Menetrier's disease is a rare, characterized by the presence of large rugal folds involving part or all of the stomach. Patients with hypertrophic gastropathy often have distressing abdominal symptoms, weight loss, and edema due to gastric protem loss. A 64 year old male patient was admitted with the chief complaint of epigastric discomfort. Endoscopic, radiologic and histologic examination, revealed characteristic findings of Menetrier's disease. A case of Menetrier's disease is reported with a brief review of literature.

3.The Clinical Implication of Intravesical Prostatic Protrusion to Lower Urinary Tract Symptoms in Patients of Prostatic Volume 30~50gm.

PURPOSE: Intravesical prostatic protrusion (IPP) is a morphological change of prostate protruded into bladder and might be related to bladder outlet obstruction (BOO) depending on the extent of protrusion. There is a high possibility that lower urinary tract symptoms (LUTS) tends to get worse as IPP grows. Therefore, it is necessary to examine the effect of IPP on LUTS after revision of prostate volume in order to identify the direct effect of IPP on BOO and LUTS. MATERIALS AND METHODS: A retrospective study of 296 male patients diagnosed with benign prostate hyperplasia (BPH) between August 2006 and December 2008 were performed. The patients were evaluated with international prostate symptoms score (IPSS) and quality of life (QoL), uroflowmetry (UFR), post-void residual urine volume (PVR), prostate volume and IPP as measured by transurectal ultrasound (TRUS). The changes of IPSS after 8 weeks of medication treatment and the occurrence rates of transurethral resection of prostate (TURP) and acute urinary retention (AUR) were compared. By checking IPSS and UFR, the improvement of voiding symptom was evaluated. RESULTS: The population of patients with moderate enlargement (30-50gm) of prostate extent was 147 (49.6%) out of 296, the whole examined population. There were correlation between IPP and IPSS (p=0.002) and the storage symptoms score was significantly increased (p=0.014). After 8 weeks of medication treatment, both moderate BPH patients with IPP showed similar improvement in storage symptom compared to non-IPP groups. But, the TURP was significantly performed more with IPP (p=0.040) than non-IPP groups and more AUR has occurred (p=0.013). After TURP, IPP group resulted in improvement of IPSS, storage symptoms score and voiding symptoms score statistically compared to non-IPP group. As the change of UFR and IPSS after TURP had shown, the improvements of voiding volume, maximal flow rate (Qmax), and average flow rate (Qavg) in IPP groups were smaller however, it is no correlation in statistical view. CONCLUSIONS: Moderate BPH group with IPP has a higher possibility of having AUR and surgical treatment while showing significant correlation with storage symptoms. A further prospective study is necessary for identification of improvement of IPSS and UFR after TURP and IPP should be checked carefully during TRUS.

4.Migratory defect of mesencephalic dopaminergic neurons in developing reeler mice.

Reelin, an extracellular glycoprotein has an important role in the proper migration and positioning of neurons during brain development. Lack of reelin causes not only disorganized lamination of the cerebral and cerebellar cortex but also malpositioning of mesencephalic dopaminergic (mDA) neurons. However, the accurate role of reelin in the migration and positioning of mDA neurons is not fully elucidated. In this study, reelin-deficient reeler mice exhibited a significant loss of mDA neurons in the substantia nigra pars compacta (SNc) and a severe alteration of cell distribution in the retrorubal field (RRF). This abnormality was also found in Dab1-deficinet, yotari mice. Stereological analysis revealed that total number of mDA neurons was not changed compared to wild type, suggesting that the loss of mDA neurons in reeler may not be due to the neurogenesis of mDA neurons. We also found that formation of PSA-NCAM-positive tangential nerve fibers rather than radial glial fibers was greatly reduced in the early developmental stage (E14.5) of reeler. These findings provide direct evidence that the alteration in distribution pattern of mDA neurons in the reeler mesencephalon mainly results from the defect of the lateral migration using tangential fibers as a scaffold.

5.Correlation between Carbapenem Prescription Trends and Imipenem Resistance in Acinetobacter baumannii at an Intensive Care Unit between 2006 and 2010.

