1.The Study of Talar Tilt Angle of Ankles in Korean Adult

An appreciation of the normal variation of the Talar Tilt angle under stress roentgenogram on the ankle joint may throw some light on the diagnosis and treatment in an ankle joint injuries. The purpose of this study is to show the normal range of the Talar Tilt angleof the ankle joint under various circumstances and has been to compare the results of each angle. The author have undertaken the neutral ankle mortise view and inversion and eversion stress roentgenogram in all ankles and measured its Talar Tilt angle. The data were based on study of free chosen 50 normal Korean adult male 25 and female 25 respectively. The results were summerized as follows; 1. Talar Tilt angle was O degree in neutral ankle mortise view. 2. In inversion stress roentgenogram, Talar Tilt angle ranged from 0 to 15 degrees, 97% of cases were less than 10 degrees. 3. In eversion stress roentgenogram, Talar Tilt angle ranged from 0 to 7 degrees. 4. In inversion stress roentgenoram, the difference between right and left of same male and female ranged from 0 to 7 degrees. 5. It is very important to check accurately Talar Tilt angle under the same method and same conditions.

2.Survey on Fertility Rate and Family Planning in Sub-urban Area, Taegu.

It is very desirable to clarify the situation of fertility, family planning and attitude toward family size of the women residing in the sub-urban slum area because of underprivilege of socio-economiccondition of the residents and negligible health services (especially MCH and family planning) to the people in the area. The study area was selected a slum and primary fixation place of the people immigrated from rural area: Taegu, Dong Ku, Shin Chun Dong, 2 Ka and Taegu, Suh Ku, Ri San Dong, 1 Ka The survey was conducted by family planning workers at Health Center from December 1974 to February 1975. The results were obtained as follows: Thirty-eight percent of residents immigrated from rural area and most of there consist of nuclear family. Average monthly income per family was 38,000 won and 74 percent of them haven't their own houses. Number of pregnancy and live births per womon were 3.8 and 3.0 respectively. The rates of spontaneous and induced abortion to total married women were 9.5% and 36% respectively. Regarding delivery conditions, 86% of deliveries were performed at home and 70% of them were cared by non-professional person or without helpers. Ideal number of children account for 1.58 for sons, 1.08 for. daughters 2.655 for both, but the rate of 2 children regardless sex difference was only 3.4%. The factors influencing to family size were age and educational level of women and economic status of the hosehold. Regarding family planning, the rate of experience of contraception in the past was 48 percent while the rate of present contraception was 24%. Most frequent pregnancy order at the first contraception was 4th and most frequent age group of the women was 35-39 years in the past while the most frequent pregnancy order and the age of women were 3rd and 30-34 years at the first contraception in the present. Average fertility rate per year to total eligible women for 5 years 20.8% indicating somewhat higher than those of oter places. Age specific marital fertility rate showed higher in younger age groups than those of other places, and total fertility rate accounts for 1548.4 showing similar to rural total fertility rate in 1970 and higher than those of other urban areas.

3.Eruptive Neonatal Hemangiomatosis.

Eruptive neonatal hemangiomatosis is an uncommon disorder, which is divided into two types according to the extent of involvement: Benign type with cutaneous hemangiomatosis only and diffuse type with widespread hemangiomas of skin and viscera. The organs commonly affected are the gastrointestinal tract, brain, liver and lung. The Diffuse type is often fatal. We herein report a case of eruptive neonatal hemangtomatosis with cutaneous and hepatic involvement. The size of the hepatic arteriovenous malformation was markedly decreased after 2 months' therapy with oral prednisolone.

4.Postoperative Changes of Arterial Blood Gas Tension and Ventilatory Function .

