1.Effect of aerobic and anaerobic training on collagen fibers of tendons in rats.

The purpose of this study was to observe the effect of training on morphological properties in the collagen fibers of tendons. Wistar strain rats at 7 weeks old were assigned to high speed endurance (H), moderate speed endurance (M), jump training (J) and control (C) groups. The amount of exercise for H group (speed at 30m/min for 60 minutes) and M group (speed at 17m/min for 75 minutes) was equalized according to the amount of oxygen consumed during training. As for the anaerobic training (J group), rats made high jumping form the standing position, wearing jacket with 50% of body weight, 50 times/day, which lasted for 4-5 minutes. The training period was designed for 5 days per week for 16 weeks.
The follwing is a summary of the results obtained:
1) Increases in fiber areas of both Slow-Twitch (ST) and Fast-Twitch (FT) fibers were observed in the H, M and J groups. The ST fiber area of H group was significantly larger than that of C group. Significant increase in FT fiber area was observed in both M and J groups.
2) Hypertrophy of collagen fiber in gastrocnemius muscle tendon was observed in Hand M groups; especially, M group showed much larger increase than H group. It tended to be greater for J group as compared with C group.
3) Collagen fiber area and diameter of tendon in plantar muscle were slightly larger for H and M groups relative to C group, but these differences were not significant.
These results suggested that both types of aerobic training induced hypertrophy in the collagen fibers of the tendons and increased the tensile strength; especially, the group which trained longer hours induced the higher effect.
2.Effects of exercise under restricted food intake on protein metabolism in rats.

Effects of treadmill running and restricted food intake on the metabolism of protein in several tissues were studied in male Wistar rats, aged 5 weeks and weighing about 100 g. Rats were divided into control group under free food intake (C), sedentary group under restricted food intake (Sr), exercise group under restricted food intake (Er), Food intake was restricted approximately to 66% of fed ad libitum for control group. Exercise program consisted of treadmill running for 60 minutes a day at 30 m/min on 6 days a week for 12 weeks.
A. The effects of sedentary under restricted food intake.
When compared with C
1) Lowering of body weight and increment in mg tissue/g body weight ratios. (M. Gastrocnemius, Liver, Brain, Testis, M. Soleus, M. Plantaris)
2) Significantly low level of protein content in gastrocnemius muscles, liver and kidney.
3) Inhibition in incorporation of 14C-amino acid mixture into brain and kidney protein in vivo.
B. The effects of exercise under restricted food intake.

The effects were the same as those of Sr.

1) Lowering of body weight and increment in mg tissue/g body weight ratios. (Liver, Heart, Kidney, Testis)
2) Significantly low level of protein content in gastrocnemius muscles.
From this study it was suggested that, under the food restriction of long duration, the maintenance of protein content in brain in spite of the decrease of protein content in the other tissues indicated the existence of internal mechanism for life conservation in severe environment. And loaded with exercise under food restriction, protein content in gastra cnemius muscles decreased. Therefore, it was conceivable that skeletal muscles protein was consumed as energy for exercise.
3.PHYSIOLOGICAL STUDY OF RACE-WALKING

The purpose of this study was to clarify some of the characteristics of race-walking, especially the relationship between walking speed and oxygen requirement, and stride in race-walking and normal walking, and to examine whether race-walking is effective for the maintenance and promotion of health.
The subjects were five male race-walkers (race-walker group) and five male college students (control group) .
The results obtained were as follows:
1. Under race-walk conditions, the highest speeds attained in the race-walker and control groups were 200-220 m/min and 160 m/min, respectively. Under normal walking conditions, however, the values were 140 m/min in both groups.
2. A lower oxygen requirement was observed at slower speed during normal walking and at a higher speed (over 130 m/min) during race-walking.
3. Oxygen requirement (ml/kg/100 m) in the race-walker group was minimal at 60-80 m/min during race-walking and at 60 m/min during normal walking. Values in the control group were minimal at 60 m/min under both walking conditions.
4. The oxygen requirement in the race-walker group was less than that of the control group under both walking conditions.
5. Under normal walking conditions, as the speed increased, both step-length and step frequency gradually increased, until step-length reached a limit of 80 cm. Thereafter, walking was maintained only by an increase in step frequency. However, in the race-walkes group, the subjects were capable of increasing their step-length further, and maintaining a higher speed (up to 220 m/min) .
6. It was suggested that race-walking is one of the most efficient exercises for maintaining and improving health.

