1.Stasis Dermatitis: A Case Report.

A case of stasis dermatitis with hepatomegaly is reported. The patient is 50 year old male. About 3 years ago a dark-brownish macule on the left shin was appeared with the severe itching sensation and spread to the margin with edema, ulceration, and appeared the varricous veins on the margin, Biopsy on the ulceration revealed the hemociderin deposition and fibrosis in the dermis. The physical examination revealed that the liver was enlarged 3cm, below the right costal margin and liver biopsy showed early cirrhotic change, but without impairment of liver function. He was treated with massage, elastic bandage for 2 weeks with good response.

2.Posterior Impingement Syndrome of the Ankle Joint in Classical Ballet Dancer.

In the classical ballet dancers, ankle joints are frequently overused. Especially the Pointe and the Releve are the two basic steps of ballet dance which cause the narrowing of the posterior aspect of the ankle joint. Posterior impingement syndrome of the ankle joint is defined as impingement occuring at the anatomical interval between the posterior tibial articular surface and os calcis. The purpose of this study is to describe the anatomy and etiology of this injury and to review the non-operative and operative treatments. Sixteen cases of posterior impingment syndrome had been treated between March 1994 and March 1995. Ten dancers were professional and six were students. The patients was divided 3groups by etiologic factor. Group 1 is anatomical problem factor, Group 2 is overuse factor and Group 3 is ankle sprain factor. Thirteen dancers (81.2%) improved with conservative treatment including local steroid injection. Among the other three dancers, operation was performed in one case. The other two cases stopped dancing. The result was good in one dancer after operation. In the classic ballet dancers, posrerior impingement syndrome was frequent disorder and major etiologic factors are anatomical problem and overuse. Operation was necessary in only group l. It is good result in conservative treatment and operation.

4.Association of Herpes Zoster and Lymphosarcoma: Report of one Case.

Herpes zoster is generally believed to be caused by the activation of varicella-zoster virus present in the body since an original infection with varicella. According to the thesis, the virus remairis in a latent state in the cells of the sensory ganglia until immunity has waned sufficiently to pezmit multiplication of the virus and clinical infection. There are a number of reports that in patient with malignant disease, especially Jymphoma, a frequenry af association of zoster is greater and severity of its symptom is increased to be marked contrast to the benign uneventful course of zoster seen in healthy person. The author observed one case of zoster seen in patient with malignant disorder, and frequency of association of zoster in malignant disease and its etiological factors are reviewed in the literature.

5.Clinical Review of 57 Dead Cases of Tuberculous Meningitis in Children.

The following results were obtained through the clinical review of 57 dead cases of tuberculous meningitis which were treated at Mokpo Children's T.B. Hospital during the period of 7 years from January 1970 till December 1976. 1) Mortality rate was higher in the younger age group. 2) Seasonal incidence was highes in summer, and next in autumn. 3) Tuberculin test revealed positive reaction in 42.3%, which was much lower than that of other tuberculous patients. 4) The frequent clinical symptoms on admission were in the order of nuchal rigidity, fever and vomiting. 5) In studying the relationship between the finding of CSF and the prognosis, the following was noted:Mean Leukocyte count of dead cases increased, mean protein level increased, and mean chloride level decreased more than those of other patients of tuberculous meningitis. 6) No significant relationship was found between the tuberculousis of outside of meninges and morality rate. 7) The later the treatment started from the onset of disease, the higher the stages of disease on treatment were, the higher the morality rate.

6.Clinical Review of Miliary Tuberculosis in Children.

Clinical studies were made on 83 miliary tuberculosis at Mokpo Children's T.B. Hospital during the period of January, 1971 through December, 1976. The results were as follows. 1. The highest incidence was noted in the group aged 1 to 3 years of age and 78.3% of all cases was under 6 years. 2. The incidence in summer was noted to be the highest and reverse in the fall. 3. The 26 cases (31.3%) had another T.B. patients in the family, most of them were their parents. 4. As the result of tuberculin skin test, the 45 cases(59.2%) of total 76 cases were noted as positive reaction, except uncertain 7 cases. 5. the chief complaints on adimission were as following orders such as cough, fever, general weakness, vomiting, generalized convulsion, dyspnea etc. 6. The positivity of A.F.B. in sputum or gastric juice was found in 42.2% from direct smear and culture. 7. According to hematologic examination, ESR increased and anemia appeared mostly. Increased leukocyte count was noted in 55.4%. 8. Among complications and associated diseases, tuberculous meningitis was the most as38.6% out of all cases, and the next was tuberculous lymphadenitis. 9. The miliary shadow on X-ray finding in 82.5%disappeared within 4 months after admission, and it persisted over 7 months in 2 cases. 10. The treatment was, initially as a rule, done as triple therapy with INH, PAS and SM in 59 cases, and the secondary tuberculous regimens were administered to 24 cases. The rate of death was noted as 31.3%(26cases), and most of them were under 3 years of age.

7.The Effect of CO2 Insufflation and Trendelenburg-lithotomy Position on Intraocular Pressure during Laparoscopy.

BACKGROUND: The laparoscopy requires carbon dioxide (CO2) insufflation and Trendelenburg position for operational convenience. However, the above circumstances affect the cardiopulmonary systems significantly and intraocular pressure (IOP) may be also influenced. METHODS: In 27 non-glaucoma patients right and left intraocular pressure (RIOP, LIOP) were measured 5 minutes after induction of general anesthesia (control value), 15 and 30 minutes after CO2 insufflation and endelenburg-lithotomy position. RESULTS: The control values of RIOP and LIOP were 11.3 4.7 mmHg and 11.5 4.7 mmHg respectively. At 15 minutes after CO2 insufflation and Trendelenburg-lithotomy position, they increased to 16.6 5.3 mmHg and 17.0 5.9 mmHg (p<0.05). At 30 minutes, 18.4 3.5 mmHg and 18.2 4.1 mmHg (p<0.05). CONCLUSION: CO2 insufflation and Trendelenburg-lithotomy position increase IOP significantly in non-glaucoma patients during laparoscopy.

10.An Experimental Study on the Influence of DDVP upon the Kidney.

With the progress of farming and foresting, the demand of agricultural pesticide has been remarkably increased and accordingly, its intoxication has been widely publicized. Among the pesticides, organophosphorus chemicals has been believed to be almost nontoxic because they are not accumulative in the tissue. This study is attempted to examine the effects of DDVP which has been used most frequently on the kidney. And they were divided into normal and experimental groups. The results obtained from this experiment are as follows. 1) BUN, serum creatinine are increased in the group of DDVP administered while serum cholinesterase is suppresed significantly. 2) Microscopically, swelling and degeneration of tubular cells and hyaline casts in tubules. Ultrastructually swelling of mitochondria, partial fragmentation of cristae and electron dense bodies in the kidney are observed in the group of DDVP administered.
