2. Clinical study on the changes in magnetic resonance spectroscopy of cerebellum in patients with post-stroke depression

Objective To study the metabolic changes of cerebellum in patients with post-stroke depression (PSD) by proton magnetic resonance spectroscopy (1H-MRS) and discuss the relationships between the cerebellar changes and depression severity. Methods Stroke patients were selected as the subjects in present study. Data of demographic characteristics, individual history and life style of all subjects were collected. Forty patients with stroke and 20 healthy volunteers as control (NORM group) were enrolled. Hamilton Depression Scale (HAMD) and Self-rating Depression Scale (SDS) were used to test the subjects. According to the scores of depression severity, the stroke group was further divided into a group with post-stroke depression (PSD group) and a group without post-stroke depression (CONT group). All the patients of 2 groups received T1WI, T2WI, DWI and 1H-MRS examination. The cerebral infarction volume and the distribution and severity of leukoaraiosis were evaluated. The ratios of NAA/Cr, Cho/Cr and Cho/NAA in the cerebellum were calculated. Moreover, the relationships between the ratios and the scores of HAMD were analyzed in all post-stroke patients. Results There was no significant difference in the ratios of NAA/Cr, Cho/Cr and Cho/NAA in bilateral cerebellum between CONT group and normal control (NORM) group (P>0.05). The Cho/Cr and Cho/NAA ratios in the cerebellum contralateral to the stroke region were higher in PSD group than in NORM and CONT groups (P<0.05), and the Cho/Cr and Cho/NAA ratios in the cerebellum ipsilateral to the stroke region were similar with those in NORM and CONT groups (P>0.05). However, there was no significant difference in the NAA/Cr ratios in bilateral cerebellum among three groups (P>0.05). The family APGAR questionnaire survey scores, age-related white matter changes (ARWMC) scale total scores, 14-day National Institutes of Health Stroke Scale (NIHSS) scores, and the Cho/Cr and Cho/NAA ratios in the contralateral cerebellum were associated with post-stroke depression. Multiple linear regression analysis showed the Cho/Cr and Cho/NAA ratios were related to the HAMD scales (P<0.05). Conclusion The results preliminarily reveal that the cerebellum involves in the development of post-stroke depression.
3.Effect of high altitude hypoxia on cognitive flexibility.

OBJECTIVETo explore the effects of high altitude on cognitive flexibility.
METHODSSimulated hypoxia at an altitude of 3 600 m was performed in a hypobaric chamber. Twenty-three volunteers without hypoxic experience were selected and the mean age was about 25.1 years. The physiological parameters (heart rate, blood pressure and oxygen saturation) were measured. Task switch paradigm was used to explore the cognitive flexibility in each phase, and the changing anxiety state was evaluated simultaneously.
RESULTSReaction time (RT) switch cost in hypoxia phase showed a significant increase compared with the baseline; anxiety level in hypoxia phase was higher than the adaptation phase; a remarkable negative correlation between anxiety level and RT switch cost was found in adaptation phase, whereas a positive correlation was found in landing phase.
CONCLUSIONHigh altitude (3 600 m) affects cognitive flexibility and anxiety state. Anxiety before the hypoxia exposure improves the cognitive flexibility performance, while anxiety after the hypoxia exposure hampers the performance because of the post-hypoxia effect.

4.Study on physical activities status of citizens from different subpopulation in Pudong, Shanghai.

OBJECTIVETo understand the levels of total physical activities among different populations, and the distribution of four domains.
METHODSWith proportional stratified sampling and cluster sampling method, 3555 subjects were selected including officials, company staff, high school students, community population and floating population.
RESULTSThe proportions for inactive, insufficiently active, minimally active and health enhancing physical active (HEPA active) as a whole were 20.5%, 10.1%, 26.5% and 42.9% respectively. Community population had the highest level of HEPA active which was 65.6%. Floating population had the highest level of inactivity of 33.1%. Apart from the male floating population, the active level in transportation, physical activity among the investigated were the highest compared to other three active domains. Leisure-time physical activity was in the opposite.
CONCLUSIONThe type of physical activities among general citizens were mainly of physical activity related to transportation but less leisure-time physical activity was seen. Floating population had the highest level of both HEPA activity and inactivity.

