1.Clinical research of domestic image-guided radiotherapy system XGS-10 on intensity modulated radiotherapy of nasopharyngeal carcinoma

Objective To evaluate the application value of domestic image‐guided radiotherapy system XGS‐10 made in Weidu medical company limited in intensity‐modulated radiotherapy for nasopharyngeal carcinoma .Methods A total of 30 nasopharyngeal carcinoma cases with intensity modulated radiatiotherapy by Varian accelerator were incorporated into the study .All patients ac‐cepted image‐guided radiotherapy system XGS‐10 of Chongqing Weidu medical company limited to obtain frontal images and CBCT to obtain CT images respectively .The images were matched with the planning images and target center ,then got the linear set‐up errors of left to right (X) ,superior to inferior (Y) ,and anterior to posterior (Z) ,and the time of acquiring and matching was recor‐ded .Results The correlation of both two instruments′setup errors in X ,Y ,Z direction is good ,the difference of setup errors was within the acceptable range ,and was not statistically significant (P>0 .05) .the time of acquiring images was (15 ± 2)s and that of matching images was (20 ± 2)s by XGS‐10 ;the time of acquiring images was (120 ± 8)s and that of matching images was (240 ± 10)s by Varian CBCT .Conclusion The setup error of XGS‐10 of intensity‐modulated radiotherapy for nasopharyngeal carcinoma was equal to CBCT ,but the time of acquiring and matching time of XGS‐10 was shorter than that of CBCT ,which could shorten the positioning time and reduce radiation dose for patient .As a result ,it is conducive to real‐time clinical radiotherapy guidance .
2.Establish Assessment Model of 18 Years of Age in Chinese Han Population by Mandibular Third Molar.

OBJECTIVE:
To explore the value of estimating chronologic age based on the grades of mandibular third molar development. To evaluate whether mandibular third molar could be used as an indicator for estimating the age under or over 18 years.
METHODS:
The mineralization status of mandibular third molar of 1 845 individuals aged 10 - 30 was graded and marked based on Demirjian's classification of grades reformed by Orhan. Gender difference was examined by t-test. A cubic regression model was established to analyze the correlation between third molar and chronologic age. Each grade of age cumulative distribution diagram and ROC curve was respectively performed to evaluate the relationship between third molar and the age of 18. Using Bayes discriminant analysis, an equation was established for estimating the age of 18.
RESULTS:
The inner-rater reliability was 0.903. Statistical analysis showed a moderate correlation between age and grade. Significant differences of both genders were found only in grade D and H (P < 0.05). Males at the grades from 1 to D and females at the grades from 1 to C were under 18 years old, and both males and females at grade H were over 18 years old. The area under the ROC curve was 0.797 (P < 0.05).
CONCLUSION
Third molar development shows a high correlation with age, and combined with other indicators, it can be used to estimate the age of 18.

3.The mobilization of autologous bone marrow stem cells in the treatment of heart failure with Chinese medicine.

Heart failure (HF) is a severe heart disease. The use of autologous bone marrow stem cells (BMCs) mobilization in the treatment of HF has been a hot topic to research both in Western medicine and Chinese medicine (CM). There are many clinical trials and experiments on study of BMCs mobilization for HF therapy, including integrative medicine. The effect of BMCs mobilization is favorable for cardiac repair, while some advantages of CM support the advanced study of its application in BMCs mobilization to treat HF. In addition, with mechanisms of autologous BMCs mobilization for the treatment of HF that will be revealed in the future, especially stem cells niches, integrative medicine would play an important role in this clinical thought of therapy model gradually. Simultaneously, CM should adapt the new approaches of stem cells progresses on HF treatment as holding characteristics of itself.

4.Application of~(125)I seed permanent plantation in osseous metastases

Objective To evaluate the value of ~(125)I permanent plantation in treating osseous metastases.Methods Twenty-two patients with osseous metastases were accepted radioactive seeds ~(125)I permanent plantation.The curative effect was appraised according to the degree of ostalgia relieving and the changing of the radiology imaging in patients.Results Accepted radioactive seeds ~(125)I permanent plantation,relief of pain was obtained and the effective rate is 91%(20/22).However none of the patients showed severe side-effect.Among 32 lesions in 22 cases followed-up by CT in 2 months,4 obtained CR, 18 obtained PR,10 NC and 0 PD.The responsive rate was 68.7%.Conclusion ~(125)I permanent plantation procedure can be a safe and effective method in treating osseous metastases and obtaining good clinical effects with minimal damage and few comnlications.
5.Relationship between dyslipidemia and gene polymorphism in Tibetan population.

