1.The levels of nerve growth factor and IL-4 in induced sputum and characteristics of airway inflammation in cough variant asthma

Objective To observe sputum cytology counts, the levels of nerve growth factor (NGF) and IL-4 in cough variant asthma (CVA) patients and the change of their levels after using glucocorticoids combined with β2-adrenergic agonists one month, and to investigate CVA's characteristics of airway inflammation. Methods Totally 36 patients with untreated CVA were selected, as well as 23 healthy controls. Coughed up sputum cells were obtained and HE strained for differential cell counting in each enrolled patient. In induced sputum's supernatant, the levels of NGF and IL-4 were determined by ELISA.Results Before treatment, CVA patients had a median eosinophils (EOS) percentage of 8%, which was significantly higher than that after treatment (2%, P<0.05) and in healthy control group (1%, P<0. 001). The levels of NGF and IL-4 in induced sputum of CVA group were (9. 50 ± 1.69) ng/L and (257.37 ± 53.57) ng/L. After treatment, they were (8.78±1.02) ng/L and (228.60 ±52.93)ng/L in CVA group, (6.98±0.69) ng/L and (166.44±24.75) ng/L in healthy control group. The levels of NGF and IL-4 before and after treatment in the CVA group , as compared with the healthy control group, had statistically significant differences (all P<0.001). In CVA group before and after treatment, the level of NGF and IL-4 paired difference was significant (P<0.001). The percentage of induced sputum EOS correlated with sputum supernatant concentrations of NGF and IL-4 (P < 0.01). In induced sputum supernatant, the concentrations of NGF and IL-4 were significant correlated (P<0.01). Conclusions Glucocorticoid joint long-term β2 agonist inhaled treatment significantly reduced NGF, IL-4 and EOS levels and reduced eosinophilic inflammation, which are closely related with the nerve-immune mechanism, NGF as well as IL-4 participated the inflammation. Induced sputum examination is non-invasive, economical,simple, easily accepted by patients, and repeatable, widely used in clinical.
2.Significance of Hidden Rheumatoid Factor IgM,Anti-Cyclic Citrullinated Peptide Antibodies,Antikeratin Antibodies,Combined Test in Diagnosis of Juvenile Idiopathic Arthritis

Objective To explore the significance of hidden rheumatoid factor-IgM(HRF-IgM),anti-cyclic citullinated peptide antibodies(anti-CCP),antikeratin antibodies(AKA)combined test in the diagnosis of juvenile idiopathic arthritis(JIA).Methods The anti-CCP and rheumatoid factors(RF)were tested by enzyme linked immunosorbent assay(ELISA)in serum samples from 60 children with JIA,27 systemic lupus erythematosus(SLE),18 kawaski diseases and 50 healthy children who were enrolled to serve as normal controls.Indirect immumofluorescence method was used to test AKA.Using the QAE-Sephadex-A50 to separate RF then tested HRF-IgM by ELISA.Results Positive anti-CCP were found in sera of 13 patients with JIA(21.7%)and specificity 96.8%.RF were found lower test both with the JIA and other groups(?2=0.17 P=0.68).The results of AKA were similar to the anti-CCP,they were both lower sensitivity and higher specificity.There were 65% sensitivity and 85.3% specificity in the HRF-IgM,and great significant difference in positive rate in JIA and other group(?2=40.22 P=0.0001).Conclusions JIA children in the studied series has a higher detection rate and value of HRF-IgM as compared to the other control groups.HRF-IgM,anti-CCP,AKA combined test in JIA can prove to be a valuable reference index for diagnosing suspected JIA children.
3.Anterior corpectomy decompression and titanium mesh bone iraft fusion combined with titanium nate fixation for the treatment of the multilevel cervical spondylotic myelopathy.

OBJECTIVETo explore the clinical effects of anterior corpectomy decompression and titanium mesh bone graft fusion combined with titanium plate fixation in treatting multilevel cervical spondylotic myelopathy.
METHODSThe clinical data of 48 patients with multilevel cervical spondylotic myelopathy underwent surgical operation were retrospectively analyzed from October 2010 to January 2013. There were 37 males and 11 females, aged from 37 to 76 years old with an average of 54.6 years. Thirty-five cases were two-segment lesion, 7 cases were three-segment lesion, 6 cases were four-segment lesion. All the patients were treated by anterior corpectomy decompression and titanium mesh bone graft fusion combined with titanium plate fixation. ROM, JOA, VAS and SF-36 scores were recorded before and after operation(including 3, 6, 12 months after operation and final follow-up). Fusion degree and spinal canal decompression condition were observed by radiographic data.
RESULTSAll patients were followed up from 14 to 48 months, with an average of 27.3 months. At 12 months after surgery, radiographic data showed that all patients obtained bony fusion, spinal canal decompression were sufficient. Preoperative vertebral canal sagittal diameter of the most serious segment were (5.13 +/- 1.32) mm, 12 months after surgery were (9.94 +/- 1.22) mm, there was statistically significance (t=2.463, P=0.014); the degree of vertebral canal decompression were (92.15 +/- 2.35)%. Postoperative ROM, JOA, VAS and SF-36 scores were obviously improved than that of preoperative (P<0.05); there was no statistically significance of ROM, JOA, VAS and SF-36 scores in each time after operation (P>0.05).
CONCLUSIONAnterior corpectomy decompression and titanium mesh bone graft fusion combined with titanium plate fixation can obtain higher fusion rate, complete thoroughly decompression, improvement of clinical symptoms and well safety in treating multilevel cervical spondylotic myelopathy.

