1.Surgical Treatment of the Metastatic Tumors of the Femur.

Metastasis to bone with destruction of the skeletal system is a common problem in old patients with malignancy. Destruction of the musculoskeletal system poses major problems for the patients, including uncontrollable pain, forced immmobilization, pathologic fracture and hypercalcemia. The purpose of this study is to evaluate the effectiveness of surgical treatment and to suggest treatment guidelines for impending or established pathologic fractures due to metastasis of the femur. Between 1985 and 1995, thirty six patients who had pathologic or impending fractures in the femur underwent surgical treatment. The indications of surgery for pathologic fractures are 1) the possibility of survival time more than three months, 2) more benefits of sungical treatment than those of conservative treatment, 3) the possibility of rigid internal fixation, 4) patient s general condition allowing surgery, and those in impending fractures are 1) cortical destruction more than 50% or lytic lesion more than 2.5cm. The method of surgery was excision of the tumor and rigid internal fixation to allow early ambulation with or without bone cement augmentation. The results were analyzed by three criteria, the performance status, pain relief and survival time. Pain relief more than Lots was achieved in 80.6% of patients according to the Kaiko's criteria. The performance status improved more than one grade in all patients except one who died of acute respiratory failure and the mean improvement was 1.8 grade by the Functional Classification of the New York Heart Association. Postoperatively, most of the patients had only slight to moderate limitation of physical activities and felt comfortable at rest and ordinary activities. The survival time was 20.9 months in patients with single metastasis and 10.2 months in patients with multiple metastases. But there was no statistical significance due to large deviation of survival time in each case. In summary, surgical stabilization of the pathologic lesion involving the femur provided definite improvement of quality of life including pain relief and early ambulation. In selected patients with single metastasis, low grade malignancy, or good performance status, postoperative survival time may be expected to be prolonged. More aggressive treatment for the metastatic lesion of the femur is suggested.

2.Promoter Polymorphism (rs12770170, -184C/T) of Microseminoprotein, Beta as a Risk Factor for Benign Prostatic Hyperplasia in Korean Population.

PURPOSE: Benign prostatic hyperplasia (BPH) is the most common prostate disease in aging men. Microseminoprotein-beta (MSMB) is abundant in semen. In this study, we investigated association between single nucleotide polymorphisms (SNPs) at the promoter of the MSMB gene and the risk for developing BPH in a Korean population. METHODS: We genotyped two promoter polymorphisms (rs12770171, -184C/T and rs10993994, -2C/T) of the MSMB gene by direct sequencing. Ninety-five BPH patients and 78 control subjects were recruited for this study. SNPStats and Haploview version 4.2 were used for genetic analyses. Multiple logistic regression models (codominant, dominant, recessive, and log-additive models) were applied to determine the odds ratio (OR), 95% confidence interval (CI), and P-value. RESULTS: Genotype frequency of the rs12770171 SNP showed significant difference between BPH patients and controls (OR, 2.14; 95% CI, 1.07-4.27; P=0.032 in the codominant 1 model; OR, 2.31; 95% CI, 1.19-4.47; P=0.011 in the dominant model; and OR, 2.05; 95% CI, 1.17-3.61; P=0.009 in the log-additive model). Moreover, the SNP also showed association between the two groups (OR, 2.05; 95% CI, 1.19-3.52; P=0.009). The rs10993994 SNP was not associated with BPH. In haplotype analysis, CC and TT haplotypes were associated with BPH (P<0.05). CONCLUSIONS: This result indicates that a promoter polymorphism (rs12770170, -184C/T) in the MSMB gene may be associated with BPH development in a Korean population.

3.A Case of Yellow Nail Syndrome.

We report a 58 year old man who complained that his nails didn't grow for a year and developed yellowish discoloration. On physical examination his nails showed yellow greenish discoloration, increased corvexity, loss of lunulae and cuticles. C]iest X ray revealed atelectasis on right lower lung field and bronchiectasis on left lower lung field. Mild restrictive pattern was observed in pulmoniry function test. We gave him 800IU of Vitamin E claily for 3 months, but rio remarkable changes have been observed as yet..

10.Shape-up LUTS/BPH Guideline in Korea: Medical Treatment.

Benign prostate hyperplasia (BPH) is the most common cause of lower urinary tract symptoms (LUTS) in men older than the middle age, and is a pathologic process that contributes to lower urinary tract symptoms in aging men. Despite the significantly different proportion of men with moderate to severe symptoms, a clear trend toward an increase in symptom scores with advancing age is noticeable. Complications of LUTS and BPH, such as urinary tract infections, bladder decompensation, bladder stones, hematuria, urinary incontinence, upper urinary tract deterioration with renal insufficiency, and others are in general rare in properly supervised patients. Medical therapies investigated for BPH include alpha-adrenergic blockers, 5alpha-reductase inhibitors, combination, aromatase inhibitors, and numerous plant extracts. Medical therapy is now the usual first- line management of uncomplicated BPH with either an alpha-adrenergic blocker or (if the prostate is large) a 5alpha-reductase inhibitor. Combination therapy with both an alpha-adrenergic blocker and a 5alpha-reductase inhibitor has been demonstrated to be the most effective means of preventing disease progression.
