Objective To explore the related pelvic floor anatomy to the pathological vaginal relaxation and key points of the vaginal tightening surgery. Methods The vaginal tightening surgery was performed in 24 cases of vaginal relaxation. The key points of this operation included levator ani muscle suturation and perineal body reconstruction, and anal sphincter reconstruction as in case of the muscle injury grade Ⅲ.Results The degree of levator ani muscle separation was positively correlated with that of vaginal relaxation in all the 24 cases.18 cases were followed up from 6 months to 2 years, and had no complications of rectovaginal fistula and infections. The average level of perineal body was increased from 2.3 cm to 3.5 cm.Vaginal length of 6 cm from vaginal orifice was proper with good tightness. The patients felt strong anal contraction, enhanced ability of vaginal tightening and improved quality of sex life. There were no more infections of genitourinary tract. Conclusions Through levator ani muscle suturation and perineal body reconstruction,it can get the vaginal tightening effect.

Informed consent is an important ethical symbol in clinical research, and researchers have the responsibility to fully inform participants of the research information before conducting clinical research. However,it is difficult to obtain complete informed consent form participants or their guardians within a narrow treatment time period in clinical research conducted in emergency situations.Currently,in addition to traditional general informed consent, there are also reality-accepted informed consent, including exemption of informed consent, broad informed consent, and deferred informed consent.By introducing the origin and development process of deferred informed consent in clinical research, this paper sorted out the current application status of deferred informed consent, proposed the prerequisites for applying deferred informed consent in emergency situations, and explored the issues that need to be noted during the application process of deferred informed consent. It is hoped to provide an ethical defense and ethical procedure for the application of deferred informed consent in clinical research in emergency situations.

Drugs under special management include narcotic drugs, psychotropic drugs, toxic drugs for medical use, radiopharmaceuticals, and pharmaceutical precursor chemicals.Supervising and guiding the clinical use of drugs under special management is one of the important responsibilities of the Pharmaceutical Management and Drug Therapy Committee(Group)of medical institutions. The standard for drugs under special management is led by the Pharmaceutical Professional Committee of the China Hospital Association, which standardizes 16 key elements of organizational management, process management, and quality control management drugs under special management in medical institutions. It can guide the standardized implementation of Pharmaceuticals under special control work in various levels and types of medical institutions. This article elaborates on the methods and contents of formulating standards for Pharmaceuticals under special management,to provide reference and inspiration for medical institutions to carry out special drug drug management and daily related work.

OBJECTIVE To establish an analytical method based on ultra-performance liquid chromatography-quadrupole-time- of-flight mass spectrometry (UPLC-Q-TOF/MS) combined with UNIFI platform for rapid identification of the chemical constituents in Abrus cantoniensis Hance. METHODS The chromatographic separation was performed on ACQUITY PRM HSS T3 FIT column for gradient elution with the mobile phase consisted of 0.1% formic acid aqueous solution-acetonitrile. The flow rate was 0.3 mL/min, and injection volume was 2 μL. Electrospray ionization was used to collect the mass spectrometry data of the chemical constituents of A. cantoniensis Hance with full information tandem mass spectrometry technology in positive and negative ion modes. The chemical constituent database of A. cantoniensis Hance was established. Targeted and non-targeted analyses were conducted based on UNIFI platform, and chemical constituents were further identified in combination with accurate molecular mass, secondary fragment ion information and equivalence with reference substances and literature data, etc. RESULTS Totally 46 compounds of A. cantoniensis Hance were successfully identified, including 19 flavonoids, 8 triterpenoids, 3 alkaloids, 5 organic acids and 11 other components. Among them, 11 compounds were firstly found in A. cantoniensis Hance, and 9 compounds were confirmed by reference substance. CONCLUSIONS The analytical method based on UPLC-Q-TOF/MS combined with UNIFI platform can quickly identify the chemical constituents of A. cantoniensis Hance. Flavonoids and triterpenes are the main components in A. cantoniensis Hance.

OBJECTIVE To analyze the chemical components that were the absorbed in blood and liver tissue of rats after intragastric administration of aqueous extract from Abrus cantoniensis, and to speculate its possible metabolic pathways, providing reference for basic analysis of pharmacological substance in A. cantoniensis. METHODS Male SD rats were randomly divided into A. cantoniensis group (0.63 g/kg, calculated by crude drug) and blank group; they were given relevant drug solution/ultrapure water intragastrically. After a single dose, plasma and liver samples of rats in each group were collected. UPLC-Q-TOF/MS technology was used to identify chemical components that were absorbed in the blood and liver tissue of rats. RESULTS Totally, 30 chemical constituents were identified from the water extracts of A. cantoniensis, including alkaloids, flavonoids, organic acids, iridoids (such as L-abrine, schaftoside, isoshaftoside). Ten prototype components and nine metabolites (such as decarboxylation and sulfation metabolites of protocatechuic acid, reduced sulfated metabolites of p-hydroxybenzoic acid) were identified from plasma samples; six prototype components and five metabolites (such as sulfated metabolites of p-hydroxybenzoic acid, decarboxylation and sulfation metabolites of p-hydroxybenzoic acid) were identified from liver samples. The main metabolic pathways included hydroxylation, demethylation, methylation, sulfation, glucuronidation, etc. CONCLUSIONS Alkaloids, flavonoids and organic acids are the main components of the aqueous extract from A. cantoniensis that are absorbed into the blood and liver, their metabolism mainly involves hydroxylation, demethylation, and sulfation.