Pulmonary comp1ications following surgery continue to be a major cruse of postoperative mobidity. The purpose of this study was to measure the effects of various surgical procedures upon arterial blood gas tension and ventilatory function in the early postoperative period. 30 patients with no history of cardiopulmonary disease before surgery were selectively chosen and divided into 4 categories depending on the location of the operative incision and the type of anesthesia. Group l was composed of upper abdominal incision cases, Group ll lower abdominal incision cases, and Groups lll and lV had nonabdominal surgery. Groups, l, ll and lll received general anesthesia, while lV was liven spinal anesthesia. Lung volumes were measured and blood samples were taken preoperatively and 70 minutes after surgery. Patients were maintained in semisitting position before and after surgery. Vital capacities were significantly reduced in the postoperative period in the general ane- sthesia groups, and tidal volumes were significantly reduced in the upper and lower abdominal incision groups. The mean PaO2 value after anesthesia decreased significantly in the upper and lower abdominal incision groups, but there were no significant differencea in the PaCO2 value. It seems that supplemental oxygen and encouragement of deep inspirations are essential and beneficial in the early postoperative period, especially for the patients with abdominal surgery.

5.Clinical Evaluation of Intravenous Regional Aneetheeia using Bupivacaine .

We prospectively studied the clinical effect of intravenous bupivacaine for regional anes-thesia in the lower & upper extremities in sixty consecutive patient from September, 1985 to truly 1986. 0.2% bupivacaine was used and dosage was varied between 1.0mg/kg and 2.5mg/kg. according to the 7ype of surgery, site, body weight and the size of the limb. The amount of bupivaeaine required was rather depend upon the size of the limb thats the patient's body weight. The effect of the anesthesia for a variety of extremity procedures was rated as exoellent, good or fair in 98.3% of the patient.

6.The Effects of Tracheal Intubation with McCoy or Macintosh Laryngoscope on Blood Pressure, Heart Rate.

BACKGROUND: The aim of this study was to compare the cardiovascular changes followed by laryngoscopy with the McCoy laryngoscope blade with those followed by laryngoscopy with the Macintosh laryngoscope blade. METHODS: Forty eight patients were randomly divided into two groups. Following induction with fentanyl 2 mcg/kg and thiopental 5 mg/kg, and muscle relaxation with vecuronium 0.1 mg/kg, the vocal cords were visualized with either the McCoy or the Macintosh laryngoscope blade, then tracheal intubation was performed. Heart rate and arterial blood pressure were measured just before and after laryngoscopy, and 1, 3 and 5 min later. RESULTS: There was a significant increase in both heart rate and arterial blood pressure after tracheal intubation using the Macintosh laryngoscope. Also, use of the McCoy blade resulted in a significant increase in both heart rate and arterial blood pressure. CONCLUSIONS: There was no significant difference on arterial pressure and heart rate to laryngoscopy and tracheal intubation with either the McCoy blade or the Macintosh.

7.A Comparison of 10% Lidocaine Spray and Intravenous 2% Lidocaine on Mean ArterialPressure and Heart Rate Following Tracheal Intubation.

BACKGROUND: Laryngoscopy and tracheal intubation for general anesthesia are potent stimuli to the cardiovascular system due to sympathetic stimulation. The aim of this study was to compare the hemodynamic responses with different administration routes of lidocaine on blood pressure and heart rate changes associated with tracheal intubation. METHODS: Sixty patients were randomly assigned to spray group (Group 1, n=20) received 10% lidocaine 1.5 mg/kg spray to intratracheally immediately before tracheal intubation, IV group (Group 2, n=20) received 2% lidocaine 1.5 mg/kg intravenously before 90 sec tracheal intubation and control group (Group 3, n=20). Anesthesia was induced with thiopental 5 mg/kg IV, vecuronium 0.1 mg/kg and 50% N2O-O2- 2vol % enflurane. After 5 minutes, tracheal intubation was performed. Mean arterial pressure and heart rate were measured at preintubation and immediately postintubation, 1, 3, 5 mins after tracheal intubation. RESULTS:The differences of mean arterial pressure which were measured at preintubation and immediately postintubation and 1 minute after intubation in group 1 were significantly lower than those in other group (p<0.05). The differences of heart rate which measured at preintubation and 1 minute after intubation were lower than those in group 3 (p<0.05). CONCLUSIONS: For the suppression of sympathetic stimulation following tracheal intubation, 10% lidocaine spray to the laryngotrachea is an effective method to suppress cardiovascular response.

