2.Transvaginal Color Doppler Imaging in the Assessment of Utero-placental Blood Flow in Normal and Abnormal Early Pregnany.

Flow velocimetry waveforms of the uterine artery and subtrophoblastic blood flow were analyzed at normal early pregnancy, missed abortion and blighted ovum, by using transvaginal color Doppler ultrasonography. Results show that a progressive fall in S/D ratio and RI with advancing in gestation. Among the three groups, the S/D ratio and RI of both uterine arteries were not a significant difference in all gestational age. Characteristic subtrophoblastic blood flows were obtained in 60.3%, 47.2% and 53.3% of normal pregnancy, missed abortion and blighted ovum, respectively. The S/D ratio and RI of subtrophoblastic blood flow were not a significant difference among the three groups. Although the number of cases studied is small and not prospective study, the further study about this will give us some understanding to the pathophysiology of early pregnancy failure.

3.A Study of the Rubella Antibody in Korean Fertile Women in the Last 5 Years (1992~1996).

In the past two decades, the incidence of rubella infection has decreased due to a nation-wide rubella vaccination program. Therefore the rate of rubella Ig G antibody has decreased. This means an increasing vulnerability to rubella infection among the group, with negative Ig G antibodies. The study was conducted in January of 1992 to April of 1996. The subjects were 1,010 fertile women(ages 20/45 years). The study was performed at Park Women`s Clinic to investigate the status of rubella antibody in Korean fertile women. The results were as follows. 1. The positive rate of rubella Ig G and Ig M antibody were 80.5% and 1.5%, respectively. 2. The mean age of positive rubella Ig G group was 28.5+/-3.6 years. The positive rate of rubella Ig G antibody was decreased by an increase in age with a significant level at p < 0.05. 3. The positive rate of rubella Ig G antibody was 63.3% in 1992, 71.3% in 1993, 87.5% in 1994, 81.1% in 1995 and 83.8% in 1996. The increase in the positive rate of rubella Ig G antibody by year for 5 years was statistically significant at p < 0.05. 4. The positive rubella Ig M antibody was noted in 15 women. Among them, one woman was tested in 1993, 5 women in 1995 and 9 women in 1996. The incidence of rubella infection had increased by years, and was statistically significant at p < 0.05. 5. The mean titer of rubella Ig G antibody was 47.18+/-50.04 IU/ml. The titer of rubella Ig G antibody was decreased by the increased in at age(p < 0.05). We observed a low positive rate of rubella Ig G antibody as compaired with other studies that were performed before 1990. Therefore, to prevent sporadic rubella infections, rubella vaccinations should be given to unmarried women.

4.The Significance of Cervical Erosion with Benign Cellular Change on Papanicolaou Smear.

The significance of cervical erosion with benign cellular change on Papanicolaou smear has not been evaluated. A retrospective review of 430 coloscopically directed punch biopsies performed over 15 months revealed 206 cases who had a benign cellular change on cytologic smear. Of these 206 smears, 48(23%) had clear cervix without erosion, 99(48%) had mild erosion on cervix, 41(20%) had moderate cervical erosion and 18(9%) had severe cervical erosion. The false negative rate that revealed more than mild dysplasia on histologic diagnosis was 2% in patients without cervical erosion, 3% in patients with mild cervical erosion, 5 % in patients with moderate cervical erosion, and 11% in patients with severe cervical er- osion. Koilocytotic change that suggested human papilloma virus infection was 17% in each group regardless cervical erosion. In conclusion, benign cellular change on Pap smears in patients with severe cervical erosion may be associated with significant cervical cancer pathology.

6.Relationship Between ESR, TTT, HBsAg in Viral Hepatitis.

