1.A Case of Cutaneous Leishmaniasis Treated with Cryosurgery.

Cutaneous leishmaniasis (oriental sore) is usually a self-limited infection of the skin caused by the protozoan Leishmania tropica. The disease is endemic to the Mediterranean, Asia, Africa, and the Middle East. It has been seen in this country among many Korean technical experts and labourers working in the endemic areas of the disease. Our patient had acquired cutaneous leishmaniasis in Saudi Arabia and it had remained active for six months. He had been treated with antimony and metronidazole but failed because of severe side effects. And then we treated the patient witb cryosurgery and the skin lesions were followed by resolution with cosmetically acceptable scar in 4 months. The brief review of literature on the treatment of cutaneous leishmaniasis was undertaken.

2.A Study for Assessing the Irritancy of Toilet Soaps and Soap Ingredients.

In the present study soap chamber test was carried out for assessing irritancy of toilet soaps and soap ingredients. The results obtained in the present study are summarized as follows; This chamber test can discriminate the differences of irritancies between various soaps and soap ingredients. 2. Soap irritancy is not directly related with its alkalinity. 3, Erythema score and scaling score tend to parallel each other with weak correlation(r=p 46), but erythema score and fissuring score show strong correlation (r=p77) Scaling score and fissuring score show moderate correlation(r=p.64), 4. In the eighteen kinds of toilet soaps, two soaps are most irritating, harsh soap and the total irritancy scores of all soaps are within 1. 9 5. 1. Baby soaps are not milder than the common toilet soaps. 5. Generally, total irritancy score of soap ingredients is concentration dependent. Average total irritancy scores of five main soap ingredients in 4 times of recommended allergen concentration are as follows; Builders 5. 30, Perfumes l. 88, Color 0. 70, Fatty acids p. 35 and Superfat p.

3.Two Cases of Aplasia Cutis Congenita.

Aplasia cutis congenita is an anomaly characterized by absence of localized areas of the integument. The most common type of aplasia cutis congenita is a circular or oval, sharply outlined ulcer 1 to 3cm in diameter on the vertex of the scalp at the level of the posterior fontanel. Two cases of aplasia cutis congenita are presented. The first case was in a male newborn infant with a localized defect on his scalp. The second case was also in a male newborn infant who had extensive symmetrical involvement of his heels. No similar conditions and associating other congenital anomalies were found in the family members of reported cases.

4.The Use of May Anatomical Bone Plate in The Treatment of Distal One

Many modalities of the treatment of fractures of distal 1/3 of tibia were introduced. But, May anatomical bone plate is considered to be a valuable method in treating distal 1/3 tibial fractures because of reduction potentiality by their contour, ability of firm fixation, decreased complications. Twenty two patients having distal 1/3 tibial fractures, were treated by May anatomical bone plate, at the Department of Orthopedic Surgery, Inchon Chirstian Hospital from January 1988 to December 1993. The results were follows; 1. Ninteen cases(86%) out of 22 were closed fractures and the remaining 3 cases(14%) were open fracrues, the comminuted fractures were 6 cases(22%) and the segmental fractures were 2 cases(9%). 2. Average interval from injuy to operation was 16.8 days and 21 cases had associated injuries. 3. The mean duration of bone union was 16.9 weeks. 4. Seven cases(31.8%) of delayed union and two cases(9.1%) of superficial infection were noted.

5.A Case of Multiple Cutaneous Leiomyoma.

We present a case of multiple cutaneous leiomyoma in a 45 year old woman. About 20 years ago, multiple skin colored papules developed on right cheek and man4ibular area. About 10 years after the onset of the disease a uterine myoma developed which wae tree,ted by total hysterectomy. The skin lesions were gradually increasing in number and size, Some larger nodular lesions changed to brown in color and became painful after cold exposure. The histopathologic finding showed typical leiomyoma arieing from arrector pilorum muscle.

6.Cutaneous metastatic adenocarcinoma of stomach : nodular and inflammatory carcinoma.

A 65-year-old male patient presented multiple cutaneous nodules on trunk and an erythematous inflammatory lesion resembling erysipelas on left side of neck. Result of skin biopsies disclosed groups of metastatic adenocarcinoma cells in the dermis, subcutaneous tissue, and lumina of the lymphatic vessels. Histopathologic examination of the stomach, along with the gastroscope, established that the cutaneous metastasis was of stomach origin.

The bleeding duodenal varices ia uncommon and frequeintly present a difficult diagnostic problem While the bleeding duodenal varices is a complication of portal hypertension, the possibility of bleeding duodenal varices in the absence of eaophageal varix bleeding should be kept mind. For the surgeon, the mikin technical problem is that even when identified by frequently impalpable, and invisible to the naked eye unless they are actively bleeding at the time of surgery. We described simple intra-operative endoscopic technique successfully to identify and bleeding varices in the duodenal third portion prior to the partial duodenal resection. This report summarized our experience and review of literature.

Angiodysplsia of the small bowel is uncommon, but provably remains frequently undiagnosed. In the small intestine, angiodysplasia presents a taxing surgical problem and is uauslly diagnosed for unexplained gastrointestinal bleeding. For the surgeon, the main technical problem is that even when identified by frequently impalpable, and invisible to the naked eye unless they are actively bleeding at the time of surgery. Ensocopy is often unfruitful because the majority of lesions are submucosal and rarely exceed a few milimeters in diameter. We described a simple intraoperative endoscopic transillumination technique used successfully to identify an angiodysplasia in the jejunum prior to the small bowel resection. This report summarized our experience and review of literature.
