3.Comparison of Optical Performances in Eyes Implanted With Aspheric and Spherical Intraocular Lenses After Cataract Surgery.

PURPOSE: To compare optical performances in eyes implanted with aspheric and spherical intraocular lenses (IOLs) after cataract surgery. METHODS: In 88 eyes of 69 patients, spherical (Sensar AR40; Acrysof natural SN60AT) or aspherical (Tecnis ZA9003; Acrysof IQ SN60WF) IOLs were implanted. After one month, contrast sensitivity, depth of focus, and spherical aberration were measured and compared between the groups. RESULTS: Contrast sensitivities of ZA9003 and SN60WF were significantly higher in 12,18 cycles per degree (CPD) under photopic conditions and were also higher in 6,12,18 CPD under mesopic conditions compared to those of respective spherical IOLs. Depths of focus were 1.31 D in ZA9003, 1.67 D in SN60WF, 1.52 D in AR40, and 1.49 in SN60AT, and the differences were not significant. Spherical aberration (Z40) with a 4 mm pupil was -0.032 microm in ZA9003, 0.022 microm in SN60WF, 0.076 microm in AR40, and 0.072 microm in SN60AT. Spherical aberration of SN60WF was significantly lower than spherical IOLs, and that of ZA9003 had the lowest among all IOL groups. Depth of focus significantly correlated with spherical aberration. CONCLUSIONS: Aspheric IOLs were superior in mesopic contrast sensitivity and equal in depth of focus to spherical IOLs. Individual corneal spherical aberration should be considered in the choice of IOLs.

4.Erythema Nodosum Associated with Idiopathic Granulomatous Mastitis in Young Pregnant Woman.

Idiopathic granulomatous mastitis (IGM) is a rare chronic inflammatory condition of unknown etiology. It occurs in young, parous women who had experienced breast feeding, and is manifested as a tender breast mass. Histologically, a discrete granulomatous lobulitis consisting of giant cells, neutrophils, epitheloid cells, and foamy macrophges is shown. Extramammary manifestations of IGM are rare. Erythema nodosum (EN), arthralgia, and episcleritis have been reported, supporting the theory that there is an autoimmune component in the etiology of IGM. Coincidence of IGM and EN is rare. Herein, we report a 28-year-old pregnant woman in whom granulomatous mastitis presented with arthralgia and EN.

5.A Case of XX Male Syndrome with Anophthamia.

XX male has a male phenotype with testes or gonads of testicular type and a female chromosomal constitution of 46, XX with no evidence of either ovarian tissue or female genital organs. Generally, they have normal male genitalia and all are infertile. We experienced a neonate with anophthalmia, hypospadia, small penis, and normal testes, whose chromosomal analysis demonstrated 46, XX. Polymerase chain reaction revealed the existence of a sex-determining region of Y (SRY). These findings suggest that the translation of an SRY on the X chromosome led to the development of a male phenotype. We report the case with a review of the related literature.

6.Interleukin-17 in Various Ocular Surface Inflammatory Diseases.

Recently, the association of Th-17 cells or IL-17 with ocular inflammatory diseases such as uveitis, scleritis and dry eye syndrome was discovered. We assessed whether interleukin (IL)-17 was present in the tears of various ocular surface inflammatory diseases and the tear IL-17 concentrations were clinically correlated with various ocular surface inflammatory diseases. We measured concentrations of IL-17 in tears of normal subjects (n = 28) and patients (n = 141) with meibomian gland dysfunction (MGD), dry eye syndrome (DES), Sjogren syndrome (SS), Stevens-Johnson syndrome (SJS), graft-versus-host disease (GVHD), filamentary keratitis, and autoimmune keratitis associated with rheumatoid arthritis or systemic lupus erythematosus. Clinical epitheliopathy scores were based on the surface area of corneal and conjunctival fluorescein staining. The mean concentrations of IL-17 in tears of patients with filamentary keratitis, GVHD, autoimmune keratitis, SS, DES, MGD, SJS were significantly higher in order than that in normal subjects. Tear IL-17 concentration was significantly correlated with clinical epitheilopathy scores in the patients with systemic inflammatory disease, while tear IL-17 was not correlated with clinical severity of the cornea and conjunctiva in the dry eye patients without any systemic inflammatory disease. Tear IL-17 is likely to correlate clinically with corneal disease severity only in the patients with systemic inflammatory disease.

7.Laser therapy in peri-implantitis treatment: literature review.

Peri-implantitis is the most common reason for a late failure and can occur even after years of successful osseointegration. The role of microbial plaque accumulation in the development of peri-implantitis has been well documented. On the other hand, the ideal method of implant surface decontamination to re-establish the health of peri-implant tissue remains to be determined. Removal of bacterial deposits is essential in the treatment of peri-implant infections, and various therapeutic approaches have been described in the literature, including mechanical debridement, disinfection with chemotherapeutic agents, and laser therapy. Recently, there has been a plenitude of scientific data regarding the use of laser irradiation to achieve titanium surface decontamination. Thus, research is focusing on lasers' potential use in the treatment of peri-implantitis. The aim of this literature review is to analyze and evaluate the efficacy of laser therapy for the treatment of peri-implantitis.

8.A Case of Severe Localized Reaction due to Extravasation of Contrast Media.

Extravasation of contrast media into subcutaneous tissue is a relatively frequent complication. Although most patients complain of initial swelling, tightness, and burning pain at the site of extravasation, some experience little or no discomfort. In most cases, conservative management is recommended. However, severe injury after extravasation of contrast media have been reported, including skin ulceration, tissue necrosis, and compartment syndrome, in which cases surgical intervention should be considered. We report a case of a 53-year-old man who had severe localized reaction due to extravasation of contrast media for a follow-up brain CT scan after subarachnoid hemorrhage.

10.Evaluation of the validity of Qraycam for tooth examination during epidemiological surveys.

OBJECTIVES: The aim of this study was to evaluate applicability of the Qraycam device for detecting caries and filling body during tooth examinations. METHODS: Fifty-two subjects aged 25 to 34 years were recruited for tooth examination. Two examiners (an epidemiologic expert and a non-expert) performed visual tooth examination using only dental operating light, dental mirror, and air-drying without a dental explorer. Pictures or videos of every tooth surface were obtained under visual ray and 405 nm blue ray, respectively, by using Qraycam. The two examiners then evaluated these videos or images more than 7 days after visual examination. RESULTS: The results of the visual, visible ray image, and 405 nm blue ray image examinations showed very good kappa agreement with the gold standard for both examiners. The 405 nm blue ray image examination showed higher kappa agreement than visible ray image examination, and was similar to visual examination. Accuracy of detecting caries was enhanced by using 405 nm blue ray images from Qraycam. Accuracy of detecting filling body on 405 nm blue ray image examination was almost same as that by visual examination. CONCLUSIONS: Tooth examination with Qraycam images revealed high agreement with the gold standard and showed accuracy for detecting caries and filling body. Therefore, Qraycam would enhance the quality of oral epidemiologic survey including tooth examination and save cost and time of survey.