We observed 32 cases of viral hepatitis who were admitted to to pediatric ward of Korea General Hospital for 1 year from Oct. 1979 to Sept. 1980 and the following results are obtained. 1) Eight among 32 eases of viral hepatitis are positive for HBsAg in their serum samples tested by solid phase radioimmunoassay. 2) High ESR is found in 69.6% of cases with type A hepatitis. In type B hepatitis ESR within normal limit is 87.5% of cases. 3) Values for GPT are higher than those of GOT in 70.8% of patients with type A hepatitis. In type B hepatitis GOT higher than GPT are 87.5% of cases. 4) Elevated levels of TTT are observed in 83.4% of patients with type A hepatitis. In type B hepatitis TTT within normal limit is 75.0% of casses. We may conclude that ESR, SGOT, SGPT and TTT can be a very useful additional test as a diagnostic aid in the differentiation of the two types of viral hepatitis, when tested early in the disease.

7.A Case of Ruptured Acute Appendicitis Presenting as Pneumoperitoneum in Low Birth Weighted Premature Baby.

Acute appendicitis is very rare in premature neonates. Preoperative diagnosis of this condition is difficult, and then it leads to high morbidity and mortality. We report 9-day-old premature male with ruptured acute appendicitis presented with pneumoperitoneum on plain films of the abdomen. Awareness of this rare condition and possible differential diagnosis in this age group is also discussed.

8.Fine Needle Aspiration Cytologic Diagnosis of Thymoma Presenting as a Thyroid Nodule: A Report of Two Cases.

Thymoma is the most common anterior mediastinal tumor in adults. Rarely, it is presented as the anterior neck mass, commonly located in the anterolateral aspect of the neck or adjacent to the thyroid. We experienced two cases of fine needle aspiration cytology of thymoma, mimicking thyroid mass. The first case was an ectopoic cervical thymoma in a 31-year-old female. The fine needle aspiration cytology was misinterpreted as reactive hyperplasia of lymph node. But the histologic diagnosis was thymoma, predominantly lymphocytic type. The second case was an invasive thymoma in a 66-year-old female, who complained a large anterior neck mass. The fine needle aspiration cytology revealed biphasic population of some clusters of epithelial cells and scattered lymphocytes. The cytologic diagnosis was thymoma and was confirmed as invasive thymoma after the biopsy. Therefore, when the cytologic feature of anterior neck mass shows the both lymphocyte and epithelial component, the differential diagnosis should include the possibility of thymoma.

9.A Case of leukemic Reticuloendotheliosis: A case report and literature review.

Leukemic reticuloendotheliosis (L.R.E.) was first described as a clinical and pathological entity by Ewald in 1923. LRE is a rare neoplastic disease of the hematopoietic system that is characterized cliniclly by chronic course with an insidious onset. marked splenomegaly with absence of substantial lymphadenopathy and predominant in male and characterized histologically by the presence of circulating abnormal mononuclear cells with many cytoplasmic projections, which have been refered to ??airy cells frequently. Splenectomy appeared to be the most beneficial treatment at present for those patients with massive splenomegaly and hypersplenism and chemotherapy is of little benefit in the treatment in LRE. Recently, the authors had the opportunity to observe a two year and eight old male child at this hospital whose clinical course and pathologid features were consistent with LRE. Splenectomy is performed and discharged with relatively good conditions.

10.Plexiform Neurofibroma Involving the Ascending Colon and its Mesentery in a Child.

The neurofibromatosis type 1 (NF-1) is a rare hereditary disease of autosomal dominant fashion with the overall incidence of one in 3,000. It is characterized by cafe-au-lait spots of skin, multiple cutaneous neurofibromas and a broad spectrum of clinical finding. Plexiform neurofibroma is a frequent complication of NF-1 but symptomatic involvement of the gastrointestinal tract in children with NF-1 is rare. It may present with complications such as obstruction, dysfunction, pain and hemorrhage. We report that a 13-year-old female diagnosed with NF-1 at the age of 10 years had abdominal pain and showed huge plexiform neurofibromas of right mesocolon in abdominal cavity. She also presented with multiple cafe-au-lait spots and axillary freckling. She was performed a right hemicolectomy with complete excision of the mass and had an uneventful course.
