1.Evaluation of Risk Priority of the Dispensing Process by Using a Risk Management System in Community Pharmacies

Objective: It is important to prevent dispensing errors for the safety of patients. For this purpose, it is necessary to improve the dispensing process on the basis of risk priority. Quantitative evaluation of risk in each operation makes it possible to compare the risk relatively. In this study, we analyzed data reported in the Pharmacy Risk Error Management System (PREM-S) to evaluate the risk priority in the medicine preparation step of the dispensing process.
Methods: Data of 1,202 cases concerning medicine preparation step were extracted from in 2,458 cases reported into PREM-S between May 2010 and April 2011. We classified the medicine preparation step into 31 categories based on the operation. We analyzed the levels in each category as adverse effects on patient’s health by the errors and also the relationship between adverse effect and detection time. The level represents the degree of adverse effect for the patient.
Results: Information on levels of the 31 categories was obtained from the analysis. Eight categories included reports of effects on the health of patients. Three categories such as calculation of powder medicine included level 4 cases. Detection time was significantly different between level 1 and levels 2 to 4, but there was no significant difference between levels 2 to 4.
Conclusion: The results suggest that analysis of data reported in PREM-S enables evaluation of the risk priority systematically and efficiently. Evaluation of the risk priority will contribute to prevention of dispensing errors and health effects derived from them.
2.Renal Sarcoidosis Monitored with Gallium Scintigraphy: Report of a Case

A 60-year-old male was diagnosed as having sarcoidosis four months previously because of bilateral hilar lymphadenopathy seen on a chest X-ray, uveitis, and elevation of serum angiotensin converting enzyme. In summer, the patient suffered from hypercalcemia, hypercalciuria, and renal dysfunction. Renal biopsy showed interstitial nephritis with gallium scintigraphy demonstrating high uptake in the bilateral hilum, eyes, and kidneys. Corticosteroid treatment was effective in remarkably improving the renal disorder and radiographic abnormalities.
The calcium metabolic abnormalities and the granulomatous interstitial nephritis showed a marked response to corticosteroid therapy, so that immediate administration of corticosteroid may be justified even in cases without histological evidences. Gallium scintigraphy is suggested to be useful in noninvasive examination in acute stage of sarcoidosis.
3.Injuries of water polo players - In special regard to the knee joint.

102 water polo players were interviewd about the injuries caused by water polo. 13% of the subjects had knee pain at least once a year. 73% of which were chronic cases. For 24 players, clinical examinations, including X-ray examination, A-P knee laxity measurement, isokinetic strength measurement of knee muscles, and photo analysis of the vertical egg beater kick motion were performed to elucidate the cause of knee pain.
The summary of the results were as follows;
1. The most common site of knee pain was the medial portion of the knee joint.
2. Protrusion of the medial femoral epicondyle and sharpning of the tibial intercondylar eminentia as well as spur formation of the superior pole of the patella were observed in most subjects.
3. Injured subjects had stiff knees and weak knee extensor muscles.
4. The vertical egg beater kick motion must be the main factor of the painful knee.

BACKGROUND: Leber's hereditary optic neuropathy (LHON) is a mitochondrial disorder with optic nerve atrophy. Although there are no other associated neurological abnormalities in most cases of LHON, cases of "LHON plus" have been reported. CASE REPORT: The proband was a 37-year-old man who had visual and gait disturbances that had first appeared at 10 years of age. He showed horizontal gaze palsy, gaze-evoked nystagmus, dysarthria, and cerebellar ataxia. Brain and orbit MRI disclosed atrophy of the optic nerve and cerebellum, and degenerative changes in the bilateral inferior olivary nucleus. Mutational analyses of mitochondrial DNA identified the coexistence of heteroplasmic G11778A and homoplasmic T3394C mutations. CONCLUSIONS: These results suggest that the combination of G11778A and T3394C mutations leads to an atypical LHON phenotype.

6.Association between fruit and vegetable allergies and pollen-food allergy syndrome in Japanese children: a multicenter cross-sectional case series

BACKGROUND: Recently, the prevalence of food allergies during childhood is increasing, with fruits being common allergens. However, data on allergens that cause fruit and vegetable allergies and pollen-food allergy syndrome (PFAS) in childhood are relatively few. This study aimed to examine the allergens in fruit and vegetable allergies in pediatric patients and to determine the association between fruit and vegetable allergies and PFAS.OBJECTIVE: This study aimed to examine the current status of fruit and vegetable allergies in Japanese children.METHODS: This was a multicenter case series observational study. The participants included children aged <15 years who developed allergic symptoms after eating fruits and vegetables and subsequently received treatment in the Pediatric Department of 6 hospitals in the Osaka Prefecture in Japan during the study period from August 2016 to July 2017. Participants' information was obtained using a questionnaire, and data were obtained by performing several types of allergy tests using blood samples.RESULTS: A total of 97 children (median age, 9 years; 56 males) were included in the study. Apple was the most common allergen, followed by peach, kiwi, cantaloupe, and watermelon. A total of 74 participants (76%) exhibited allergic symptoms due to PFAS; moreover, pathogenesis-related protein-10 (PR-10) was the most common allergen superfamily. On the contrary, in the group where neither PR-10 nor profilin was sensitized, kiwi and banana were the most common allergens, and the age of onset was lower than that in the PFAS group. Specific antibody titer was significantly associated with Birch for Bet v1 and latex for Bet v2 (r = 0.99 and r = 0.89).CONCLUSION: When we examine patients with fruit and vegetable allergies, we should first consider PFAS even in childhood specifically for children greater than 4 years old.