Carbapenem resistance in Acinetobacter baumannii has increased rapidly worldwide. It is generally assumed that carbapenem prescription in a hospital has a significant impact on imipenem resistance in A. baumannii. However, there are few studies validating these assumptions with statistical data. We performed a surveillance study to investigate the relationship between carbapenem prescription trends and the imipenem resistance rate of A. baumannii in an ICU. Carbapenem prescription data in the WHO anatomical therapeutic chemical (ATC)/defined daily dose (DDD) format for the period from 2006 to 2010 were obtained from the hospital electronic pharmacy records. In the same period, microbiologic data for the ICU were extracted from the laboratory information system. Imipenem resistance rates of A. baumannii increased from 4.3% in 2006 to 83.8% in 2010 (P <0.05; r2=0.85). Carbapenem prescription had increased from 19.71 DDD per 1,000 inpatient-days in 2006 to 36.99 DDD per 1,000 inpatient-days in 2010 (P <0.05; r2=0.95). Carbapenem prescription rate correlated with the imipenem resistance rate in A. baumannii (P <0.05; R=0.9). The results of our study demonstrated a correlation between carbapenem prescription trends and imipenem resistance in A. baumannii.

6.A Large Malignant Peripheral Nerve Sheath Tumor in the Neurofibromatosis Patient: A Case Report.

PURPOSE: Malignant peripheral nerve sheath tumors most often arise from the anatomically discernible peripheral nerve or neurofibroma. METHODS: A 55-year-old man had a rapidly growing pedunculated large mass on the sacrolumbar junction for 2 years. He has congenital neurofibromatosis type I. He had multiple caf-au-lait spots and multiple neurofibromas on the entire body. The mass developed from a subcutaneous nodule on the sacrolumbar junction and grew rapidly. The preoperative punch biopsy revealed a malignant peripheral nerve tumor. The mass was completely excised with 1 cm free margin above the deep fascial plane. RESULTS: There was no evidence of recurrence of tumor for 19 months of follow-up examination. CONCLUSION: Malignant peripheral nerve sheath tumor is very rare and has unique feature. We report a successful case of malignant peripheral nerve sheath tumor with the review of the literatures.

8.Lumbar Discectomy Using Tubular Retractor and Microendoscopy.

STUDY DESIGN: Retrospective study OBJECTIVES: To compare the surgical results of microendoscopic discectomy using a tubular retractor with those of a conventional discectomy for lumbar herniated nucleus pulposus. SUMMARY OF LITERATURE REVIEW: Minimally invasive approaches to the lumbar spine have been attempted with success over the past 25 years. MATERIALS AND METHODS: This study examined 36 cases who underwent lumbar discectomy using a tubular retractor and microendoscopy (Group A) and 30 cases who underwent a conventional discectomy (Group B). The operating time, intraoperative blood loss, postoperative hospital stay were recorded. The clinical results were assessed using the Korea Version Oswestry Disability Index (KODI), and the radiological results were evaluated from changes in disc height. RESULTS: The patients in Group A had a longer operation time(mean, 95.56+/-23.57 minutes vs 81.17+/-35.30 minutes, p=0.062), less intraoperative blood loss (mean, 58.61+/-97.08 cc vs 161.00+/-88.64 cc, p=0.001) and a shorter hospitalization stay (mean, 8.22 +/-4.99 days vs 17.33+/-10.98 days, p=0.001) than group B. There was significant improvement in the mean KODI score for Group A; 6.36+/-7.18 and Group B; 5.97+/-5.14. However, there was no significant difference in the pain improvement (mean, 1.14 +/-1.15 vs 1.30+/-1.06, p=0.559) and walking index (mean, 0.25+/-0.44 vs 0.30+/-0.47, p=0.656). In terms of social life, Group A had better results (mean, 0.20+/-0.48 vs 0.67+/-1.01, p=0.018). The disc height according to radiography decreased from 8.44+/-1.98 mm to 7.40+/-1.59 mm in Group A and 9.07+/-1.93 mm to 7.67+/-1.90 mm in Group B, but there were no statistical differences in the changes in disc height between the two groups (p=0.143). CONCLUSION: Microendoscopic discectomy is an effective procedure with good outcomes in treating lumbar disc herniation that allows less tissue trauma, compared with conventional open discectomy.

The urinary bladder is seldom injured by external forces, as it is located in the pelvic cavity, behind the symphysis pubis, and protected by the bony pelvis. Gynecological procedures are a major cause of bladder injury. Herein, a case of extraperitoneal bladder rupture, identified 6 days after vaginal delivery in a 27-year-old woman, with no history of cesarean section or pelvic surgery, and who was cured of the bladder rupture using conservative treatment with the indwelling of a urethral Foley catheter, is reported.