8.Inhibition of Interleukin-1 Signal by Annexin-1 in Phorbol Myristate Acetate Stimulated Lymphocytes.

Annexin-1 (ANX1) is a 37 kDa protein that is induced and secreted by glucocorticosteroid hormone. The secreted ANX1 has been believed to exert its function by binding to its putative rnembrane receptor. In this report we demonstrate that ANXl receptor (ANX1R) signal blocks the interleukin-1B (IL-1B) receptor signal pathway in human peripheral blood mononuclear cells (PBMCs). When PBMCs were treated with both IL-1B (100 ng/ml) and PMA (10 ng/ml) in the absence or presence of dexamethasone for 5 days, dexamethasone (100 nM) suppressed lymphocyte proliferation to 24% of the control. However addition of anti-ANX1 polyclonal antibody of 1:200 and 1:1,000 dilution to this system induced recovery of proliferation to 80% and 40%, respectively, when compared to the control. In the mixed lymphocyte reaction, dexamethasone suppressed lymphocyte proliferation to 9% of that of control when stimulated with IL-1B (100 ng/ml) and phorbol myristate acetate (10 ng/ml). Addition of anti-ANX1 polyclonal antibody (1:1,000) to this system also recovered the proliferation to 20% of that of the control system. In the ANX1 receptor induction experiment using flow cytometry, ANX1 receptor expression on lymphocytes, CD4+ T cells, CD8+ T cells and monocytes increased depending on the externally added IL-1B ranging from 10 to 1,000 ng/ml. From these results, it is evident that dexamethasone induces ANX1 secretion into the culture medium and anti-ANX1 polyclonal antibody abolishes the effects of dexamethasone. Furthermore these results imply that extracellular ANX1 exerts its effects by binding to the receptor on the cell membrane and the activated signal(s) of ANX1R block IL-1B receptor signal in the lymphocytes.

9.Comparison between the continuous Infusion and the Intermittent Administration of Vecuronium.

The neuromuacular blocking effects of the repeated bolus injection and continuous injection of vecuronium have been compared in 20 patients by means of evoked twitch tension. Group I received a loading dose (0.08 mg/kg) of vecuronium and followed by maintenance doae (0.02 mg/kg) when twitch tension had recovered to 25% of control. Group II received a loading doses (0. 08mg/kg) of vecuronium plus a continuous infusion delivering 0.06 mg/kg/hr. Anesthesia was maintained with nitrous oxide, oxygen and 1.5% enflurane. The neuromuscular blocking effect were reversed by the neostigmine 1.0 mg and robinul 0.2 mg when twitch tension had recovered 25% control in both groups near the end of operation. With repeated injection of vecuronium (group I), the duration of blockade of 25% recovery was 21.6+/-4.2 minutes and the recovery times from 25% to 75% was 6.8+/-1.9 minutes The continuous infusion of vecuronium (group II) produced consistent neuromuscular blockade at the average level of 90% twitch depression. The times from the end of infusion to 25% and fr,om 25% to 75% are averaged 16.0+/-2.5 and 4.2+/-0.7 minutes respectively. The reeovery index was more rapid in infusion group than maintenance group (P<0.05).

10.Pedunculated Lipofibroma.

A 45-year-old woman had a bean-sized, asymptomatic, soft nodule on the right buttock for 4 years. Histopafhologic evaluation revealed acanthosis and a central core of adipose tissue with thin papillary and reticular dermis containing eccrine glands. Staining with alcian blue showed a slight increase of mucopolysaccharides in the reticular dermis. We made the diagnosis of pedunculated lipofibroma as a solitary form of nevus lipomatosus superficialis.
