1.Epidemiological characteristics of elderly pulmonary tuberculosis in Xianning, Hubei Province, 2011-2021

Abstract: Objective To analyze the incidence of pulmonary tuberculosis infection in the elderly(≥ 65 years old) in Xianning City from 2011 to 2021, and to provide scientific basis for formulating strategies and measures for tuberculosis prevention and treatment in the elderly. Methods Descriptive epidemiological analysis was used to analyze the epidemic characteristics and influencing factors of elderly (this article studies elderly people aged 65 and above) pulmonary tuberculosis in Xianning City, Hubei Province from 2011 to 2021. Results From 2011 to 2021, the average annual registration rate of elderly patients with pulmonary tuberculosis in Xianning City, Hubei Province was 211.52/100 000 (5 289/2 500 496), which showed a downward trend (χ2 trend=58.06, P<0.05), and significantly higher than the average registration rate of 96.04/100 000 patients with pulmonary tuberculosis (26 463/27 554 195)(χ2=2 896.69, P<0.05). The average proportion of the registered number of elderly patients with pulmonary tuberculosis in the whole population was 19.99% (5 289/26 463), which showed an upward trend (χ2trend=272.05, P<0.05). The average positive rate of etiology in elderly patients with pulmonary tuberculosis was 51.86% (2 743/5 289), which was significantly higher than that in the whole population 46.41% (12 282/26 463) (χ2=52.53, P<0.05). The average proportion of retreated patients in elderly patients with pulmonary tuberculosis was 8.92% (472/5 289), which was significantly higher than that of retreated patients in the whole population 6.33% (1 674/26 463) (χ2=47.23, P<0.05). The sex ratio of male to female in elderly patients with pulmonary tuberculosis was 2.92∶1. Among the six counties (districts) under the jurisdiction of Xianning City, Hubei Province the registration rate of elderly tuberculosis patients in Xian'an district was the highest 262.20/100 000 (1 267/483 220), and that in Tongcheng County was the lowest 123.83/100 000 (575/464 338), with significant differences (χ2=233.67, P<0.05). Conclusion The overall incidence of tuberculosis in the elderly in Xianning City , Hubei Province showed a downward trend, but the proportion of the incidence of tuberculosis in the elderly in the whole population has increased year by year, suggesting that the elderly are the key population of tuberculosis prevention and control in Xianning City in the future, and comprehensive prevention and control measures should be taken to reduce the incidence of tuberculosis in the elderly population.
2.Regulation of pyloric rhythm by I(A) and I(h) in crayfish stomatogastric ganglion.

The stomatogastric ganglion (STG) of shellfish includes 30 neurons and produces pyloric rhythms. It is the common model to study central pattern generator (CPG). Regulation of pyloric rhythms not only is related to the property of single neurons in STG but also depends on the connections and property of the whole neuronal network. It has been found that transient potassium current (I(A)) and hyperpolarization-activated cation current (I(h)) exist in certain types of neurons of STG. However, roles played by these two currents in maintaining and regulating the pyloric rhythms are unknown. In the present study, in vitro electrophysiological recordings were performed on crayfish STG to examine the role played by I(A) and I(h) in regulation of pyloric rhythm. 4AP (2 mmol/L), a specific inhibitor of I(A), caused a decrease in pyloric cycle (P < 0.01), an increase in PD (pyloric dilator) ratio, a decrease in PY (pyloric) ratio (P < 0.01) and delay of phases of LP and PY firing. ZD7288 (100 μmol/L), a specific inhibitor of I(h), caused a decrease in pyloric cycle (P < 0.01), an increase in PD ratio (P < 0.01), an increase in LP (lateral pyloric) ratio (P < 0.01), a decrease in PY ratio (P < 0.01) and delay of phases of LP and PY firing. These results indicate that I(A) and I(h) play important roles in regulating pyloric rhythms in crayfish STG.

3.Epidemiological study of clinical symptoms of sub-health

Objective To epidemiologically analyze the common clinical symptoms of sub-health in Beijing area.Methods By analysis of sub-health,epidemiologlcal data and the method of factors analysis, we studied the features and properties of various symptoms and their relationships that represent various kinds of sub-health.We used the structural equation modeling to validate the relationship between the symptom groups and between the symptom groups and their components.Results The amount of the factors whose weight Values were larger than 1.0 was 13,and the cumulative value of all the factors was 55.67%.With the domain knowledge,however,the total factors were merged into 10.We took fatigue as the main hidden variable to build a structural equation model.The function was x2/df=3.65,and GFI,AGFI,CFI,Tucher Lewis indexes were all above 0.9.The direct effects between the hidden variables showed that fatigue is closely related to fatigue of eyes,sleep,and pain with the weighted coefficient ranging from 0.7 to 1.0. Fatigue is somehow related to the symptoms of stool,pee,fauces,mouth,and sweat with the weighted coefficients about 0.5.The indirect effects between fatigue and hidden vailables showed that fatigue has an effect on moods indirectly with standardized indirect effects and standardized total effects was 0.702 and 0.112.respectively.Conclusions The symptoms of sub-health are complex with a dominating symptom and many other secondary symptoms.The common symptom groups are maladjustment of mood,fatigue, abnormity of sleep,maladjustment of stool and pee,double-minded,fatigue of eyes,and pain.The results of structural equation model shows that the causality between symptoms that are in a symptom group is different,so is the causality between the symptom groups,which shows that the symptoms of the sub-health are of diversity.
4.Effect of Zhibai Dihuang Pill on uPA of Uu-infection infertility patients.