5.Neural Mechanisms of Discogenic Back Pain: How Does Nerve Growth Factor Play a Key Role?.

It was reported that nerve fibers were present in the inner part of lumbar intervertebral discs from patients with discogenic pain. Because there are no nerve fibers in the inner part of annulus fibrosus in normal condition, this finding suggests nerve ingrowth into the disc may be a cause of discogenic pain. Disc degeneration is often asymptomatic, thus, to understand the differences between symptomatic and asymptomatic disc, it is necessary to understand the pathogenesis of discogenic pain. We recently revealed that over 90% of the nociceptive dorsal root ganglion (DRG) neurons innervating the disc are sensitive to nerve growth factor (NGF), which is related to inflammatory pain. This indicates that discogenic pain is closely related to inflammation and NGF may play a key role. The increase of inflammatory mediators in symptomatic discs has been reported; we therefore studied the effects of disc inflammation and found that it induces sensitization of disc-innervating neurons and nerve ingrowth into the disc. More recently, it was shown that annular rupture induces nerve ingrowth, an increase of inflammatory mediators in the disc, and upregulation of calcitonin gene-related peptide, a pain-related molecule in DRGs. These findings led us to believe that annular rupture triggers inflammation and nerve ingrowth, inflammatory mediators then further promote nerve ingrowth into the disc and sensitization of disc-innervating neurons, and discogenic pain finally becomes chronic. NGF, found in symptomatic discs, may act as a key factor in generating chronic discogenic pain by sensitizing disc-innervating neurons and stimulating nerve ingrowth into the disc.

6.Incidence and Consequence of Falls among Stroke Rehabilitation Inpatients in Convalescent Rehabilitation Ward : Data Analysis of the Fall Situation in Multi-institutional Study

We collected and analyzed large-scale data concerning the fall of stroke inpatients in convalescent rehabilitation wards. Three hundred seventy-four of 1,107 inpatients experienced one fall or more, and 16 factors associated with falls were clarified by the chi-square test. To extract the significant item from a multifactor, the logistic regression analysis of 16 factors was carried out, and we developed an assessment sheet for the risk degree of first fall prediction in stroke inpatients. We selected eight variables as the items on the assessment sheet : history of previous falls, central paralysis, visual impairment, sensory disturbance, urinary incontinence, use of psychotropic medicines, mode of locomotion, and cognitive impairment. The total score of the assessment sheet was ranged from 0 to 10 and the mean score of fallers (6.4±1.5) among subjects was significantly higher than that of non-fallers (5.1±1.9) (p <0.001). When the subjects were classified into three groups, a significant difference (p<0.001) in the tendency of fall incidence in term of days after admission was found among the three groups on the basis of the Kaplan-Meier survival curve.
7.Suppressive Effects of Mesenchymal Stem Cells in Adipose Tissue on Allergic Contact Dermatitis.

BACKGROUND: Allergic contact dermatitis (ACD), which is accelerated by interferon (IFN)-γ and suppressed by interleukin (IL)-10 as regulators, is generally self-limited after removal of the contact allergen. Adipose tissue-derived multipotent mesenchymal stem cells (ASCs) potentially exert immunomodulatory effects. Considering that subcutaneous adipose tissue is located close to the site of ACD and includes mesenchymal stem cells (MSCs), the MSCs in adipose tissue could contribute to the self-limiting course of ACD. OBJECTIVE: The aims of the present study were to elucidate the effects of MSCs in adipose tissue on ACD and to examine any cytokine-mediated mechanisms involved. METHODS: Ear thickness in a C57BL/6 mouse model of ACD using contact hypersensitivity (CHS) elicited by 2,4,6-trinitro-1-chlorobenzene was evaluated as a marker of inflammation level. Five and nine mice were injected with ASCs and phosphate-buffered saline (PBS), respectively. After ASC or PBS injection, real-time reverse transcription-polymerase chain reaction and enzyme-linked immunosorbent assay were performed. RESULTS: Histology showed that CHS was self-limited and ear thickness was suppressed by ASCs in a dose-dependent manner. IFN-γ expression in the elicited skin site and regional lymph nodes was significantly lower in ASC-treated mice than in control mice. IL-10 expression did not differ between treated and control mice. The suppressive effects of ASCs on CHS response did not differ between IL-10 knock-out C57BL/6 mice and wild-type mice. CONCLUSION: The present findings suggest that MSCs in adipose tissue may contribute to the self-limiting course of ACD through decreased expression of IFN-γ, but not through increased expression of IL-10.

