1.MRI diagnosis and differential diagnosis of bladder leiomyoma

Objective To investigate the value of MRI in the diagnosis and differential diagnosis of bladder leiomyoma.Methods Clinical data,pathological files and MRI findings of 8 patients with bladder leiomyoma that verified by pathologic were analyzed retrospectively.Results 4 cases of bladder leiomyoma were located in front bladder wall, 3 cases were in post bladder wall and 1 case in right bladder wall;meanwhile, 6 cases were endovesical(75.0%),1 case was intramural(12.5%) and another 1 case was extravesical(12.5%).All lesions displayed equal signals on T1WI, equal or slightly high signals on T2WI with mild and moderate enhancements;bladder wall adjacent to tumors and structures around were not invaded by tumors.6 cases of all displayed inhomogeneous signals on T2WI and with some patchy or stripe high signals in it, the signals turned to equal on a fat-saturated T1WI which was similar with muscle of pelvic wall.Conclusion MRI features of bladder leiomyoma showed certain characteristics.
2.Value of fast track surgery principles in the perioperative management of liver cancer patients after hepatectomy

Objective To investigate the value of fast track surgery (FTS) principles in the perioperative management of liver cancer patients after hepatectomy.Methods Forty patients with primary liver cancer who were admitted to the First People's Hospital of Qinzhou from September 2011 to July 2013 were enrolled in this prospective study.All the patients were randomly divided into the FTS group (20 patients) and the control group (20 patients) according to the random number table.The perioperative management of patients in the FTS group was guided by the FTS principles,patients in the control group were managed with traditional methods.The intraoperative condition,time for portal occlusion,operation time,volume of intraoperative blood loss and blood transfusion,time to drainage tube removal,time to flatus and defecation,duration of postoperative hospital stay,expenses,changes of C-reactive protein on postoperative day 1,3,6,recovery of hepatic function and incidence of postoperative complications.All patients were followed up via phone call and out-patient examination till September 2013.All data were analyzed using the t test or chi-square test.The non-normal distribution paramenters were analyzed using the rank sum test.Results All patients were cured with no perioperative death.The time for postoperative drainage tube removal,time to flatus and defecation,duration of postoperative hospital stay and expenses were (2.3 ± 1.0)days,(2.5 ±0.5)days,(3.1 ±0.7)days,(7.0 ±0.8)days and (3.6 ±0.3) × 104 yuan in the FTS group,and (4.6 ± 0.7) days,(4.3 ± 0.7) days,(4.8 ± 0.4) days,(8.5 ± 0.9) days and (4.1 ± 0.3) ×104 yuan,with significant differences between the 2 groups (t =0.74,0.34,1.70,0.23,0.57,P < 0.05).The levels of C-reactive proteins at postoperative day 1,3,6 were (56 ±7)mg/L,(122 ±7)mg/L and (35 ±7)mg/L in the FTS group,and (198 ± 24) mg/L,(137 ± 5) mg/L and (49 ± 8) mg/L,with significant differences between the 2 groups (F =64.91,P <0.05).The levels of prealbumin at postoperative day 1,3,6 were (196 ± 14) mg/L,(243 ± 17) mg/L,(260 ± 10) mg/L in the FTS group,and (198 ± 24) mg/L,(199 ± 16) mg/L and (245 ± 7) mg/L in the control group,with significant differences between the 2 groups (F =22.69,P < 0.05).The levels of alanine transaminase at postoperative day 1,3,6 were (379 ±34)U/L,(166 ± 12)U/L,(49 ± 14)U/L in the FTS group,and (367 ±75)U/L,(210 ±28)U/L,(197 ±22)U/L in the control group,with significant differences between the 2 groups (F =4.51,P < 0.05).One patient was complicated with peritoneal effusion and 1 with thoracic effusion in the FTS group; 4 patients was complicated with peritoneal effusion,3 with thoracic effusion,4 with pulmonary infection and 2 with incisional infection in the control group,with no significant difference in the complication between the 2 groups (x2 =0.78,1.11,4.44,2.11,P > 0.05).All the patients were followed up for 2-24 months,no patients received reoperation or re-admitted to the hospital due to complications.Conclusion The application of FTS principle in the perioperative management of liver cancer patients after hepatectomy is safe and effective,it could alleviate the post-operative stress reaction and accelerate the recovery of liver function and patients' condition.
3.Diagnosis and treatment of ketamine-associated cystitis:A report of 3 cases and literature review