OBJECTIVETo study the effect of Zhibai Dihuang Pill (ZBDHP) on urokinase-type plasminogen activator (uPA) and sperm quality in ureaplasma urealyticum (Uu) infection infertile patients.
METHODSRecruited were 80 infertility patients with Uu infection at Andriatrics Clinics and Department of Reproduction, including 130 cases of positive Uu semen and 50 cases of negative Uu semen. Patients with positive Uu semen were randomly assigned to the observation group (72 cases) and the control group (58 cases) according to the visit sequence. All patients took antibiotics for 2 weeks. Patients in the observation group additionally took ZBDHP, 6 g each time, twice daily. Those in the control group additionally took Vit E (100 mg each time, twice per day) and ATP (40 mg each time, twice per day). The therapeutic course for all was 90 days. Semen parameters and uPA contents of the sperm membrane were detected and comparatively analyzed.
RESULTSThe sperm membrane uPA content, the sperm motility, the sperm viability, and the percentage of normal morphology sperm in Uu positive infected patients were lower than those in Uu negative infected patients with statistical difference (P < 0.05), but with no significant difference in the sperm density between the two groups (P > 0.05). There was no statistical difference in pre-treatment sperm membrane uPA contents and sperm parameters between the two groups (P > 0.05). Compared with before treatment in the same group, the sperm membrane uPA content, the sperm motility, the sperm viability, and the percentage of normal morphology sperm obviously increased in the two groups with statistical difference (P < 0.05). After treatment, the sperm membrane uPA content increased more obviously in the observation group, with statistical difference when compared with the control group (P < 0.05).
CONCLUSIONSInfection with Uu leads to decreased uPA content of sperm membrance and the sperm motility. ZBDHP could effectively treat Uu infected infertility possibly through fighting against Uu damaged sperm membrane and make the sperm membrane uPA content return to normal, and elevate the fertilizability of sperms.

5.A Retrospective Clinical Analysis of 118 Cases of Small Intestinal Bleeding

Background:Small intestinal bleeding is difficult to diagnose and treat because of its complex etiology and limit to examination method. Aims:To analyze the etiology,diagnosis,treatment and prognosis of small intestinal bleeding. Methods:The clinical data of 118 consecutive patients with small intestinal bleeding admitted from Oct. 2006 to Oct. 2016 at Daping Hospital,the Third Military Medical University were retrospectively analyzed. Results:Melena was the most common manifestation of small intestinal bleeding (41. 5%),followed by dark bloody stool,positive fecal occult blood test,hematochezia,and anemia with unknown cause. The major causes of bleeding were benign or malignant tumors (43. 2%),vascular lesions (28. 0%)and inflammatory lesions (15. 3%). Diagnosis was made by means of capsule endoscopy,colonoscopy,digital subtraction angiography (DSA),barium meal examination,multi-slice CT (MSCT)and CT enterography (CTE). Forty-one patients were treated by surgical operation,7 by selective arterial embolization,2 by endoscopic therapy,56 by conservative therapy,and all these patients achieved hemostasis. One patient died of massive hemorrhage and 11 were discharged with giving up of treatment. Conclusions:The leading cause of small intestinal bleeding is tumor,followed by vascular and inflammatory lesions. Capsule endoscopy is able to make definite diagnosis with high accuracy,and MSCT is the most widely used diagnostic approach. In addition to conventional treatment,surgical operation,interventional and endoscopic therapies also play important roles in treating small intestinal bleeding.
6.Semantic priming processing in children with Chinese developmental dyslexia