5.Effects of drug combination on optic disc parameters and retinal nerve fiber layer thickness of high intraocular pressure type POAG patients

·AIM:To study the effects of timolol and latanoprost on optic disc parameters and retinal nerve fiber layer thickness of high intraocular pressure type POAG patients, and to provide guidance for clinical research.·METHODS: Totally 240 patients ( 336 eyes ) with high intraocular pressure type POAG in our hospital from November 2013 to November 2015 were randomly divided into control group and observation group, and each with a total of 120 cases (170 and 166 eyes, respectively). The patients in control group only received latanoprost treatment, while the observation group was treated with timolol and latanoprost treatment. After 3mo of treatment, we observed the therapeutic effects, and measured the optic disc parameters and retinal nerve fiber layer thickness. The incidence of adverse reactions was observed in the following 1a.·RESULTS: The patients of observation group and the control group after treatment showed the improvement rates of 97. 6% and 80. 6%, respectively, and the observation group was significantly better than the control group, the two groups showed statistically significant (P<0. 05). The eye rim area, rim volume, rim volume and diameter of two groups after treatment were higher than before ( P < 0. 05 ) , while those of the observation group was significantly higher than control group ( P<0. 05 ) . Two groups of patients with vertical cup to disc diameter decreased obviously ( P<0. 05 ) , the observation group was significantly lower than the control group, the difference was statistically significant ( P<0. 05 ) . Between the two groups of patients, during the period, incidences of dry eyes, corneal and conjunctival hyperemia, infiltration blurred vision and other complications had no significant difference ( P>0. 05), and the 1a follow-up of patients showed no other adverse reactions.·CONCLUSION: The use of timolol and latanoprost therapy for high intraocular pressure type POAG patients showed that the patients improve obviously, the visual acuity has more improved and the parameters of optic disc and retinal nerve fiber layer thickness are also improved.
6.Development of new radial artery puncture and fixation device

Objective To develop a new radial artery puncture fixing device to conquer the deficiencies of traditional method. Methods The device was composed of an arm support,a hand support,a wrist part,an arm fixing belt,a palm fixing belt and a tourniquet.The arm support was concave-shaped,and was connected with the hand support with the wrist part.The wrist part had a raised block which was gifted with an inflatable bag.The balloon linked with an inflation balloon with an inflation tube.There was a valve between the inflation tube and balloon.The fixing belt had one end directly connected with the arm support, hand support and wrist part, and the other end linked with the other ends of the above components with Velcro. There was a compression hemostatic balloon at the internal surface of the tourniquet. Results The device behaved well in puncture time and success rate,and decreased the incidence rates of errhysis and hematoma after withdrawing the puncture needle while increased the satisfaction of medical staffs and patients. Conclusion The device gains advantages in simple structure,convenience,practicability and safety,and meets the desired requirements.[Chinese Medical Equipment Jour-nal,2018,39(5):44-46]
7.Treatment of Retrogastric Pancreatic Pseudocysts by Laparoscopic Transgastric Cystogastrostomy

This paper discusses variations of laparoscopic transgastric cystogastrostomy in management of retrogastric pancreatic pseudocysts for 8 patients with symptom or pseudocysts (larger than 6 cm) companied with clinical manifestations.Using a Harmonic scalpel,two 3-5-cm incisions were made in the anterior and posterior gastric wall respectively.In the last step,the anterior gastrotomy was closed with an Endo-GIA stapler.All cases were successfully treated without large blood loss and without conversion to open surgery.The mean operative time was 114.29±19.24 min,blood loss was 157.14±78.70 mL,and mean hospital stay was 8.29±2.98 days,Gastric fistula occurred in one case on the postoperative day 7,and closed 1 month later.No bleeding was seen in all patients during the perioperative follow-up period.CT scans,given one month after the surgeries,displayed that the pancreatic pseudocysts disappeared or decreased in size,and ultrasounds showed no fluid or food residue in stomas at the third and fifth month following surgery.No patient experienced a recurrence during the follow-up period.Transgastric laparoscopic cystogastrostomy is a minimally invasive surgical procedure with a high rate of success and a low rate of recurrence,accompanied by rapid recovery.It is easy to master,safe to perform and may be the preferred option to treat retrogastric pancreatic pseudocysts.
8.The toxic and protective effects of Polygonum multiflorum on normal and liver injured rats based on the symptom-based prescription theory.

