1.Does the Frequency of Passive Motion Exercises during the Early Period after Arthroscopic Rotator Cuff Repair Affect the Range of Motion at 3 Months Postoperatively?

Objective : To investigate whether the frequency of passive motion exercises during the early period after arthroscopic rotator cuff repair affects the range of motion at three months postoperatively. Methods : We retrospectively evaluated 64 patients with rotator cuff tears who underwent arthroscopic repair. Range of motion (ROM) in forward flexion, abduction, external rotation, and internal rotation were assessed preoperatively and at three months postoperatively. Patients were supervised to wear an axillar pillow for six weeks and to commence passive ROM exercise at seven days postoperatively. Passive ROM exercises for 9 hours per week or more were performed in 41 patients (group A), and 3 hours or less of exercises were performed in 23 patients (group B). Results : There were no statistical differences between the two groups in age or gender proportion. The mean differences from preoperative ROM to postoperative ROM were 4° and 17° (group A and B, respectively) in flexion, 5° and 18° in abduction, -14° and -12° in external rotation, and -3 and -2 spinous processes in internal rotation, and there were no differences in all directions between the two groups. Even when subjects were limited to the patients without shoulder contractures preoperatively, there was no difference between the two groups. Conclusion : Early passive motion exercise for long hours after arthroscopic rotator cuff repair did not give a positive effect on early improvement in ROM. Scar formation and adhesion at the subacromial space may influence this outcome.
3.Nutritional Food Active Hexose Correlated Compound (AHCC) Enhances Resistance against Bird Flu

AHCC is a nutritional food that has been broadly adopted in Japan as well as other countries. Several laboratories have demonstrated that AHCC has immune modulating effect. Increasing immunity against bird flu virus, H5N1, may help to prevent the next pandemic. We hypothesize that uptaking AHCC improve immunity against infection with this virus. Administration of AHCC for 7 days effectively improved survival rate by 30%, and this effect can last for 3 to 4 weeks. Our results indicate a potential role of AHCC in helping to build up immunity for preventing the pandemic of bird flu.
4.Six Cases of Infected Abdominal Aortic Aneurysm

Infected abdominal aortic aneurysm (IAAA) are rare, but life-threatening. This time we experienced six cases of infected abdominal aortic aneurysm. We measured the soothing of bacteremia by two weeks of antibiotic treatment before operation, if not in a state of impending rupture or rupture. The in situ prosthetic graft replacement surgery was the first choice. In five cases, we replaced by an in situ dacron graft with Rifampicin. However, one case that was by pondylitis caused by Helicobacter cinaedi was treated by extra-anatomical bypass. There was no post-operative infectious complication. In addition, surgery/hospital death was 0%.
5.Hyperbaric Oxygen Treatment of Carbon Monoxide Poisoning in the Past 5 Years

Hyperbaric oxygen (HBO) therapy for acute carbon monoxide (CO) poisoning is performed after oxygen therapy and breathing therapy. The usefulness of HBO therapy in emergency treatment has been reported. In this study, we examined the effectiveness of HBO for CO poisoning that was performed at our hospital over the past 5 years. Subjects were 23 patients who had HBO therapy for CO poisoning in the period January 2008-November 2013. The male to female ratio of the cases was 14:9 and the mean age was 54.6±20.8 years. The cause was suicide in 39.1% of cases and accident in 60.9%. The mean number of treatments was 5.4±6.8. The atmosphere absolute was 2ATA:2.8ATA=12:11. The ratio of direct conveyance of the patient to our hospital to indirect conveyance of the patient from another hospital was 15:8. The effectiveness of HBO therapy at the time of discharge was 73.9%. However, delayed neuropsychiatric sequelae (DNS) was detected in 3 of the 23 cases of CO poisoning. No correlations were found between elapsed time after CO inhalation and various blood parameters. However, time to start of therapy is important for DNS, and our results suggest that early treatment at a hospital with a hyperbaric chamber is necessary.
6.A Comprehensive Study of Outcome of Bilateral Cataract Surgery Performed on Patients Living on Remote Islands, Postoperative Management at their Homes and Postoperative Complications

