2.The Experience of Surgical Management for Parathyroid Hyperplasia.

Parathyroid hyperplasia is a pathologic finding that can be found in hyperparathyroidism. Unlike parathyroid adenoma, treatment of parathyroid hyperplasia is still quite controversial. In addition, the relative merits of two alternative surgical approaches-subtotal parathyroidectomy versus total parathyroidectomy with autotransplantation have not been clearyly elucidated. The records of 10 patients who had parathyroid hyperplaisa and who underwent parathyroid surgery at the Department of Surgery, Hanyang University Hospital, between April 1992 and April 2003 were retrospectively reviewed. The 10 patients were comprised of 3 males and 7 females. The age distribution was between 29 and 73 years. The presenting clinical manifestations were associated with bone pain in 8 patients, muscle weakness in 4, headache in 4, gastrointestinal symptoms in 3, renal symptoms in 3 and psychologic symptoms in 2. The serum parathyroid hormone level was elevated in all patients. The serum alkaline phosphatase level was elevated in seven among the ten patients. Histopathologic findings revealed chief cell hyperplasia in all patients. Postoperative transient hypocalcemia occurred in 5 patients and they were supplied with oral calcium and calcitriol for several months. There were no major complications. The results indicate that a subtotal parathyroidectomy can be performed without mortality or morbidity and provides good control of primary parathyroid hyperplasia, A total prathyroidectomy with autotransplantation can be performed without mortality or morbidity and provides good contril of secondary and tertiary parathyroid hyperplaisa.

5.The Efficacy of Recombinant Human Follicle Stimulating Hormone (rhFSH) in Human IVF-ET Program.

OBJECTIVES: Recently, recombinant FSH (rFSH) has been manufactured using a Chinese hamster ovary cell line transfected with the gene encoding human FSH. Both rFSH and urinary gonadotropin (uFSH) could be used for controlled ovarian hyperstimulation (COH). However, uFSH implies a number of disadvantages, such as batch-to-batch inconsistency, no absolute source control, dependence on large amounts of urine, low specific activity, and low purity. The purpose of this study was to evaluate the efficacy of rFSH in human IVF-ET program. MATERIALS AND METHODS: A total of 508 infertile women was enrolled in this study. They are classified into rFSH group (n=177) or uFSH group (n=331), and all of them were matched by age and cause of infertility in same period. The Puregon(R) (Organon, Holland) was used as rFSH, and the Metrodin-HP(R) (Serono, Switzeland) and Humegon(R) (Organon, Holland) was used as uFSH. We subdivided the patients into three age groups. The outcomes of IVF-ET program were analyzed using the statistical package for social sciences (SPSS). RESULTS: There was no significant differences in the level of estradiol on hCG injection day, the numbers of retrieved oocytes, matured oocytes, fertilized oocytes, transferred embryos, frozen embryos between the two groups. The total dose (IU) of gonadotropin for COH was significantly lower in the rFSH group compared to uFSH group (1339+/-5491.1 vs 2527.8+/-1075.2 IU, p<0.001). Clinical pregnancy rate per embryo transfer in the rFSH group showed increasing tendency, compared to the uFSH group, but there was no statistical significance (35.2% vs 29.3%). Our results demonstrated that the relative efficiency of rFSH compared with uFSH is higher in older patients. CONCLUSIONS: The ovarian stimulatory effect and clinical outcome of recombinant FSH was similar to that of the urinary gonadotropin. The IVF-ET cycles with significantly lower dose of gonadotropin in rFSH group showed comparable results. Therefore, we suggest that recombinant FSH is more potent and effective than urinary gonadotropin.

6.The Effect of Vecuronium on the Intraocular Pressure.

In ocular surgery, many studies have been carried but in order to evaluate intraocular pressure chance during induction and maintenance of anesthesia. In this study, Vecuronium (Norcuron), new monoquaternary analogue of pancuronium, has been used in a group of patient undergoing elective sugery in order to evaluate its influe-nce on intraocular pressure. 40 patients without a history of eye digease, in age from 20 to 60 years, were evaluated. Their condition fell into ASA class l. On arrival, initial measurement of intraocular pressure(as control value) was taken with Shiotg tonometer after instilling a 0.5% pontocaine into conjuntival sac. Subsequent measurements were taken after thiopental injeotion as soon at the lid reflex was lost, then 2 minutes after Vecuronium injection, 1,3,5,10 and 20 minutes after endot-racheal intuhation. Blood pressure and pulse rate were measured together. The results were as follows, 1. At 2 minutes after intubation, intraocular pleasure was decreased as compared with the control value. 2. At 1 minute after intutation, intraocular pressure was more increased than the value of 2 minutes after vecuronium injection. but was not significantly changed as compared with control valule. 3. All values measured followed 3 minutes after intubation were significantly derceased. 4. Mean arterial pressure and pulse rate were increased at 1 minute after intubation but all the other values were not significantly changed as compared with control value.

7.Successful Development of Cordyceps bassiana Stromata from Beauveria bassiana.

A specimen of Beauveria bassiana was collected from Yang-yang of Gangwon province, Korea in October 2006. Conidial isolates were prepared from the specimen by the dilution method and inoculated in brown rice medium for fruiting body production. After nearly two months incubation for perithecial stromata developed from single isolates as well as from their combinations. They were determined as Cordyceps bassiana by observing the stromatal characters and their conidial structures. This is the first report of the development of C. bassiana from B. bassiana cultures.

9.Acute Abdominal Pain in Patients with Hemorrhagic Fever with Renal Syndrome in the Emergency Department.

PURPOSE: Severe abdominal pain and signs of peritoneal irritation in some patients with hemorrhagic fever with renal syndrome (HFRS) can mimic an acute surgical disease of the abdomen. We performed this study to analyze the clinical features and the laboratory and radiographic findings of HFRS patients who had acute abdominal pain upon visiting the emergency department (ED). METHODS: The electronic medical records were retrospectively investigated during a 3 year period between January 2006 and December 2008. RESULTS: Among the 44 patients with HFRS at the ED, 21 patients (47.7%) complained of acute abdominal pain. On physical examination, three patients (14.3%) had rebound tenderness. The most common laboratory findings were thrombocytopenia, azotemia and proteinuria. Ascites, pericholecystic fluid collection, peri-renal fluid collection and pleural effusion were the major findings of abdominal computed tomography. CONCLUSION: Acute abdominal pain can be a presenting symptom of HFRS at the ED. In an endemic area, HFRS should be suspected in young patients who have acute abdominal pain, fever (or history of fever), tenderness (and rebound tenderness) of the abdomen, thrombocytopenia and proteinuria. Failure to recognize HFRS might delay the appropriate diagnostic approach and treatment.
