2.A Study of the Rubella Antibody in Korean Fertile Women in the Last 5 Years (1992~1996).

In the past two decades, the incidence of rubella infection has decreased due to a nation-wide rubella vaccination program. Therefore the rate of rubella Ig G antibody has decreased. This means an increasing vulnerability to rubella infection among the group, with negative Ig G antibodies. The study was conducted in January of 1992 to April of 1996. The subjects were 1,010 fertile women(ages 20/45 years). The study was performed at Park Women`s Clinic to investigate the status of rubella antibody in Korean fertile women. The results were as follows. 1. The positive rate of rubella Ig G and Ig M antibody were 80.5% and 1.5%, respectively. 2. The mean age of positive rubella Ig G group was 28.5+/-3.6 years. The positive rate of rubella Ig G antibody was decreased by an increase in age with a significant level at p < 0.05. 3. The positive rate of rubella Ig G antibody was 63.3% in 1992, 71.3% in 1993, 87.5% in 1994, 81.1% in 1995 and 83.8% in 1996. The increase in the positive rate of rubella Ig G antibody by year for 5 years was statistically significant at p < 0.05. 4. The positive rubella Ig M antibody was noted in 15 women. Among them, one woman was tested in 1993, 5 women in 1995 and 9 women in 1996. The incidence of rubella infection had increased by years, and was statistically significant at p < 0.05. 5. The mean titer of rubella Ig G antibody was 47.18+/-50.04 IU/ml. The titer of rubella Ig G antibody was decreased by the increased in at age(p < 0.05). We observed a low positive rate of rubella Ig G antibody as compaired with other studies that were performed before 1990. Therefore, to prevent sporadic rubella infections, rubella vaccinations should be given to unmarried women.

4.Primary Sclerosing Mucoepidermoid Carcinoma with Eosinophilia of the Thyroid: Description of a Case and Review of the Literature.

Primary sclerosing mucoepidermoid carcinoma with eosinophilia (SMECE) of the thyroid gland is a very rare disease. We present the clinical and histopathologic findings of a 37-year-old woman recently diagnosed with SMECE of the thyroid gland. The patient, clinically euthyroid, who presented with a neck swelling since last 2 years along. Fine needle aspiration cytology suggested thyroid papillary carcinoma. Total thyroidectomy, central neck dissection and right selective neck dissection were performed. Although SMECE is considered to be a relatively slow growing and non-aggressive tumor, occasional metastasis does occur. We report an additional case of SMECE, with metastasis to regional lymph nodes. Physicians should be aware of extended operation, including total thyroidectomy and/or neck node dissection for metastatic lesion of the neck node. More standardized treatment is likely to evolve in the future.

5.Clinical Application of Digital Infrared Thermographic Imaging for the Prediction of Foot Ulcer Development in Diabetic Patients.

OBJECTIVE: To investigate the regional differences of skin blood flow and to evaluate the effects of foot temperature on the severity of neuropathic pain and to predict the development of plantar foot ulceration by measuring of the temperature variations on the plantar surface of feet in the diabetic patients. METHOD: We measured the temperature variations on plantar surface of the feet in controls (n=18) and diabetic patients with(n=20) or without(n=23) polyneuropathy. The surface temperature from the 3rd metatarsal head(MTH), greater toe(GT), heel, medial and lateral longitudinal arch(LA) was measured by Digital Infrared Thermographic Imaging(DITI). RESULTS: The mean foot temperature of diabetic patients with polyneuropathy was significantly increased compared to controls or diabetic patients without polyneuropathy(p<0.001). The surface temperature readings of the GT, medial LA and the 3rd MTH tended to be increased in controls and patients with polyneuropathy. The mean plantar surface temperature was significantly increased according to the duration of diabetes mellitus(DM)(p<0.05). CONCLUSION: The results suggest that DITI provides a diagnostic modality in the prediction of neuropathic foot and increased risks of foot ulcer development in the diabetic patients.

6.The investigation for the change of HBsAg positive rate of grade junior high high-schoolers for recent 3 years in Kangwon province.

BACKGROUND: Since 1983, vaccination for Hepatitis B virus(HBV) has been recommended for all neonates in Korea. Therefore, significant change of HBsAg positive rate is expected. The objective of this study is to investigate HBsAg positive rate of grade, junior high, and high schoolers in Kangwon Province. METHODS: From 1997 to 1999, total 120,220 subjects, who were born in 1981 to 1992, were enrolled in this study. HBsAg and anti-HBs were tested by reversed passive hemagglutination and passive hemagglutination, respectively. If their results had been vague, we perfomed enzyme-linked immunosorbent assay for confirmation. RESULTS: HBsAg positive rate of entire study group was 2.5%(3,045 out of 120,220). Those of schoolboys and schoolgirls were 2.6%(1,567 out of 60,076), 2.5%(1,478 out of 60,144), respectively. Significant statistical difference was not observed in comparison of HBsAg positive rate according to sex(p=0.09). HBsAg positive rate of grade schoolers was 1.4%(676 out of 48,871), which was significantly lower than that of junior high(3.2%, 557 out of 17,577) and high-schoolers(3.4%, 1,812 out of 53,772)(p< 0.01). From 1997 to 1999, HBsAg positive rate has significantly decreased from 2.8%(1,434 out of 50,434) to 1.9%(407 out of 20,991)(p< 0.01). HBeAg positive rate of HBsAg positive subjects was 62.9%(1,000 out of 1,590). CONCLUSION: HBsAg positive rate of grade-schoolers in Kangwon province Korea, tended to be lower than that of junior high, or high-schoolers, and is also thought to be lower than that of the past, which was caused by nationwide vaccination programme.