Purpose: We examined the safety and efficacy of cataract surgery and postoperative management in our hospital and at the homes of the patients who live on medically underserved remote.
Patients and methods: A total of 27 patients (54 eyes), who were followed in our hospital or at their homes were enrolled in this study. Cataract surgery was performed on them between January 2009 and January 2010 and we could follow up six months postoperatively. We divided these patients into two groups:group I (GI) consisted of 13 patients who could come to our hospital regularly during both preoperative and postoperative periods, and group II (GII) consisted of 14 patients who could not come to our hospital regularly during either preoperative or postoperative periods. Cataract surgeries were performed on all the patients in GI and GII in our hospital. The patients in GI were hospitalized for three days and those in GII were for seven days. After cataract surgery, the patients in GI had their eyes checked regularly in our hospital and those in GII were in their homes where the doctor visited. Postoperative ophthalmic clinical tests were conducted to examine visual acuity, intraocular pressure and fundus.
Results: GI comprised three males and 10 females. Their age averaged 79.3. GII comprised four males and 10 females. Their age averaged 82.6. Preoperative ophthalmic examinations found that preoperative average visual acuity (LogMAR and decimal visual acuity in parentheses) and spherical equivalent in GI and GII were 0.69 (0.41), 0.80 (0.33) and -0.43 dioptors, -0.42 dioptors respectively, showing no significant differences between the two groups. Postoperative ophthalmic examinations found that, best corrected visual acuity (LogMAR) was significantly increased to 0.36 (0.66) and 0.44 (0.53) in GI and GII respectively, showing no significant differences either.
Conclusions: We concluded that we could get safe and efficient cataract surgery and postoperative management combined with prolonged hospitalization and house calls on the patients who live in the isolated islands.
7.Implementation of an online-based JICA Knowledge Co-Creation Program on capacity development among African community health professionals

Introduction Asahikawa Medical University has been conducting a JICA Knowledge Co-Creation Program (KCCP) “Health Administration for Community Health Officers in Africa” in Japan since 2008. Due to the COVID-19 pandemic, the KCCP in 2020 was a hybrid of distance learning and following on-site training in Japan, which the latter was canceled thereafter. This paper reports on how this hybrid program was organized and implemented.Methods: Preparation The program was held in January-February 2021, with eight official participants from five countries and nine observers from two countries. The participants were required to engage in self-learning by PowerPoint materials with lecturers’ audio descriptions and zoom-based interactive meetings. The PowerPoint materials were freely accessible at our Google Drive account. There were 10 zoom sessions during the program.Results: Implementation At the beginning, all the lecture materials were converted to mp4 clips and uploaded to a Youtube channel due to the difficulties downloading heavy files with limited internet connection. A total of 24 videos were created with an average length of 58 minutes in each. Zoom-based interactive meetings were held regularly in the morning in the African continent. Almost all of the sessions were productive, but the participants were sometimes interrupted due to a weak network environment or their regular work assignments. Most provided contents were well understood.Discussion Despite some technical and environmental difficulties, the new-style, web-based training course facilitated the participation of observers as well as regular participants, providing more opportunities for interaction and discussion among trainees than a previous old-fashioned, on-site program. Improvement of the training environment particularly for participants is necessary to produce better training outcomes in the future, such as renting a hotel room and securing virtual connections.
8.Trend and characteristics of minimally invasive surgery for patients with endometrial cancer in Japan

