2.Computed tomographic findings of ovarian tumors

The diagnosis of ovarian tumor has been mainly dependent on manual pelvic examination and ultrasonography. Butin case of malignant ovarian tumor, CT has more advantages over ultrasonography in assessing anatomic details,relationships to bowel loops, precise extents of tumors and follow-up examinations after surgery. Authors analyzedCT features of 46 cases of patholgocially proven ovarian tumors for recent 4 years at keimyung University DongsanHospital. The results were as follows: 1. The msot common tumor was serous cystadenocarcinoma(9 cases:20%),followed by metastases(8 cases: 17%), mucinous cystadenocarcinoma(7 ases:15%), mucinous cystadenocarcinoma(5cases:11%), teratoma(5 cases:11%), lymphoma(3 cases: 7%) and dysgerminoma(2 cases:4%). 2. The ovarian tumors werevariable in size from 2.5cm to 33cm in diameter. Most of the solid tumors were smaller than 10cm in diameter andmost of the cystic tumors were larger than 10cm in diameter. Usually mucinous tumors were much larger than seroustumors. Mucinous cystadenomas were the largest tumors. 3. Unilateral tumors(left 19,right 13 cases) were morecommon than bilateral tumors(12 cases). Bilateral tumors were seen in serous and mucinous cystadenocarcinoma,metastases and lymphoma. 4. CT features of mucinous cystadenomas were smooth margins and thin wall of the tumormasses and multiloculated cysts with internal septa in all 7 cases. 5. In contrast, CT demonstration ofbilaterality, irregular margin, thick wall, enhancing solid lesions, septal irregularity, adhesion to adjacentstructures, peritoneal/omental implantation, ascites and hydronephrosis were signs suggesting malignancy. CTfeaturs of the serous cystadenocarcinoma were mostly solid to mixed nature(83%), irregular margin(75%), enhancingsolid lesion(67%), papillary growth (75%), internal septa(58%), multilocularity (58%) and calcification (25%) indescending order of frequency. 6. On CT, mucinous cystadenocarcinoma were irregular-marginated, thick-walled,cystic tumors with enhancing solid lesion, septal irregularity and signs of metastasis, although there were somecases having similar features of benign. 7. Among the extrapelvic CT findings of malignant epithelial ovariantumor, peritoneal/omental implants(11 cases:79%) and ascites(10 cases:71%) were the most common, and indistinctuterus(6 cases:43%), bowel adehsion(5 cases:36%) and pseudomyxoma peritonei (2 cases) were descending order offrequency. 8. CT features of teratoma were diagnostic having at least three more of different tissue densitiesamong fat, water, soft tissue and calcific densities. Also there were thick wall (4 cases) and fat-fluid level(1case). 9. In 8 cases of ovarian metastases, there were solid type tumor smaller than 10cm in 4 cases and hugecystic in 2 cases. The site of primary cancer were stomach in 4 cases, colon , cervix, endometrium and unknown ineach one case. 10. In 3 cases of malignant lymphoma, the CT featurs were solid in appearance, smaller than 10cmand accompanied by lymphadenopathy in all cases.

3.Reapir of the Torn Achilles Tendon, Using the Plantaris Tendon

A major problem in the repair of the torn Achilles tendon has been providing the restoration of the anatomic continuity such that virtually normal plantar flexion power and ankle mobility result. Continuity should be restored without subsequent pain, disconfiguration, occupational limitations. If possible, postoperative complications, or tendon-rerupture should be avoided. Surgeons have long advocated the use of strips of facia and other tissues, including the plantaris tendon, to reinforce the repair of the torn Achilles tendon, The repair effected by these methods not infrequently is bulky and not too secure. Non-absorbable suture materials utilizing in the end to end anastomosis of the torn Achilles tendon cause sinus formation and discharge of suture materials. A secure method of repair for the tom Achilles tendon, which decreases sinus formation and is not bulky, should be required. The plantaris tendon, which has been used as a reinforcing material, can be utilized in the end-to-end anastomosis of the torn Achilles tendon by detaching from the muculotendinous junction. The 11 patients with the torn Achilles tendon were treated by end-to-end anastomosis using the plantaris tendon as suture material. The postoperative results were staisfactory without significant functional deficit and complication.