7.The Estimation of Breast Cancer Disease-Probability by Difference of Individual Susceptibility.

PURPOSE: The aims were to evaluate the main risk factors (RFs) of breast cancer and to estimate the individual disease-probability from combinations of RFs in Korean female. MATERIALS AND METHODS: We conducted case-control study of 1, 687 incident cases of invasive carcinoma and 1, 238 controls during 1996~2000. A breast cancer disease-probability model was established by a general modeling process using a multivariate logistic regression model, which included the main Korean RFs and synergistic interaction-terms. RESULTS: The main Korean RFs selected were age, family history of second relatives, BMI, age at first full term pregnancy, breast-feeding, and a special test on the breasts. Two synergisms were observed between age and breast-feeding, and between special test and age at first fullterm pregnancy. The disease-probability and model are shown in Table 4, and Appendix 1. CONCLUSION: The availability of previous Western models was limited for Korean female due to the differences inhazard-rates and the characteristics of breast cancer between Asian and Western females. Due to limited basic data, i.e. incidence, hazard-rate and cancer-cohorts, the developing-probability of breast cancer for Korean females was not calculated. Therefore, the disease-probability was calculated instead. This approach might be more beneficial for Koreans, and help in the decision- making for regular screening or hospital visit-interval, counseling in breast-cancer clinics, prescribing high-risk population, and in educating for primary prevention, although it over-estimates the relative probability compared to the developing-probability and the 65% predictive validity.

8.Incidence of Female Breast Cancer in a Defined Area in Korea.

This study was conducted to estimate incidence rate of female breast cancer in a defined area of Chungchongbuk-do in Korea. The presumptive breast cancer cases were selected from two different sources, i.e., medical utilization database of the National Health Insurance Corporation and the database from the National Cancer Registry. Medical students visited each hospital where the presumptive cases had been treated as a breast cancer patient, and made a dictation of medical record of each patient based on the claims stored in the Insurance Corporation from January to December 1995. The diagnoses in the claims included one of the following diagnostic codes; ICD-9 174-175(malignant neoplasms of the breast), 233(carcinoma in situ of the breast and genito-urinary system)or ICD-10 C50(malignant neoplasms of the breast), D05(carcinoma in situ of the breast and genito-urinary system). Each case has been confirmed as having a breast cancer by a breast surgeon through a medical record review. Age-standardized incidence rate of female breast cancer to the Korean population was estimated to be 10.5(95%confidence interval : 8.1-12.9)per 100,000 persons in 1995. Age-standardized rate to the world population was 9.8 per 100,000 persons, and the truncated rate for ages 35-64 was 27.2 per 100,000 persons. Validity of these estimates is discussing in comparison with previous methods of incidence estimation in Korea.

9.Anti-Tumor Effect of AG60 against Ehrlich Tumor.

BACKGROUND: AG60 is a complex of acriflavine and guanosine. Our previous study revealed that AG60 had not only in vitro antitumor activities in several human cancer cell lines, but also strong antitumor effects in animal experiments using p388 or S180 cells-implanted mice. METHODS: Antitumor effects of AG60 were compared with those of Adriamycin, acriflavine, guanosine or control group. Body weight, tumor weight change, and survival time were measured in Ehrlich carcinoma cells implanted ICR mice. RESULTS: Body weights in AG60, acriflavine, or Adriamycin treated groups were significantly lower than those in control group during 30 day observation period(p<0.05). The percent tumor growth inhibition of AG60, Adriamycin, acriflavine, or guanosine two weeks after last treatment was respectively 86% (T/C%=14), 83% (T/C%=17), 68%(T/C%=32), 41% (T/C%=59). According to above data, tumor growth inhibition in AG60 treated group was significantly stronger than that in control, acriflavine or guanosine treated group(p<0.01), but there was no significant difference between AG60 and Adriamycin treated group. Mean survival time in control, AG60, Adriamycin, acriflavine, or guanosine treated group was respectively 33+/-3.9 days, 68+/-4.2 days, 54+/-5.8 days, 36+/-3.8 days, 50+/-8.1 days. CONCLUSIONS: The anti-tumor effect of AG60 against Ehrlich tumor was significantly stronger than that of control, acriflavine or guanosine, and comparable with Adriamycin. Mean survival time in AG60 treated group was significantly longer than that in control, acrifavine, guanosine or Adriamysin treated group.

10.Factors Associated with the Use of Pap Test in a Rural Area.

OBJECTIVES: To construct basic data to develop strategies for achieving higher Pap test coverage rate by evaluating factors associated with the use of Pap test through population-based survey. METHODS: 16.4%(671) of the 4,090 women, who were eligible population for this study, in 3 Myens of Chung-ju City participated in this study voluntarily from July 21 to 26, 1997. After basic physical examination by trained doctors, they were interviewed with structured questionnaire by well-educated interviewers. RESULTS: It shows that only 54.3% of study participants experienced Pap test. The strongest factor which is related with the use of Pap test was the history of having breast screening tests(aOR=8.71, 95% CI=4.25-17.84). Probability of ever having Pap test was also higher in married women(aOR of single=0.46, 95% CI=0.29-0.72), younger(Ptrend<0.05), more educated (Ptrend<0.001), non-smoker (aOR of smoker=0.26, 95% CI=0.12-0.55), women of ever having hepatitis test(aOR=2.60, 95% CI=1.73-3.88) in multiple linear logistic analysis. CONCLUSIONS: This study suggests that several factors significantly associated with the use of Pap test, and especially, high-risk population for cervical cancer such as women of older ages, less educated, living alone are less likely to have the Pap test. We should concentrate on encouraging high-risk women in the use of Pap test to improve Pap test coverage rate.