Objective:
Owing to the potential benefits of minimally invasive hysterectomy for endometrial cancer, the practice pattern has recently shifted in Japan. This study examined the trends in minimally invasive surgery (MIS) in patients with endometrial cancer in Japan.
Methods:
This retrospective observational study examined the Japan Society of Obstetrics and Gynecology Tumor Registry database between 2015–2019. This study examined the time-specific proportion change and predictors of MIS use in initial endometrial cancer treatment in Japan, and compared it with the use of abdominal surgery. Additionally, the association between hospital surgical treatment volume and MIS use was examined.
Results:
A total of 14,059 patients (26.5%) underwent minimally invasive hysterectomy, and 39,070 patients (73.5%) underwent abdominal hysterectomy in the study period. Patients who underwent MIS were more likely to be treated at high-volume centers, younger, central, or western Japan residents, registered in recent years, and had a tumor with stage I disease, type 1 histology, and less myometrial invasion (all adjusted p<0.05). The proportion of MIS treatments increased from 19.1% in 2015 to 34.3% in 2019 (p<0.001). On multivariable analysis, treatment at high-volume centers was a contributing factor for MIS (adjusted odds ratio=3.85; 95% confidence interval=3.44–4.30). MIS at high-volume centers increased significantly from 24.8% to 41.0% (p<0.001) during the study period, whereas MIS at low-volume centers remained at median 8.8%.
Conclusion
MIS has increased significantly in recent years, accounting for nearly 34% of surgical management of endometrial cancer in Japan. High-volume treatment centers take the lead in performing MIS.
9.A Case in which Kampo Medicine was Effective for Pain Management after Tonsillectomy where NSAIDs and Acetaminophen were Difficult to Use

Tonsillectomy is an operation generally carried out in otolaryngology. Postoperatively, the pain is very strong and it is often difficult to eat for several days. This time, we experienced a case after tonsillectomy in which rikkosan was effective for pain management. The patient was a 23-year-old woman. With a diagnosis of habitual tonsillitis, we performed her tonsillectomy. Postoperative pain was strong, however, it was difficult to use both non-steroidal anti-inflammatory drug and acetaminophen due to the history of drug eruption. Pain was alleviated immediately after taking rikkosan Subsequently we managed pain by using rikkosan as needed. Following a good postoperative course, she was discharged 6 days after surgery.

Background/Aims:
Metabolic dysfunction-associated steatotic liver disease (MASLD) was recently proposed as an alternative disease concept to nonalcoholic fatty liver disease (NAFLD). We aimed to investigate the prognosis of patients with biopsy-confirmed MASLD using data from a multicenter study.
Methods:
This was a sub-analysis of the Clinical Outcome Nonalcoholic Fatty Liver Disease (CLIONE) study that included 1,398 patients with NAFLD. Liver biopsy specimens were pathologically diagnosed and histologically scored using the NASH Clinical Research Network system, the FLIP algorithm, and the SAF score. Patients who met at least one cardiometabolic criterion were diagnosed with MASLD.
Results:
Approximately 99% of cases (n=1,381) were classified as MASLD. Patients with no cardiometabolic risk (n=17) had a significantly lower BMI than patients with MASLD (20.9 kg/m2 vs. 28.0 kg/m2, P<0.001), in addition to significantly lower levels of inflammation, ballooning, NAFLD activity score, and fibrosis stage based on liver histology. These 17 patients had a median follow-up of 5.9 years, equivalent to 115 person-years, with no deaths, liver-related events, cardiovascular events, or extrahepatic cancers. The results showed that the prognosis for pure MASLD was similar to that for the original CLIONE cohort, with 47 deaths and one patient who underwent orthotopic liver transplantation. The leading cause of death was extrahepatic cancer (n=10), while the leading causes of liver-related death were liver failure (n=9), hepatocellular carcinoma (n=8), and cholangiocarcinoma (n=4).
Conclusions
Approximately 99% of NAFLD cases were considered MASLD based on the 2023 liver disease nomenclature. The NAFLD-only group, which is not encompassed by MASLD, had a relatively mild histopathologic severity and a favorable prognosis. Consequently, the prognosis of MASLD is similar to that previously reported for NAFLD.