4.An analysis of factors for quality-of-life in patients with chronic cough

Objective To constitute a correlation with the subjective indicators by investigation of the causes and clinical features in patients with chronic cough.Methods Totally 100 patients with chronic cough were recruited followed a diagnostic program.Airway responsiveness[by methacholine challenge test (MCT)],Leicester cough questionnaire(LCQ),visual analogue scale(VAS),cough score,age,gender and disease duration were all recorded for analysis.Results The top five causes of chronic cough in these patients were variant asthma,post infectious cough,atopic cough,eosinophilic bronchitis and upper airway cough syndrome.LCQ total score was negatively correlated with age and the VAS score(r=-0.239 and -0.470 respectively,all P<0.05),while no difference was found among patients with different causes of disease or gender(F=1.233,t=1.918,all P>0.05)and no correlation was found with BMI(r=-0.029,P>0.05).The physiological and psychological field score in female patients significantly reduced(t=2.174,1.990,P<0.05),and LCQ total score of MCT positive patients obviously reduced than negative ones(t=-2.22,P<0.05).After the treatment of two weeks,LCQ three component field and total score could be improved significantly(all P<0.01).Conclusion Gender and age may have some impact on the quality of life in patients with chronic cough.LCQ,VAS and cough score should be used to assess cough severity and evaluate therapeutic effect in patients with chronic cough.
5.Simultaneous Determination of Hydroxysafflor Yellow A and Aalvianolic Acid B in Compound Danshen Oral Solution by HPLC

OBJECTIVE:To establish a method for simultaneous determination the contents determination of hydroxysafflor yel-low A and salvianolic acid B in Compound danshen oral solution. METHODS:HPLC was performed on the column of SHIMADZU ODS-C18 with mobile phase of methanol-0.5% phosphoric acid (35:65,V/V) at flow rate of 1.0 ml/min,dection wavelength was 403 nm for hydroxysafflor yellow A and 286 nm for salvianolic acid B and column temperature was 35 ℃, the volume injection was 20μl. RESULTS:The linear range was 2.01-20.10μg/ml for hydroxysafflor yellow A(r=0.999 9)and 39.80-398.00μg/ml(r=0.999 9) for salvianolic acid B. RSDs of precision,stability and reproducibility tests were lower than 2%;recoveries were 98.26%-101.25%(RSD=0.94%,n=9)and 98.70%-101.35%(RSD=0.71%,n=9),respectively. CONCLUSIONS:The method is feasible and reproducible,and can be used for the contents determination of hydroxysafflor yellow A and salvianolic acid B in Com-pound danshen oral solution.
6.The lipid-lowering effect and possible mechanism of apobec-1 in nephrotic syndrome rabbits

Objective To observe the effect of liver acquired expression of apobec-1 on blood lipid metabolism, hepatic low density lipoprotein receptor (LDLR), and hepatic low density lipoprotein receptor related protein (LRP) in nephrotic syndrome(NS) rabbits and to explore the lipid-lowering effect and possible mechanism.Methods Thirty healthy ordinary level male new Zealand rabbits were randomly divided into 3 groups:sham operation group (n =10), N S group (n =10), and apobec-1 treatment group (n =10).Adaptive feeding was given for 1 week, then NS group and apobec-1 treatment group underwent left nephrectomy,and 1 week later,adriamycin (4 mg/kg) was used to construct the NS model by way of ear vein injection.In the 11th week after operation, apobec-1 recombinant adenovirus (1 × 1013 virus/kg) was injected through ear vein in apobec-1 treatment group.The 12th week after operation, all rabbits were sacrificed.Right kidney, liver, blood and 24 hour urine were collected.In 3 groups, 24 hour urinary protein (24UPr), albumin (Alb), blood urea nitrogen (BUN), creatinine (Cr), blood lipid were detected.Renal pathology was observed by means of HE staining.Expressions of liver LDLR, LRP were observed by using Western blot.Results (1) There were significant differences among the 3 groups in 24UPr (F =42.778, P =0.000), Alb (F =3.819, P =0.034), BUN (F =6.562, P =0.005), Cr (F =16.076, P =0.000), total cholesterol (TC) (F =17.531, P =0.000), total triglyceride (TG) (F =6.192, P =0.006), very low density lipoprotein (VLDL-C) (F =6.192, P =0.006), low density lipoprotein (LDL-C) (F =34.924, P =0.000) and apolipoprotein B100 (ApoB100) (F =5.180, P =0.012) and apolipoprotein B48 (ApoB48) (F =6.161, P =0.006).(2) Compared with the sham operation group, NS group showed that 24UPr, BUN, Cr, TC, TG, VLDL-C, LDL-C, ApoB100 increased, but Alb decreased, and there was statistical significance (all P < 0.05).(3) Compared with NS group, apobec-1 treatment group showed that TC, TG, VLDL-C, LDL-C, ApoB 100, ApoB48 decreased, and there were statistical significances (all P < 0.05).(4) Compared with the sham operation group, apobec-1 treatment group showed that 24UPr, BUN, Cr increased, but Alb, apolipoprotein B48 (ApoB48) decreased, there were statistical significances (all P < 0.05).(5) There were significant differences of hepatic protein expression among the 3 groups in LRP (F =44.180, P =0.000), LDLR (F =63.141 ,P =0.000).Compared with the sham operation group and NS group, apobec-1 treatment group showed that the expression of LRP was up-regulated (P < 0.01,0.05), while the expression of LDLR was down-regulated (all P < 0.05).Conclusions When liver acquired expression of apobec-1 in NS, it could up-regulate LRP,accelerate the elimination of ApoB48-lipoproteins, and produce a certain lipid-lowering effect.
7.Experimental study of Apobec-1 recombinant adenovirus in treatment of renal hyperlipemia