4.Effect of Ginkgo Biloba Extract on the Proliferation of Cultured Human Tenon Capsule Fibroblasts.

PURPOSE: To investigate the effect of Ginkgo biloba extract (GBE) on the proliferation of cultured human Tenon capsule fibroblasts (HTCF). METHODS: Free radical scavenging activity of GBE was assessed with a DPPH assay. Primarily cultured HTCF were exposed to 10 and 100 microgram/ml of GBE, and the effect of this extract on HTCF survival was assessed. Following 48 hr exposure to the media with or without serum, cellular survival and nitrite production were assessed by MTT and Griess assays. To evaluate whether GBE had a cytoproptective effect, HTCF were cultured in a combination of GBE and either sodium cyanide or hydrogen peroxide. RESULTS: GBE showed free radical scavenging activity. GBE increased the cellular survival of HTCF significantly in a dose-dependent manner and provided a cytoprotective effect when cells were exposed to sodium cyanide or were deprived of serum, but not when hydrogen peroxide was added to the medium. GBE decreased nitric oxide production but not to a statistically degree. CONCLUSIONS: GBE promotes proliferation of HTCF and has a cytoprotective effect in serum-deprived or hypoxic conditions. This suggests that GBE may be involved in the regulation of conjunctival wound healing by increasing the survival of HTCF.

5.Effect of Ginkgo Biloba Extract on the Survival of Cultured Human Trabecular Meshwork Cells.

PURPOSE: To investigate the effect of Ginkgo biloba extract (GBE, EGb-761) on the survival of cultured human trabecular meshwork cells (HTMC). METHODS: Free radical scavenging activity of GBE was assessed with a DPPH assay. Primarily cultured HTMC were exposed to 10 and 100 microgram/ml of GBE. 0.3 mM sodium cyanide and 100 micrometer hydrogen peroxide were added to the culture medium with GBE for 48 hr. Cellular survival and nitrite production were assessed by MTT assay and Griess assay, respectively. RESULTS: GBE showed free radical scavenging activity. GBE increased the cellular survival of HTMC significantly in a dose-dependent manner under hypoxia or serum-deprived condition, but not to hydrogen peroxide. GBE increased nitric oxide production but not to statistically significant levels. CONCLUSIONS: GBE promotes proliferation and has cytoprotective effects in the context of HTMC exposed to serum-deprived or hypoxic conditions. However oxidative stress induced by hydrogen peroxide did not have an effect on proliferation of HTMC. In addition these effects were not related to the production of nitric oxide.

6.Molecular Mechanism of the G2/M Arrest in Breast Cancer Cell Lines (T47D and MDA-MB231) Induced by Genistein.

PURPOSE: To analyze the effect of the growth control on human breast cancer cells with genistein treatment and to investigate the mechanism of genistein-induced G2/M arrest in T47D and MDA-MB231 breast carcinoma cells by Cdc25C expression. METHODS: We analysed the proliferartion of the two cell lines by using MTT proliferation assay, flow cytometric analysis, real-time quantitative RT-PCR and western blotting and investigated the effect of genistein on cell survival, cellular toxicity, cell cycle progression-related genes and their mRNA and protein alterations. RESULTS: The DNA flow cytometric analysis of both cell lines treated with genistein showed a dose-dependent growth inhibition and accumulation in the G2/M phase of cell cycle. The expression of p21 mRNA and protein increased in both cell lines following genistein treatment but p27 expression was unchanged. Furthermore, decreased Cdc25C expression with decreased polo-like kinase (PLK) 1 expression and increased PLK3 expression were observed after genistein treatment. The decreased level of Cdc25C in the nucleus was associated with decreased phosphorylation of Cdc25C by PLK1. The expression of PLK3 was increased with a dose-dependent and a time-dependent manner and was associated with decreased Cdc25C expression. Check point kinase (CHK) 1 and CHK2 revealed different expression patterns each other. The CHK1 expression was independent of the presence of genestein. CHK2 expression increased in MDA-MB231 cells associated with decreased Cdc25C expression but not in T47D. CONCLUSION: These results suggest that genistein induces a G2/M arrest in human breast cancer cells, the mechanism of which is due, in part, to decreased in Cdc25C phosphatase by a regulatory effect of PLK1, PLK3, and CHK2 as well as increased expression of the cyclin dependent kinase inhibitor p21(WAF1/CIP1).