OBJECTIVETo investigate the relationship between SNPs reported in previous studies and the blood lipid level in the Tibetan population.
METHODSRandom cluster sampling was employed in 5 areas (Lhasa, Shigatse, Shannan, Nagqu, and Nyingchi). The levels of cholesterol (TC), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C), and low-density lipoprotein cholesterol (LDL-C) from blood samples were determined and DNA was extracted for genotyping and statistical analyses.
RESULTSAmong 1 318 subjects aged >18 years enrolled in this study, 367 had dyslipidemia with a prevalence of 27.8%, of whom dyslipidemia males accounted for 33.1% and dyslipidemia females -24.5%. Results of the correlation analysis between all SNPs and TG showed that the SNPs of rs714052 and rs964184 were related to the serum TG level. Subjects with rs714052 CC genotype had the lowest TG level, and the highest TG level was found in those with rs714052 TT genotype. The serum TG level in individuals with TC genotype lied in between the above two population groups. Subjects with rs964184 CC genotype had the lowest TG level, and the highest serum TG level was noted in those with rs964184 GG genotype.
CONCLUSIONSeveral SNPs were found to be related to the serum TG level in the Tibetan population. The APOA5 gene and MLXIPL gene may be closely associated with the serum TG level in this ethnic population group.

6.Risk Assessment of Echocardiography Parameter Scoring System for 1 Year Re-admission in Patients With Left Ventricular Systolic Dysfunction

Objective: To establish an echocardiography parameter scoring system for assessing the risk of 1 year re-admission in patients with left ventricular systolic dysfunction (LVSD). Methods: A total of 412 chronic LVSD patients treated in our hospital from 2007-01 to 2016-01 were studied and the end point event was 1 year re-admission. The data included in 280 patients from 2007-01 to 2014-12 for establishing the scoring system and 132 patients from 2015-01 to 2016-01 for verifying the system. Based on 7 echocardiography parameters, the patients were divided into 7 sets of groups: ① Left ventricular diameter (LVD): Group0, n=290 and Group1, n=122;② Mitrial regurgitation (MR): Group0, n=203, Group1, n=138 and Group2, n=71; ③ Tricuspid regurgitation (TR): Group0, n=302, Group1, n=90 and Group2, n=20; ④ LVEF: Group0, n=272 and Group1, n=140; ⑤ Pulmonary artery systolic pressure: Group0, n=282 and Group1, n=130; ⑥ Hydropericardium: Group0, n=347 and Group1, n=65; ⑦ Hydrothorax:Group 0, n=261, Group1, n=86 and Group2, n=65. The parameters were identified by COX regression analysis, weighted value of scoring system was calculate by hazard ratio (HR), predictive value for1 year re-admission was assess by ROC curve and finally, scoring integration was verified by validation data group. Results: The integration score was calculated as follows: LVD>60mm=1 point; TR: Group1=1 point and Group2=3 points; MR: Group1=2 points and Group2=4 points; Hydrothorax: Group1=2 points and Group2=3 points;Hydropericardium=1 point. COX regression analysis indicated that for 1 year re-admission: HR=1.552 in Group1 vs Group0, HR=3.374 in Group2 vs Group0 and HR=4.562 in Group3 vs Group0, all P<0.05. The AUC of ROC for establishing the data was 70.0% (95% CI 0.640-0.761) and for verifying the data was 70.4% (95% CI 0.616-0.792); the best integration score was 4 points. Conclusion: Echocardiography parameter scoring system may better predict the risk of 1 year re-admission in LVSD patients which is superior to single echocardiography parameter.
7.Protective effect of lytic cocktail (inhibition of stress) on lung injury in severe burn rat.

OBJECTIVETo investigate the protective effect of inhibition of stress (lytic cocktail) on lung injury in severe burn rats at early stage.
METHODSSprague-Dawley rats inflicted with 30% TBSA full-thickness burn were randomly divided into A group (n = 36, fluid resuscitation with administration of lytic cocktail), B group (n = 36, fluid resuscitation only). Lung function was evaluated by partial pressure of oxygen (PaO2) in arterial blood and histopathologic changes on 3, 5, 7, 10 post burn day (PBD). The levels of malonyldialdehyde (MDA), myeloperoxidase (MPO), tumor necrosis factor-alpha(TNF-alpha) and interferon-gamma (IFN-gamma) in lung tissue were measured at the same time points.
RESULTSThe PaO2 level in A group on 3 PBD (12.58 +/- 0.41 kPa) was significantly higher than that in B group (8.86 +/- 0.23 kPa, P < 0.01). Compared with those in B group, the levels of MDA and MPO were significantly decreased in A group at each time point (P < 0.05 or 0.01), the levels of TNF-alpha on 3, 5, 7 PBD (P < 0.05 or 0.01) and IFN-gamma on 5, 7, 14 PBD (P < 0.01) were also decreased in A group. Swollen lung mesenchyme was alleviated, infiltration of inflammatory cell was lessened in A group.
CONCLUSIONLytic cocktail combined with immediate fluid resuscitation can inhibit stess response, downregulate the expression of inflammatory factor, ameliorate lung function in severe burn rat at early stage.

8.Influence of stress inhibition on invasive infection in burn wound during early postburn stage in severely burned rats.