Objective To explore the effect of the liver acquired expression of apolipoprotein B editing catalytic polypeptide-1 (Apobec-1) on hyperlipidemia of renal disease. Methods Thirty healthy ordinary level male New Zealand rabbits were randomly divided into three groups: sham operation group, nephropathy group, and Apobec-1 treatment group (Each group has 10 rabbits). Adapt feeding for one week, nephropathy group and Apobec-1 treat-ment group underwent left nephrectomy, and one week later, adriamycin (4 mg / kg) was used to construct the ne-phropathy model by ear vein injection. The eleventh week after operation, apobec-1 recombinant adenovirus (1 × 1013 Virus/ kg) was injected by ear vein in apobec-1 treatment group. The twelfth week after operation, all rabbits were sacrificed. Right kidney, liver, blood and 24h urine were left. In three groups, 24 hour urinary protein (24UPr), albumin (Alb), blood urea nitrogen (BUN), creatinine (Cr), blood lipid were detected. Renal pa-thology was observed by HE staining. Expressions of liver apobec-1, apolipoprotein B48(ApoB48)were observed by Western blot. Results ① Compared with the sham operation group, nephropathy group showed that 24UPr, BUN, Cr, total cholesterol (TC), total triglyceride (TG), very low density lipoprotein (VLDL-C), low density lipoprotein (LDL-C), apolipoprotein B100 (ApoB100) were increased(P < 0. 05), but Alb was decreased(P <0. 05). ② Compared with the nephropathy group, Apobec-1 treatment group showed that TC, TG, VLDL-C, LDL-C, ApoB100, ApoB48 were decreased(P < 0. 05). ③ Compared with the sham operation group, Apobec-1 treat-ment group showed that 24UPr, BUN, Cr were increased( P < 0. 05), but Alb, ApoB48 were decreased( P <0. 05). ④ Compared with the sham operation group and nephropathy group, Apobec-1 treatment group showed that the expression of Apobec-1 and ApoB48 were up-regulated (P < 0. 01). Conclusion When liver aquires expres-sion of Apobec-1 in hyperlipidemia of renal disease, it can reconstruct ApoB mRNA, increase the synthesis of ApoB48-lipoprotein, and play a certain lipid-lowering effect.
8.Expression of PPAR-γ in patients with chronic rhinosinusitis without or with nasal polyps.

OBJECTIVE:
To investigate the expression of peroxisome proliferator-activated receptor γ (PPAR-γ) in the sinus mucosa of chronic rhinosinusitis without nasal polyps (CRSsNP) and chronic rhinosinusitis with nasal polyps (CRSwNP).
METHOD:
Using immunohistochemistry and RT-PCR, testing the content of PPAR-γ in sinus mucosa and nasal polyps of patients with chronic sinusitis without nasal polyps and normal inferior turbinate, and analyzing the differences.
RESULT:
The PPAR-γ was found both in sinus mucosa of CRSsNP, nasal polyps of CRSwNP and in normal inferior turbinate; (2) The expression of PPAR-γ in sinus mucosa of CRSsNP and nasal polyps of CRSwNP was lower than that in normal inferior turbinate tissue, and the differences was prominent in statistics; (3) No differences in the expression of PPAR-γ were obtained in sinus mucosa of CRSsNP and nasal polyps of CRSwNP.
CONCLUSION
The PPAR-γ expressed in human nasal mucosa, meanwhile it was descending in sinus mucosa of CRSsNP and nasal polyps of CRSwNP. The long-term inflammations resulted from CRSsNP and nasal polyps of CRSwNP might relate to PPAR-γ, it suggested that PPAR-γ agonists may offer a new therapy for the treatment of Chronic Rhinosinusitis.