In the areas of home medical care and self-medication, the role of the pharmacist is growing, partly as a result of Japan’s aging society and the need to reduce medical costs. In response, the Kinki University Faculty of Pharmacy implemented a physical assessment practical training seminar in order to improve the physical assessment skills of practicing pharmacists. A series of questionnaires were conducted among pharmacists to investigate their perceptions of physical assessment practical training seminars. The results of the questionnaires were analyzed using Customer Satisfaction (CS) analysis and text mining. Based on a 5-point scale (1-low∼5-high), questionnaires revealed satisfaction for physical assessment practical training seminars was 4.6±0.6 (Ave.±S.D.). CS analysis revealed that the items “lectures” and “case seminars” had the highest level of satisfaction. However, items showing low levels of satisfaction were “auscultation of respiratory sounds” and “SBAR (Situation, Background, Assessment, Recommendation).” Results of text mining suggested a relationship between “physical assessment” and “difficult”. Analysis of the questionnaires showed a high level satisfaction with physical assessment practical training seminars, notably physical assessment practice methods. However, CS analysis and text mining indicate the finer techniques of physical assessment were difficult to acquire.
9.Assessment of the Initial Diagnostic Accuracy of a Fragility Fracture of the Sacrum: A Study of 56 Patients

Methods:
Fifty-six patients (13 males and 43 females) with an average age of 80.2±9.2 years admitted to the hospital for FFS between 2006 and 2021 were analyzed retrospectively. The following patient data were collected using medical records: pain regions, a history of trauma, initial diagnoses, and rates of fracture detection using radiography, computed tomography (CT), and magnetic resonance imaging (MRI).
Results:
Forty-one patients presented with low back and/or buttock pain, nine presented with groin pain, and 17 presented with thigh or leg pain. There was no history of trauma in 18 patients (32%). At the initial visit, 27 patients (48%) were diagnosed with sacral or pelvic fragility fractures. In contrast, 29 patients (52%) were initially misdiagnosed with lumbar spine disease (23 patients), hip joint diseases (three patients), and buttock bruises (three patients). Fracture detection rates for FFS were 2% using radiography, 71% using CT, and 93% using MRI. FFS was diagnosed definitively using an MRI with a coronal short tau inversion recovery (STIR) sequence.
Conclusions
Some patients with FFS have leg pain with no history of trauma and are initially misdiagnosed as having lumbar spine disease, hip joint disease, or simple bruises. When these clinical symptoms are reported, we recommend considering FFS as one of the differential diagnoses and performing lumbar or pelvic MRIs, particularly coronal STIR images, to rule out FFS.
10.Development of an assessment sheet for fall prediction in stroke inpatients in convalescent rehabilitation wards in Japan.

OBJECTIVEWe conducted a study to develop an assessment sheet for fall prediction in stroke inpatients that is handy and reliable to help ward staff to devise a fall prevention strategy for each inpatient immediately upon admission.
METHODSThe study consisted of three steps: (1) developing a data sampling form to record variables related to risk of falls in stroke inpatients and conducting a follow-up survey for stroke inpatients from their admission to discharge by using the form; (2) carrying out analyses of characteristics of the present subjects and selecting variables showing a high hazard ratio (HR) for falls using the Cox regression analysis; (3) developing an assessment sheet for fall prediction involving variables giving the integral coefficient for each variable in accordance with the HR determined in the second step.
RESULTS AND DISCUSSION(1) Subjects of the present survey were 704 inpatients from 17 hospitals including 270 fallers. (2) We selected seven variables as predictors of the first fall: central paralysis, history of previous falls, use of psychotropic medicines, visual impairment, urinary incontinence, mode of locomotion and cognitive impairment. (3) We made 960 trial models in combination with possible coefficients for each variable, and among them we finally selected the most suitable model giving coefficient number 1 to each variable except mode of locomotion, which was given 1 or 2. The area under the ROC curve of the selected model was 0.73, and sensitivity and specificity were 0.70 and 0.69, respectively (4/5 at the cut-off point). Scores calculated from the assessment sheets of the present subjects by adding coefficients of each variable showed normal distribution and a significantly higher mean score in fallers (4.94 +/- 1.29) than in non-fallers (3.65 +/- 1.58) (P = 0.001). The value of the Barthel Index as the index of ADL of each subject was indicated to be in proportion to the assessment score of each subject.
CONCLUSIONWe developed an assessment sheet for fall prediction in stroke inpatients that was shown to be available and valid to screen inpatients with risk of falls immediately upon admission.
