1.The clinical experience of total thyroidectomy in the treatment of thyroid carcinoma

Objective To evaluate the therapeutic value of total thyroidectomy in the thyroid carcinoma.Methods 87 patients of thyroid carcinoma were operated by one surgeon with total thyroidectomy from 1985-2000 in our hospital, We carried out a retrospective research for the complication rates and postoperative iodine 131 treatment. Results Observing after operation revealed that there were 2(2.3%) of recurrent laryngeal nerve damage, 1(1.1%) of thypocalecimia, 1(1.1%) of atelectasis, and no deaths, permanent laryngeal nerve damage, or permanent hypocalcemia.Conclusion Total thyroidectomy is safe and effective procedure in the treatment of thyroid carcinoma, this approach can offer the advantage of removal of tissue at risk for malignant transformation,and enhances the use of the postoperative iodine 131 treatment.
2.Delayed percutaneous coronary intervention versus conservative strategy after thrombolysis for ST-segment elevation myocardial infarction

Objective To evaluate the role of routine delayed percutaneous coronary intervention (PCI) after thrombosis in the management of patients with ST-segment elevation myocardial infarction. Methods Ninety-eight patients with STEMI who underwent routine delayed PCI after thrombosis and other 82 patients with STEMI who were given conservative strategy after thrombolysis were enrolled in this study and were investigated during their in-hospital period and 6-month follow-up regarding major adverse clinical events (MACE) and the cardiac structure and function by echocardiography. The patients were divided into the control group (conservative strategy after thrombolysis), therapy group 1 (routine delayed PCI after successful thrombolysis), and therapy group 2 (routine delayed PCI after unsuccessful thrombolysis). Results Compared with conservative strategy after thrombolysis, routine delayed PCI after thrombosis decreased in-hospital mortality (4.9% vs 0%, 0%), shortened average hospital stay (25.3 days vs 13.5 days, 15.1days), decreased the need for revascularization for target lesion (7.3% vs 0%, 0%) and lowered the incidence of thrombosis or infarction (7.3% vs 0%, 0%), and also produced lower mortality (13.4% vs 1.4%, 0%), prevented reinfarction (12.2% vs 4.2%, 4.5%) and stroke (2.4% vs 0%, 0%), decreased the need for revascularization for target lesion (28% vs 4.2%, 4.5%) and prevented further left ventricle remodling. Conclusion Routine delayed PCI after thrombosis may to prevent recurrent ischemia, reinfarction, and reocclusion, so as to improve immediate results and 6-month prognosis.
3.An analysis of treatment for invasive pulmonary infections with Candida species

Objective To evaluate the diagnosis and treatment for the patients infected with Candida species.Methods A total of 47 patients with persistent fever,pathology of pulmonary infection,no response to the antibiotic therapy,Candida species found in sputum after long time hospitalization were retrospectively analyzed.Results The 47 patients were 24 males and 23 females.The mean age was 57.0?16.1.The mean temperature was(38.4?0.6)℃ and all lasted for over 6 days.Pulmonary infections with Candida species was diagnosed at the(17.7?5.2)th day after admission.The species isolated were 38 strains Candida albicans,5 strains Candida glabrata,2 strains Candida kruseic,1 strain Candida tropicalis,and 1 strain Candida lusitaniae.Fluconazole was given to 28 patients(2 with voriconazole as sequential therapy),itraconazole to 9(2 with voriconazole too),voriconazole in 7(including the 4 patients mentioned above),Casfungin aceate to 7,Amphotericin B to 1(Casfungin aceate was given alone later for serious adverse reactions).Results two patients infected with C.albicans and Candida glabrata separately had no improvement in the 28 patients treated with fluconazole,but voriconazole showed good effect on the 2 patients.Two patients infected with C.albicans and Candida glabrata separately had no improvement to itraconazole in the 9 patients treated with Itraconazole,but voriconazole showed good effect on them too.The 7 patients treated with voriconazole all had good effect,including 3 infected with Candida glabrata,1 with Candida lusitaniae,3 with C.albicans.Conclusion Candida albicans is the main pathogen in this group of invasive pulmonary infections with Candida species.All the patients have fever and long-time hospitalization.The currently used antifungal drugs,such as voriconazole,Casfungin aceate,are reliable.Caspofungin is generally better tolerated than other antifungal drugs.
4.Relationship between lumbosacral multifidus muscle and lumbar disc herniation.

As a common disease in clinical, the treatment of lumbar disc herniation (LDH) focused on local intervertebral disc, such as surgery and other interventional therapy treatment, but postoperative complications and recurrence rate has been a difficult problem in the field of profession. With the development of spine biomechanics and anatomy, researches on lumbar herniation also increased. Researchers discovered that the incidence and prognosis of LDH were inseparable with local muscle and soft tissue. As the deep paraspinal muscles, multifidus muscle plays an important role to make lumbar stability. Its abnormal function could reduce the stable of lumbar spine, and the chronic lumbar disease could also lead to multifidus muscle atrophy.