Objective:To determine the diagnosis, treatment, and pathogenesis of ketamine-associated cystitis.
Methods:Clinical data from 3 patients with ketamine-associated cystitis were analyzed retrospectively and discussed in light of relevant literature.
Results:In the 3 cases, 2 presented severe lower urinary tract symptoms, including frequency, urgency, dysuria, urge incontinence, and painful haematuria. Urinalysis and urine culture were negative. Imaging examination demonstrated thickening of the bladder wall and a small capacity. Inflammatory changes in the bladder mucosa were observed by cystoscopy and biopsies. After cessation of ketamine use, with the addition of steroids or hydrodistension, the symptoms in the 3 patients improved. The symptoms recurred in 2 patients, as 1 was exposed to ketamine again and 1 had severe bladder contraction after for 3-4 month follow-up.
Conclusion:Ketamine-associated cystitis is a new urinary system inlfammatory damage. Its etiology and treatment methods are not clear. early abstinence from ketamine use and early treatment are crucial for patients with ketamine-associated cystitis to avoid irreversible damage.
4.A clinical efficacy and safety study on coronary heart disease and angina treatment with Puerarin Injection

Objective To observe the clinical efficacy and safety of Puerarin Injection treatment on angina pectoris.Methods 388 patients with angina pectoris,enrolled to Tianjin Fourth Central Hospital during January 2009 and February 2011 were selected and randomly divided into treatment or control groups with 194 patients of each.Based on the conventional therapy program,one group was given Puerarin Injection as treatment group while,the other was given with Danshen Injection as control group.Clinical efficacy,the attack rate of angina pectoris,oxygen consumption,iadices on electrocardiogram,haemorheology and other adverse reactions among the two groups were compared.Results The total efficacy of the treatment group (88.14%,171/194)was significantly higher than the control group (61.86%,120/194) and the difference was statistically significant (P<0.05).During the treatment,no significant adverse events were noticed in both of the two groups of patients.Conclusion The Puerarin Injection treatment program on angina pectoris seemed effective and safe.
5.Factors influencing the diagnose on causes of dyspepsia in central area of Shaanxi province.

OBJECTIVETo investigate the proportions of functional dyspepsia (FD) and structural diseases within the dyspeptic outpatients in hospitals of different ranking in central area Shaanxi province, and to explore the safety in adopting "symptom and treatment" algorithm used in Western countries.
METHODSA clinical epidemiology survey was carried out by means of a stratified sample of 3 019 dyspeptic outpatients through standardized questionnaire. All of the patients were followed for 4 - 24 weeks, and finally received their diagnoses through a consistent criteria.
RESULTSProportionally, FD in all the outpatients took up 44.8% with 44.7% benign organic causes and 10.5% malignant diseases. In the patients who had marked alarm symptoms, the proportion of benign and malignant diseases rose to 52.1% and 29.2%, respectively. They were significantly higher than those without alarm symptoms (39.2%, 2.9%) (P < 0.01). All of the dyspeptic patients were divided into 7 groups according to different ages. 64.5% of FD patients were younger than 25 years and the rate of FD declined with age. It was found that only four patients below 35 years old had malignance. The number of cases increased significantly in age 35 - 45 group and reached 30.8% in 65 - 74 group.
CONCLUSIONThere were some differences noticed within dyspeptic patterns between local area in China and Western countries, and the "symptom and treatment" approach was not entirely suitable to the local area in China.

6.Effects of electro-acupuncture on expression of triggering receptor expressed on myeloid cells 1 in ankle joint synovial tissue of acute gouty arthritis rats.

OBJECTIVETo investigate the effects of electro-acupuncture (EA) on the expression of triggering receptor expressed on myeloid cell (TREM)l in ankle joint synovial tissue of acute gouty arthritis (AGA) rats.
METHODSForty male SD rats were randomly divided into 4 groups: normal, AGA, medication and EA group, 10 rats in each group. AGA model was established by induced monosodium urate (MSU) method, except the normal group. Tow days before AGA model was established, normal and AGA groups were lavaged with normal saline (20 ml/kg), medication group was lavaged with colchicine solution (20 ml/kg), EA(1.5-2 Hz, D.-D.wave, 9v; 1-3 rnA) was applied to "Sanyinjiao" (SP6), "jiexi" (ST41) and "Kunlun" (BL60) for 20 min, once daily;continuously for 9 days. Then observed the changes in dysfunction, and the content of TNF-α and IL-lβ detected by ELISA, the expression of TREM-l detected by immunohistochemistry and western blot.
RESULTSCompared to the normal group, the AGA group of the dysfunction index increased significantly (P<0.01), the content of TNF-α and IL-lβ increased significantly (P<0.05), the expression of TREM-l in synovial tissue increased significantly (P<0.05); the medication and EA groups compared to the AGA group, the dysfunction index decreased significantly (P<0.01), the content of TNF-α and IL-lβ decreased significantly (P<0.05), the expression of TREM-l in synovial tissue decreased significantly (P<0.05); there were not statistically significant between the medication and EA group (P>0.05).
CONCLUSIONEA treating AGA may be through down-regulating the expression of TREM -1 in synovial tissue.