OBJECTIVETo investigate the influence of inhibition of stress on the survival rate, organ dysfunction and (Th)1/Th2 cytokine profiles of the rats with invasive infection in the wound at early postburn stage.
METHODSSprague-Dawley rats inflicted with 30% TBSA full thickness burn were randomly divided into A (n = 36, with immediate resuscitation), B (n = 36, with immediate resuscitation and lytic cocktail administration). After subeschar injection of 0.1 ml Pseudomonas aeruginosa (10(8) CFU/ml) on 3rd postburn day, the subeschar bacterial quantitative analysis, the survival rate at 96 hours after bacteria injection, the parameters of organ dysfunction and the mRNA expression of IL-2, IL-4, IL-10 and IFN-gamma were determined by corresponding methods.
RESULTSThe quantity of subeschar bacteria was larger than 1 x 10(5)/gram in both groups. The survival rate in B group (66.7 +/- 2.6)% was obviously higher than that in A group (33.3 +/- 1.7)%, (P < 0.01). Inflammatory infiltration and pathological changes in the internal organs in B group were alleviated obviously compared with A group. The expression of IL-2 mRNA in B group was significantly lower than that in A group before bacterial inoculation, but increased at 48 and 96 hours after bacterial inoculation, while it was lowered in A group at the same time points (P < 0.05). The expression of IFN-gamma mRNA in A group was significantly lower than that in B group (P < 0.01), while that of IL-4 and IL-10 mRNA in A group was evidently higher than that in B group (P < 0.05 approximately 0.01).
CONCLUSIONInhibition of the stress response during early postburn stage could be beneficial to the prevention of the bacterial invasion due to the changes in Th1/Th2 ratio.

9.OCT4 expression in hepatocellular carcinoma and its clinical significance.

BACKGROUND AND OBJECTIVERecently, many studies have focused on stem cells in hepatocellular carcinoma (HCC) and found some stem cell markers in HCC, which are associated with the prognosis. OCT4, as a member of the POU transcription factor family, is a key factor to maintain self-renewal and pluripotency of embryonic stem cells (ESCs). This study was to explore the expression of the ESCs marker OCT4A in HCC, and its correlations with clinicopathologic features and prognosis of HCC.
METHODSOCT4A mRNA expression was detected in five liver cancer cell lines (SMMC-7721, BEL-7402, Hep-G2, MHCC97-L, and MHCC97-H), one immortalized liver cell line L-O2, tumor tissues with matched non-neoplastic liver tissues in 107 HCC patients, and normal liver tissues of 20 cases using semi-quantitative reverse transcription-polymerase chain reaction (RT-PCR). The correlations between OCT4A mRNA and clinicopathologic features and prognosis of HCC were analyzed.
RESULTSOCT4A mRNA was detected in SMMC-7721, BEL-7402, Hep-G2, MHCC-97L, and MHCC-97H cells, but not in L-O2 cells. The positive rate of OCT4A mRNA expression was significantly higher in the HCC tissues than in the non-neoplastic liver tissues (72.0% vs. 30.8%, P<0.001). No OCT4A mRNA expression was found in the normal liver tissues. OCT4A mRNA expression was correlated with the tumor size, vascular invasion, and TNM stage (P<0.05). Kaplan-Meier survival curves showed that patients with positive expression of OCT4A mRNA had lower overall survival and disease-free survival rates.
CONCLUSIONSOCT4A mRNA, which is highly expressed in a subset of liver cancer cell lines and HCC tissues, may be involved in the carcinogenesis of HCC. OCT4A mRNA may be a valuable biomarker for assessing the prognosis of HCC.

10.Association of single nucleotide polymorphism in human SCN7A gene with essential hypertension in Chinese.

OBJECTIVETo identify the single nucleotide polymorphisms (SNPs) in the regulatory and coding regions of human SCN7A (sodium channel, voltage-gated, type VII, alpha polypeptide) gene and to investigate the association of some of these SNPs with essential hypertension (EH) in Chinese.
METHODSThe promoter region, exons, as well as part of the introns of SCN7A gene were sequenced by a fluorescent labeling automatic sequencing method to identify and characterize the SNPs in Chinese population. SNP genotyping was performed by PCR-RFLP or direct DNA sequencing in unrelated EH patients and normotensive controls from a Chinese Han population residing in Shanghai area. Case-control studies on seven SNPs were first carried out in 96 patients and 96 normotensive controls. The positive finding was further verified in an extended study containing 288 patients and 288 controls.
RESULTSThirty-two SNPs were identified through a 13,132 bp sequencing of SCN7A gene. Among them, seven were in regulatory region, ten in coding regions, one in 3'UTR and fourteen in introns. Thirty SNPs were novel SNPs, and a cSNP in exon 18 (SNP021) was associated with hypertension.
CONCLUSIONThe SNP021 in the gene SCN7A is associated with essential hypertension of Chinese Han population in Shanghai and the role of SCN7A gene in hypertension deserves to be further analyzed.