Objective:To explore the semantic priming processing characteristics of children with Chinese developmental dyslexia(DD).Methods:According to scores of the dyslexia checklist for Chinese children, character recognition test and Raven's standard progressive matrices (RSPM), 19 primary school students from a school in Wuqia County of Xinjiang were selected as DD group.Meanwhile, 19 children were selected as chronological-age control(CA) group matching in age and RSPM scores, and 20 children were selected as reading-level control(RL) group according to the character recognition test and RSPM scores.The amplitude of N400 of children in three groups were tested by semantic priming paradigm under the conditions of semantic related, unrelated and pseudo words.Results:The accuracy of the DD group under the conditions of irrelevant semantics((0.55±0.25) vs (0.81±0.16), P<0.05), and pseudo characters((0.43±0.27) vs (0.68±0.26), P<0.05) were significantly lower than those of CA group.There was no difference between DD group and RL group, as well as between RL group and CA group(both P>0.05). There was no significant difference in the response time among the three groups of children under semantic related and unrelated conditions( F=1.23, 2.03, both P>0.05). In terms of ERP data, in semantic related condition, there was no significant difference in N400 amplitude among the three groups ( F=0.55, P>0.05). In semantic unrelated condition, the amplitude of N400 in DD group was smaller than that in CA Group((-5.92±3.70)μV vs (-9.76±3.45)μV, P<0.05), and there was no statistically significant difference between DD group and RL Group((-5.92±3.70)μV vs (-7.41±4.02)μV, P>0.05), as well as between CA gorup and RL group.As to pseudo words condition, children in DD group had a lower amplitude of N400 than those in CA and RL groups ((-6.03±2.92)μV vs (-9.98±3.40)μV, (-8.91±3.40)μV, both P<0.05). While no significant differences were found between CA group and RL group. Conclusion:Children with Chinese DD have cognitive defects in semantic and orthographic processing.The semantic cognitive defects may caused by their low reading level, rather than their inherent defect, while the defect in orthographic processing may be their inherent defect.
7.Qualitative study on the financial toxicity experience of young female breast cancer survivors

Objective:To explore the causes and influencing factors of financial toxicity in young breast cancer survivors, and to provide evidence for intervention program development to improve financial toxicity in young breast cancer survivors.Methods:Using descriptive qualitative research methods, 29 young breast cancer patients from September to December 2021 in Breast Surgery Follow-up Clinic of Fudan University Shanghai Cancer Center were interviewed. The Nvivo 12.0 qualitative data analysis software was used to analyze the data.Results:Four themes were extracted as following, direct cost of cancer treatment was the primary cause of financial toxicity, indirect costs related to cancer and treatment cannot be ignored, long-term effects of cancer and treatment further exacerbated financial toxicity, and cancer-related financial toxicity was also influenced by a variety of other factors.Conclusions:Multiple causes affected the experience of financial toxicity in young breast cancer survivors. The occurrence and risks of financial toxicity in young breast cancer survivors should be assessed. Intervention and support should be provided to meet the needs of young breast cancer survivors.
9.Analyses of chemical constituents and constituents absorbed into blood from Runing Granules by UPLC-QTOF/MS

AIM To establish a UPLC-QTOF/MS method for the analyses of chemical constituents and constituents absorbed into blood from Runing Granules.METHODS Two rats were given intragastric administration of the aqueous solution of granules,after which blood collection was made at 0,0.5,1,2,4 h.The analysis was performed on a 40℃ thermostatic Acquity UPLC BEH C18 column(100 mm×2.1 mm,1.7 μm),with the mobile phases comprising of 0.1% formic aci D-acetonitrile(positive ion)or water-acetonitrile(negative ion)flowing at 0.3 mL/min in a gradient elution manner,and electron spray ionization source was adopted in positive and negative ion scanning.RESULTS Total 166 chemical constituents were identified,containing 44 flavonoids,17 terpenes,23 alkaloids,11 saponins,29 phenolic acids,13 quinones,10 organic acids,and 19 others,along with 11 constituents absorbed into blood.CONCLUSION This accurate and stable method can provide a data support for the clinical application of Runing Granules,and lay foundations for the further researches on their quality control and material basis.

AIM To establish a UPLC-QTOF/MS method for the analyses of chemical constituents and constituents absorbed into blood from Qili Qiangxin Capsules by UPLC-QTOF/MS.METHODS The rats with heart failure after myocardial infarction were given intragastric administration of 0.5%CMC-Na suspension of Qili Qiangxin Capsules(crude drug dosage was 0.9 g/mL)for 4 weeks,after which blood collection was made.The analysis was performed on a 40℃thermostatic Acquity UPLC BEH C18 Column(100 mm×2.1 mm,1.7 μm),with the mobile phase comprising of 0.1%formic acid-acetonitrile(positive ion manner)or water-acetonitrile(negative ion manner)flowing at 0.3 mL/min in a gradient elution manner,and electron spray ionization source was adopted in positive and negative ion scanning.RESULTS Total 151 constituents were identified,containing 38 flavonoids,36 terpenoids,23 alkaloids,11 saponins,11 phenylpropanol,10 phenolic acids,4 organic acids,4 cardiac glycosides and 14 others,along with 35 constituents absorbed into blood.CONCLUSION This accurate and stable method can provide data support for the rational clinical application of Qili Qiangxin Capsules,and lay foundation for the research on related effector substances of other Chinese traditional patent medicines.
