 type cytokines induce mRNA and protein expression of eotaxin and RANTES in human nasal fibroblasts. TNF-alpha or IFN-gamma induce more RANTES than eotaxin but IL-13 induces more eotaxin than RANTES. There may be some synergic effects of cytokines for mRNA and protein expression of chemokines.

5.Detection of Bacillus anthracis using a nested PCR Method.

Bacillus anthracis is a soil pathogen capable of causing anthrax in animals and humans. To establish a method for specifically detecting B. anthracis, we used nested polymerase chain reaction. Outer and inner sets of oligonucleotide primers were designed from the protective antigen (pag) gene and from the cya gene of the plasmid pXO1. Ainplification of 482 bp or 208 bp DNA fragment obtained from a nested PCR method provided the basis for rapid and reliable assay for the detection and identification of B. anthracis.

6.The Effect of the Educational Program on Caring-Confidence and Rearing Attitude in Mothers with Newborns.

The purpose of this study was to find whether the educational program contributed to the increase of caring confidence and rearing attitude of the newborn mothers. The subjects for this study were 40 newborn mothers-20 control group and 20 experimental group-who delivered a normal neonate and were gotten general informations from the nurses at the K medical center in Seoul. Especially, the experimental group was educated for 30-40 minutes in maternal educational program through lectures, demonstrations, discussion with textbooks and visual materials by a researcher at 2-3 days after delivery. The data were collected two times-at 2-3 days after delivery and at 4 weeks after discharge-from October, 1998 to January, 1999 by investigators interviewing with a structured questionnaire. Pharis(1978)'s self confidence scale was used after some modification to measure caring confidence and Cohler's maternal attitude scale to check mother's rearing attitude. Analysis of data was done by using of frequency, %, chi-square-test, Independent sample t-test. The results of this study were as follows : 1. The Confidence of the experimental group was significant higher than the control group(t=2.519, p=.016) and the rearing attitude changed more positively(t=5.545, p=.000). 2. In the confidence scale, the statements having a significant change were holding a baby, changing diapers, feeding, and caring umblicus. 3. In the rearing attitude scale, the statements having a significant change were 12 of 20 statements.

8.The Electrolyte Changes of Pig`s Lens in Experimentally Induced Cataract.

Protein accounts for over one third of the human lens, the remaining two thirds being water. The other constotuents of the lens including lipids, amino acids, electrolytes and a variety of peptides and carbohydrates, account for about 1% of the lens wet weight. Since transparency of the lens is so highly dependent on protein order and structural integrity, it is not surprising that relatively small changes in any of these parameters might lead to the development of opacification resulting in a cataract.We have analyzed electrolytic differences between normal lens and lens of experimentally induced cataract to find the important factor in including cataract after we had extracted one hundred eight pig lenses. We divided these experimentally induced cataract into a group of normal lens capsule and another group of lenses which we performed with a 26 gauge needle. The sodium level was decreased in 15% mannitol solution and increased in normal saline. In every solution the potassium level was decreased. The chloride level was decreased in the 15% mannitol solution and increased in the normal saline solution. The calcium level also was decreased in mannitol and distilled water.Our results indicate that the potassium level of the experimentally induced cataracts decreased in any conditions and may be an important factor in inducing cataract.

9.A Korean Pedigree of Paget Bone Disease

Paget bone disease(PBD) is usually focal, but can be wide spread disorder of the skeletal remodeling characterized by greatly increased osteoclast size and activity. It has extremely variable prevalence worldwide, being common in England and northern European countries and areas populated by their descendants, but strikingly uncommon in Asia, the middle east, Africa and Scandinavia. It's occurrence also shows familial clustering, some postulates autosomal dominant inheritance. Many studies have shown that paramyxoviruses may play a critical role in the etiology of this disorder. However, the precise etiology of PBD remains unknown.We describe a kindred with PBD in 3 successive generations. The propositus, a 55-year-old man, has panostotic PBD and giant cell reparative granuloma of pagets disease involving his head, mandible, abdomen and ileum, rare tumorous complication of Paget's disease. Bowed limbs were first noticed at age 25 years, and progressed for 20 years. Giant cell reparative granuloma began manifesting at age 45 years, and responded dramatically to high-dose dexamethasone therapy. His pretreatment biochemical finding were remarkable for elevated serum ALP, 765(normal 66-220 u/L) and osteocalcin, 154(normal 6.3-30.7 mg/ml), but normal serum calcium, phosphorous, 250HD and PTH. A nondecalcified iliac crest specimen demonstrated classic histopathologic 25OHD and PTH. A nondecalcified iliac crest specimen demonstrated classic histopathologic changes of PBD on light microscopy. His decreased father had a similar degree of bony deformities beginning at age 20 years, but had not been examined. His two asymptomatic daughters, 20 and 24-year-old, were both found to be affected with widespread PBD by bone scan, radiographic study, and their serum ALP levels, 939 and 435U/L, respectively. This is the first report of familial occurance of PBD and a case of giant cell reparative granuloma of Paget's disease in Korea, where PBD is very rare.

10.Nasal Hyperreactivity to Methacholine Test in Perennial Allergic Rhinitis.

Although histamine and methacholine challenge tests are commonly used to evaluate the hypersensitivity in allergic rhinitis, some controversies still remain to standardize the test. This study was designed to evaluate the usefulness of the methacholine challenge test for the evaluation of hyperreactivity in allergic rhinitis. Twenty three patients with perennial allergic rhinitis and 17 normal subjects were included in this study. All subjects underwent saline nasal challenge and methacholine nasal challenge test with different doses (6 mg, 12 mg, 24 mg / in 0.5 ml of saline). We measured the nasal cavity volume by acoustic rhinometry in each step and counted the number of sneezes for 15 minutes. The volume of nasal secretion was also measured by tissue paper method. Besides that, we compared the data between allergic patients with a short (under 1 year) and long duration (more than 1 year). There were no differences in the frequency of sneezing and nasal volume change to each amount of methacholine in both groups. Nasal secretion increased after methacholine challenge in both groups. However, the allergic group showed significant increase after the methacholine challenge of 12 mg compared to the control group. And the volume of nasal secretion significantly increased in patients with a long duration. The nasal methacholine challenge test can be used as a diagnostic tool to detect hyperreactivity in perennial allergic rhintis. The optimal dilution was 12 mg (in 0.5 ml of saline) for the nasal methacholine challenge test.