The dosage-efficacy/toxicity relationship of the 50% alcohol extracts of Polygonum multiflorum was comparatively investigated on either normal or CCl4-induced chronic liver injury rats, by determining the general condition, serum biochemical indices and liver histopathology, coupled with the factor analysis. The dosages were 10 and 20 g raw materials per kg body weight. Compared with the normal control group, the normal high dose group showed significant increases of the serum alanine transaminase (ALT), total bilirubin (TBIL), high mobility group box 1 (HMGB-1) and interleukin-1β (IL-1β) (P < 0.05 or P < 0.01), as well the frequent incidences of inflammatory cell infiltration, hepatic sinus enlargement and fiber stripes formation in histopathological sections. Compared with the model control group, the model low dose group showed significant declines of serum ALT, aspartate transaminase (AST) and total bile acid (TBA) (P < 0.05), as well the alleviation of vacuoles of hepatocytes, but no amelioration of the inflammatory cell infiltration and fibrous tissue hyperplasia; moreover, the model high dose group showed significant degeneration declines of serum HMGB-1, tumor necrosis factor-α (TNF-α) and IL-1β (P < 0.05, P < 0.01), as well the evident alleviation of vacuoles degeneration of hepatocytes, inflammatory cells infiltration and fibrosis degree. The factor analysis showed that the low dosage treatment had almost neither injuring effect on the normal rats nor protective effect on the model rats; while the high dosage treatment showed observable injuring effect on the normal rats, expressed by the significant increases of the factor-1 (HMGB-1, TNF-α and IL-1β as the main contributors) and factor-2 (TBIL, ALT and TBA as the main contributors) relative to the normal control group. The liver protective effect of the high dosage treatment could be observed with the significant reduction of the factor-1, indicating the effective alleviation of the expression of inflammatory cytokines. In conclusion, it could illustrated the phenomenon of symptom-based prescription theory of Polygonum multiflorum on rat livers: the high dosage of the herb had either an injuring effect on normal rats, or a therapeutic effect on the rats with chronic liver injury.

9.Application of~(125)I seed permanent plantation in osseous metastases

Objective To evaluate the value of ~(125)I permanent plantation in treating osseous metastases.Methods Twenty-two patients with osseous metastases were accepted radioactive seeds ~(125)I permanent plantation.The curative effect was appraised according to the degree of ostalgia relieving and the changing of the radiology imaging in patients.Results Accepted radioactive seeds ~(125)I permanent plantation,relief of pain was obtained and the effective rate is 91%(20/22).However none of the patients showed severe side-effect.Among 32 lesions in 22 cases followed-up by CT in 2 months,4 obtained CR, 18 obtained PR,10 NC and 0 PD.The responsive rate was 68.7%.Conclusion ~(125)I permanent plantation procedure can be a safe and effective method in treating osseous metastases and obtaining good clinical effects with minimal damage and few comnlications.
10.Comparative study on preparation of Polygoni Multiflori Radix based on hepatotoxic bioassay.

Toxicity of different processed was evaluated Polygoni Multiflori Radix by determining the hepatotoxic potency for selecting processing technology. Process Polygoni Multiflori Radix using high pressure steamed, Black Bean high pressure steamed, atmospheric steamed for different time. Using normal human hepatocytes (L02) as evaluation model, hepatotoxic potency as index to evaluate hepatotoxic potency of different processed Polygoni Multiflori Radix. Analysis chemical composition of some processed products by UPLC-MS. Hepatotoxic bioassay method cloud evaluate the toxicity of different Polygoni Multiflori Radix samples. Different processing methods can reduce the toxicity of Polygoni Multiflori Radix, high pressure steamed three hours attenuated was better. Different processing methods have different effects on chemical constituents of Polygoni Multiflori Radix. Comparing with crude sample, the contents of gallic acid, 2,3,5,4-tetrahydroxyl diphenylethylene-2-O-glucoside, emodin-8-O-beta glucoside and emodin were decreased in processed products with 3 kinds of different methods. The change trend of 2,3,5,4-tetrahydroxyl diphenylethylene-2-O-glucoside content was similar with hepatotoxic potency. Different processing methods can reduce the toxicity of Polygoni Multiflori Radix. Processing methods and time attenuated obvious impact on toxicity. Recommended further research on the attehuated standard control of Polygoni Multiflori Radix concocted.
