1.Effects of rapid weight reduction on the body water and nitrogen balance in wrestlers.

Six wrestlers, whose body weights should be reduced 4-11% in relatively short period of time to be acknowledged for intercollegeate 62 kg class competition, were selected and their daily food intakes in regulation, reduction through recovery period of body weights were estimated. Simultaneously, the weights of whole urine and feces were estimated, and some urine components were analysed.
Nutrients intake except calsium and iron were maintained allowance level in regulation period, and reduced through reduction period. On third day of recovery period, the body weights were recovered to their own level, but through the period, only the intake of energy, water, sodium and vitamin A were recovered to the level of regulation period. Personal deviations in their intakes of vitamin B complex and C were distinkt, and the iron intake was under level through all period.
The average body water contents was 68%, estimated by D20 method. Negative water and nitrogen balance through regulation and reduction period turned to positive in recovery period, but not completely recovered to their nomal level.

3.Results of gastric mass survey in Tsuchiura Kyodo Hospital.

We began gastric mass survey at our hospital in May, 1980. During 3 years and 8 months since then, (May, 1980-December, 1983) we performed screening examinations to 16, 341 people by indirect radiography, and checked 2, 824 cases (17.3%) for thorough examination. Among these cases, 2, 083 (73.8%) received endoscopic examination actually, and 55 cases of gastric cancer were discovered. The discovery rate of gastric cancer was 0.336 per cent.
35 cases of these 55 gastric cancer were operated in the surgical department of our hospital. 34 cases were resected (rate of resection was 97.1%) and 33 cases were resected curatively (rate of curative resection was 94.3%). These results were better than that of gastric cancer cases from out-patient clinic of the same period. The results of cases from out-patient clinic of our hospital were: total number of cases 321, resected cases 254 (79.1%), curative resection 189 cases (58.9%), respectively.
4.NUTRITIONAL AND PHYSIOLOGICAL STUDIES ON THE RAPID WEIGHT REDUCTION IN WRESTLERS

Seven wrestlers, whose body weights should be reduced about ten per cent in relatively short period of time to be acknowledged for intercollegeate competition, were selected, and their daily food intakes in reduction through recovery period of body weight were estimated. Simultaneously, some blood and urine components were analysed, performance capacities (grip and back strength power) were measured, and gross balances of nitrogen and potassium were calculated as the difference between their intakes and urinary outputs, to discuss the influence of rapid weight reduction.
For several days of the last stage of reduction period, the average intakes per kg body weight per day were about 20 kcal for energy, about 1.7 g for protein, about 20 g for food moisture and drinking water, total intakes per day were about 2 g for sodium and about 1 g for potassium respectively.
During about seven days of recovery period, these intakes were increased to higher level, which were as average about 60 kcal for energy, about 2 g for protein, 46-73 g for food moisture and drinking water per kg body weight, about 5 g for sodium and about 3 g for potassium per day, respectively.
During reduction period, the negative balances of nitrogen and potassium, slight reduction of performance capacities and dehydration symptoms in blood components (Ht value, and contents of Hb, plasma protein plasma urea nitrogen) were observed.
During recovery period, the body weights were recovered rapidly to normal level, nitrogen and potassium were turned to positive balance, but some components (Ht value, plasma protein) were not completely recovered to their normal level.
As far as the average food composition in recovery period was concerned, not so remarkable deficiency was observed, but the personal deviations in their intakes were distinct. From these results, it is suggested that by better balanced combination of food intake, personal body conditions of these young sportsmen with high physical strength would recover to their normal level more quickly and more completely.
5.The Safety and Immune-efficacy of Fucoidan from Gagome Kombu (Kjellmaniella crassifolia) on Women with the History of Gynecological Cancer

Objective: Fucoidan is a sulfated polysaccharide in brown seaweeds. Previous studies show that fucoidan from Gagome kombu (GKF) exhibit immune-enhancing capabilities on healthy adults. In this study, we focused on the women with the history of gynecological cancer and evaluated the safety and immune-efficacy of GKF.Methods: Ten Japanese women subjects were chosen to ingest the test samples (1 drink / day containing 200 mg GKF) for 4 weeks. Before and after ingestion, blood chemistry analysis, hematological analysis, urinalysis and immune analysis were conducted. Result: Results showed no adverse clinical changes in blood and urinary analysis. In addition, no serious symptoms were observed. Moreover, decrease of serum levels of Eotaxin, IL-7, IL-17 and VEGF were observed.Conclusion: These results indicate that in the case of women with the history of gynecological cancer, GKF is a safe functional food ingredient.
6.Total Cholesterol Level for Assessing Pancreatic Insufficiency Due to Chronic Pancreatitis.

BACKGROUND/AIMS: To determine the nutritional markers important for assessing the degree of pancreatic insufficiency due to chronic pancreatitis in routine clinical practice. METHODS: A total of 137 patients with chronic pancreatitis were followed up for more than 1 year. They were divided into two groups: a pancreatic diabetes mellitus (DM) group, consisting of 47 patients undergoing medical treatment for DM of pancreatic origin, and a nonpancreatic DM group, consisting of 90 other patients (including 86 patients without DM). Serum albumin, prealbumin, total cholesterol, cholinesterase, magnesium, and hemoglobin were compared between the two groups. RESULTS: The total cholesterol was significantly lower in the pancreatic than the nonpancreatic DM group (164 mg/dL vs 183 mg/dL, respectively; p=0.0028). Cholinesterase was significantly lower in the former group (263 U/L vs 291 U/L, respectively; p=0.016). Among the 37 patients with nonalcoholic pancreatitis, there was no difference in the cholinesterase levels between the pancreatic and nonpancreatic (296 U/L vs 304 U/L, respectively; p=0.752) DM groups, although cholesterol levels remained lower in the former (165 mg/dL vs 187 mg/dL, respectively; p=0.052). CONCLUSIONS: Cholinesterase levels are possibly affected by concomitant alcoholic liver injury. The total cholesterol level should be considered when assessing pancreatic insufficiency due to chronic pancreatitis.

7.A Novel, Fully Covered Laser-Cut Nitinol Stent with Antimigration Properties for Nonresectable Distal Malignant Biliary Obstruction: A Multicenter Feasibility Study.

BACKGROUND/AIMS: Stent migration occurs frequently, but the prevention of complications resulting from covered self-expandable metal stents (C-SEMSs) remains unresolved. We prospectively assessed a newly developed C-SEMS, a modified covered Zeo stent (m-CZS), in terms of its antimigration effect. METHODS: Between February 2010 and January 2011, an m-CZS was inserted into 42 patients (31 initial drainage cases and 11 reintervention cases) at a tertiary referral center and three affiliated hospitals. The laser-cut stent was flared for 1.5 cm at both ends, with a 1 cm raised bank located 1 cm in from each flared end. The main outcome of this study was the rate of stent migration, and secondary outcomes were the rate of recurrent biliary obstruction (RBO), the time to RBO, the frequencies of complications, and overall survival. RESULTS: Of the 31 patients with initial drainage, stent migration occurred in four (12.9%, 95% confidence interval, 5.1% to 29.0%), with a mean time of 131 days. RBO occurred in 18 (58%), with a median time to RBO of 107 days. Following previous C-SEMS migration, seven of 10 patients (70%) did not experience m-CZS migration until death. CONCLUSIONS: m-CZSs with antimigration properties effectively, although not completely, prevented stent migration after stent insertion.