7.Endovenous laser treatment of 62 patients with primary varicose veins of lower extremities.

OBJECTIVETo retrospectively analyze the experiences and results of the treatment on 62 patients with primary varicose of lower extremities with endovenous laser.
METHODSAll patients were treated with endovenous laser. The laser treatment could begin when the fiber withdraw with 1 cm/2 s. The laser power was 10 - 12 w with the laser pulse duration and the interval 1 second respectively.
RESULTSThe duration of follow-up varied from 2 months to 8 months. After endovenous treatment, the varicose veins and edema disappeared in all cases. The itching and uncomfortable feeling was dissipated. No morphine-like analgesic has been used and no serious complications occurred.
CONCLUSIONEndovenous laser treatment of primary varicose of lower extremities is a safe and effective technique.

8.Effect of fertilization on phenolic components and antioxidant activities of Artemisia annua.

OBJECTIVEA pot experiment with variable fertilizer treatments was carried out to study the influence of fertilization on the concentration and accumulation of polyphenols, scopoletin, chrysosplenol-D and chrysosplenetin in roots, stems and leaves and their antioxidant activities. The main aims were to fertilize scientifically in cultivation of Artemisia annua and improve the quality of the harvest organs.
METHODThese active components in leaves, stems and roots in the squaring stage were analyzed by HPLC and antioxidant activities of the extracts were evaluated by ultraviolet visible light colorimetric method.
RESULTThe result showed the highest concentration of polyphenols, scopoletin, chrysosplenol-D and chrysosplenetin was in leaves, followed by stems and the lowest in roots. The antioxidant activities of the leaf extracts correlated positively with the concentrations of polyphenols, scopoletin, chrysosplenol-D and chrysosplenetin. Furthermore, fertilization promoted significantly the growth of A. annua, the biomass was increased by 57.37% (chemical fertilizer), 91.63% (mixture of chemical fertilizer and manure) and 92.27% (manure), respectively, compared to the blank control (without fertilizer). Fertilization, particularly mixture fertilization of chemical fertilizer and manure, increased generally the concentration and accumulation of polyphenols, scopoletin, chrysosplenol-D and chrysosplenetin as well as DPPH x scavenging ratio.
CONCLUSIONScopoletin, chrysosplenol-D and chrysosplenetin could be synthesized and stored mainly in leaves. The leaves might thus be the chief organ of A. annua for medical treatment. Finally, the mixture fertilization of chemical fertilizer and manure should be used to increase the yield and quality of A. annua.

9.Chylous leakage after retroperitoneoscopic upper-pole heminephrectomy for duplex kidney.

OBJECTIVE:
To discuss management of chylous leakage after retroperitoneoscopic upper-pole heminephrectomy for duplex kidney.
METHODS:
Between November 2004 and Februar y 2011, 39 patients underwent retroperitoneoscopic upper-pole heminephrectomy for duplex kidney, of these 5 patients had chylous leakage. The ages of the patients ranged from 32 to 60 years (mean 42). All the patients were treated conservatively, and the therapeutic effects were observed.
RESULTS:
Delayed chylous leakage in 5 patients occurred 5-31 days after surgery, and leakage occurred in 4 of the same 5 patients during the first 2 post-operative years. Chylous leakage after retroperitoneoscopic upper-pole heminephrectomy for duplex kidney preferentially occurred at the left side of duplex kidney. All the patients healed under conservative treatment.
CONCLUSION
Chylous leakage typically occurs after left retroperitoneoscopic upper-pole heminephrectomy for duplex kidney, and can be prevented by improving surgical technique; it can be completely relieved by conservative management with satisfactory results.

10.Clinical studies on different coronary artery interventional therapies through femoral artery or radial artery approaches

Objective To compare the clinical efficacies regarding femoral artery or radial artery approaches on coronary artery interventional therapies.Methods 360 patients were randomly divided into intervention group via femoral artery (TFI) or transradial coronary intervention (TRI) group.Postoperative observation on the two said groups of patients with vascular lesion characteristics,feasibility factors ( success rate of interventional therapy puncture,time of operation and hospitalization)and complications,were made.Results There were no significant differences between the two groups on the characteristics of vascular lesion (P>0.05).Success rates of the two groups were 97.78% and 96.67% respectively.The differences on success rate,time of operation were not statistically significant (P>0.05) while the average time of puncture,the mean duration of hospitalization and the rates of complications were significantly different ( P< 0.05 ).Conclusion The two methods under comparison had similar success rate and feasibility.However,the complications related to radial artery interventional therapy were much less than the femoral artery interventional therapy.As having better safety,radial artery interventional therapv seemed to have applicable value on clinical oractice.
