7.Relationship between clinicopathologic features and neoplasm recurrence,prognosis in hepatocellular carcinoma after liver transplantation

Objective To study the risk factors affecting neoplasm recurrence and survival of hepatocellular carcinoma(HCC)following liver transplantation(LT).Methods The clinicopatholo gic data,neoplasm recurrence and survival results of 118 patients with HCC receiving LT were retro- spectively analyzed and various clinicopathologic risk factors for neoplasm recurrence and survival were evaluated by univariate and multivariate analysis.Results The follow-up time ranged from 1 to 32 months.The recurrence rate was 37.3% and the mortality was 35.5%.The 12-,18-,24-month survival rate was 84.55%,70.30% and 62.24%,respectively.The 12-,18-,24-month neoplasm free survival rate was 69.05%,66.93% and 61.38%,respectively.In the univariate analysis,por- tal vein neoplasm thrombus(PVTT),Milan-criteria,neoplasm size,histological differentiation and pTNM stage were associated significantly with neoplasm recurrence,and PVTT,Milan-criteria,pre- operative alpha-fetoprotein(AFP),histological differentiation and pTNM stage were associated signif- icantly with survival rate;In the multivariate Logistic regression analysis,PVTT and histological dif- ferentiation were independent predictive factors of neoplasm recurrence,and multivariate Cox regres- sion analysis showed that PVTT and AFP independently associated with prognosis.Conclusions PVTT and histological differentiation are the most important predictive factors of neoplasm recur- rence,and PVTT and AFP independently predict the survival of patients undergoing LT.
8.Relationship between waist to hip ratio and static lung volumes in adults

AIM: The study is designed to probe for the relationship between waist to hip ratio(WHR)and static lung volumes of adults. METHODS: In July-October 2008, 1 307 healthy adults(372 males and 935 females)were selected in Heilongjiang province by means of questionnaire and physical examination. The height, weight, WHR, fat mass, percentage body fat and lung function were measured, and then grouped according to the standard of classification of WHR(central obesity male WHR ≥ 0.86, female ≥ 0.82)for analysis of the relationship between WHR and static lung volume. RESULTS: WHR was found, regardless of sex, to tend to go up with the increase in age and BMI(P<0.01), and both the fat mass and percentage body fat of the central obesity group were higher than those in the group with normal WHR(P<0.01). An independent negative correlation was found(P<0.05), also regardless of sex, between the WHR with expiratory reserve volume(ERV)in all these adults. ERV in central obesity group was lower than that in the group with normal WHR(P<0.05). Compared to the normal group, ERV in the central obesity group decreased by 11% for males and 8% for females(P<0.05). However, with regard to the relationship between WHR and VT, IC, MV, and VC, gender differences were found. For the males, a significant independent positive correlation was observed between WHR and IC(P<0.05), with IC of the central obesity group 6% higher than that in the group with normal WHR(P<0.05). For the females, significant independent positive correlation was found between WHR and MV(P<0.05), with the VT and MV of the central obesity group 7% and 6% higher(P<0.05), respectively, than that in the group with normal WHR. CONCLUSION: WHR is in an independent negative correlation with ERV. The elevation of WHR may play a role in the impairment of respiratory function. Its occurrence is accompanied by a rise of IC for the male and a rise of MV for the female. These changes in the two genders could be associated with the decrease in arterial oxygen tension caused by the decrease of ERV.

Helicobacter pylori(Hp) is an important pathogenic factor in chronic gastritis,peptic ulcer and gastric cancer.With the extensive use of broad-spectrum atibiotics in recent years,the antibiotic resistence of Hp was increasing,together with many side effets and low patient compliance,so the study of new anti-Hp drugs became a research hotspot.Domestic and foreign scholars had extracted active anti-Hp ingredients from the traditional Chinese drug curcuma,they hope to open a new way for the treatment of Hp infection.This paper was to make a brief overview on the study progress of effect of curcuma on anti-Hp.
10.Current treatment for intermediate-stage hepatocellular carcinoma

Staging of hepatocellular carcinoma (HCC) need to consider tumor factors and functional status of liver,which is different from other solid tumors.Now there are many clinical staging and prognosis systems of HCC.Intermediate stage HCC is commonly occurred at disease diagnosis,with varied clinicopathological charactertics,therapeutic options and survival outcomes of patients.In this review,the definition,current therapeutic strategies,substaging model of intermediatestage HCC and the efficacy of liver resection for patients of intermediate-stage HCC are discussed for further improving feasibility of individualized surgical therapy.