7.Is Completion Thyroidectomy Necessary in Patients with Papillary Thyroid Carcinoma who Underwent Lobectomy?

Background/Objectives:
Although thyroid lobectomy recently is considered as sufficient for low-risk papillary thyroid carcinoma (PTC), completion thyroidectomy is required due to the insufficiency of the preoperative evaluation. The aim of this study was to investigate recurrence rate and disease free survival depending on the gross extrathyroidal extension (gETE) or the number of metastatic lymph node identified in patients with PTC.Materials & Methods: We assessed 3373 patients with PTC who underwent lobectomy at Seoul St. Mary’s Hospital (Seoul, Korea) between January 2009 and December 2014. Clinicopathological characteristics and long-term surgical outcomes were retrospectively analyzed through complete chart reviews. The mean follow-up duration was 97.1 ± 21.4 months.
Results:
The rate of recurrence was higher in gETE group (1.8% vs. 6.0%, p=0.004), leading to decreased disease free survival in Kaplan-Meier analysis (log-rank p<0.001). N1 group (n=1389) was analyzed into two groups whether the number of positive nodes is more than 5 or less. For the group of the more metastatic nodes, the recurrence rate higher compared to the other group (3.0% vs. 9.3%, p<0.001). DFS was longer in the group that had lesser metastatic nodes (log-rank p<0.001). However, in terms of N1 group over 1cm (n=492), No statistical difference was observed according to the number of positive lymph nodes (4.5% vs. 9.1%, p=0.092)
Conclusion
When it comes to node positive PTC, Despite the number of positive lymph nodes was over 5, follow-up with no further surgery can be an option.

Spinal arachnoid cysts are a rare disease with an unknown origin. Because of their broad base, a total laminectomy with or without fusion has been the treatment of the choice. We encountered 5 patients with a spinal arachnoid cyst who were treated by recapping laminoplasty after pars osteotomy. This procedure has not been reported in Korea. All patients showed neurological recovery with no recurrence of the cyst. The findings on the stressed plain film confirmed bony union and stability of the posterior element.

10.Genistein Induces G2/M Cell Cycle Arrest and Apoptosis in Rat Neuroblastoma B35 Cells; Involvement of p21(waf1/cip1), Bax and Bcl-2.

BACKGROUND: The effect of genistein on different types of cells has been investigated. However, its effect on the nervous system is still unclear. The aim of the present work is to explore the effect of genistein on rat neuroblastoma B35 cells. METHODS: The effect of genistein on the proliferation of B35 cells, its cytotoxicity, the cell-cycle distribution, the ultra-structural changes and the induction of apoptosis were determined using MTT assay, LDH assay, Flow-cytometric analysis, transmission electron microscopy and Hoechst staining, respectively. Furthermore, Real-time quantitative RT-PCR and Western blotting were used to examine the transcriptional and post-translational alterations of the G2/M cell-cycle arrest marker cyclin-dependent kinase inhibitor p21(waf1/cip1) and the apoptosis-related genes after genistein treatment. RESULTS: Genistein significantly inhibits cell survival, slightly elevates the release of lactate dehydrogenase and induced apoptosis in B35 cells. Genistein increased the number of cells at S-phase and induced cells to accumulate at the G2/M phase. These G2/M arrested cells are associated with a marked up-regulation of p21(waf1/cip1) at both the mRNA and protein levels. We observed that genistein up-regulates pro-apoptotic Bax with concurrent down-regulation of the anti-apoptotic Bcl-2 protein. CONCLUSION: These observations suggest that the anticancer effect of genistein on B35 neuroblastoma cells is mediated through multiple cellular pathways including G2/M cell-cycle arrest and the induction of apoptosis.