5.Expression and significance of follicular helper T cells and galactose-deficient IgA1 in children with Henoch-Schönlein purpura.

OBJECTIVE:
To study the role of follicular helper T (Tfh) cells and galactose-deficient IgA1 (Gd-IgA1) in the pathogenesis of childhood Henoch-Schönlein purpura (HSP) and the correlation between them.
METHODS:
A total of 36 children with newly-diagnosed HSP were enrolled. They were divided into two groups: HSP nephritis (HSPN) group with 11 children and non-HSPN group with 25 children according to the presence or absence of HSPN. Another 15 children who underwent physical examination at the outpatient service were enrolled as the healthy control group. Flow cytometry was used to measure the proportion of Tfh cells (CD4CXCR5ICOS) in peripheral blood. ELISA was used to measure the levels of interleukin-21 (IL-21) and interleukin-6 (IL-6) in peripheral blood and the serum levels of IgA1 and Gd-IgA1. A Pearson correlation analysis was used to investigate the correlation of serum Gd-IgA1 concentration with Tfh cells and related factors expression in the children with HSP.
RESULTS:
Both the HSPN and non-HSPN groups had significantly higher proportion of Tfh cells and expression levels of IL-21 and IL-6 in peripheral blood than the healthy control group (P<0.05). The HSPN group had significant increases in the above indices compared with the non-HSPN group (P<0.05). Both the HSPN and non-HSPN groups had significantly higher serum levels of IgA1 and Gd-IgA1 than the healthy control group (P<0.05). The HSPN group had significantly higher serum levels of IgA1 and Gd-IgA1 than the non-HSPN group (P<0.05). In the children with HSP, serum Gd-IgA1 level was positively correlated with Tfh cells proportion and IL-21 and IL-6 levels (P<0.05).
CONCLUSIONS
Tfh cells and related cytokines and serum Gd-IgA1 are involved in the development of HSP/HSPN. Tfh cells may mediate the increased production of Gd-IgA1.

6.Global progress in peptide radiopharmaceutical research and China's opportunities

Radiopharmaceutical is an essential component of nuclear medicine and molecular imaging, as well as a key component of precision medicine. The United States Food and Drug Administration (FDA) has recently approved the marketing of several peptide-based radiopharmaceuticals, sparking a global trend of research in this area and propelling nuclear medicine into the precision theranostic era. This has created a new wave of technological competition in the field of nuclear medicine. It is the responsibility of Chinese scientists in the radiopharmaceutical field to capitalize on this opportunity, leverage the momentum, and strengthen their independent innovation capability in order to stay ahead in the future global nuclear science and technology competition. This review provides an overview of the remarkable progress made in the research, development, and translation of global peptide-based radiopharmaceuticals. It examines the advantages of peptide-based radiopharmaceuticals and outlines the current hot targets and progress in drug development in this field. Additionally, it proposes six opportunities for China to overtake others in the field of peptide-based radiopharmaceuticals and achieve technological self-reliance, based on interdisciplinary collaboration and independent innovation. Lastly, the future prospect of peptide-based radiopharmaceuticals is discussed.
8.IODINE AS AN OXIDANT FOR THE DETERMINATION OF TOTAL ASCORBIC ACID IN VEGETABLES

Iodine was used as an oxidant instead of brimine in the 2,4-dinitro-phenylhydrazine method for the determination of total ascorbic acid content in vegetables. The excess of iodine was removed with a small amount of crystalline thiourea. This process of oxidation was simple, safe and rapid.With this modified method, the ascorbic acid content of 18 vegetables were determined. The ascorbic acid values thus obtained with iodine were sometimes slightly lower than those with bromine, but were higher than those with paper chromatographic method.
9.THE APPLICATION OF PAPER PARTITION CHROMATO-GRAPHIC METHOD FOR THE QUANTITATIVE DETERMINATION OF TOTAL ASCORBIC ACID CONTENTS IN VEGETABLES AND FRUITS

(1) The contents of total ascorbic acid in 129 kinds of vegetables and fruits in Kaifeng were determined by means of both Kuo-Chen's paper ch-romatographic method and Schaffert-Kingsley's 2,4-dinitrophenyIhydrazine colorimetric method.(2) Our results indicated that the 2,4-dinitrophenylhydrazine colorimetric method was not specific in determining the total amount of ascorbic acid contents in acid extracts of vegetables and fruits and thus usually gave false results. Such extracts must be first separated from interfering substances before determination. Kuo and Chen's paper chromatog-raphic method is most suitable for this purpose.
