2.A Case of Bilateral Central Retinal Vein Occlusion.

Retinal vein occlusion(RVO)is the second most common form of retinal vascular disease next to diabetic retinopathy. Systemic risk factors such as hypertension, diabetes, cardiovascular disease, blood dyscrasia and ocular risk factors such as short axial length, glaucoma, papilledema, thyroid ophthalmopathy have been shown to be associated with RVO. It is usually unilateral disease, and bilateral occurrene of RVO is relatively uncommon. CRVO rarely occurs bilaterally with a short interval between two eyes. Having experienced a case of bilateral CRVO within one month in a 75-year-old female with short axial length, primary open angle glaucoma, hypertension and hypercholesterolemia, we report this case with literature review.

3.Correction of Epiblepharon by Excision of Redundant skin and Pretarsal orbicularis muscle without tarsal fixation.

PURPOSE: To evaluate the effect of the excision of redundant skin and pretarsal orbicularis muscle without tarsal fixation on the correction of the congenital epiblepharon. METHODS: We performed the surgery on 115 patients, 220 eyes from January 1996 to December 1999, and evaluated the functional, and cosmetic result. Surgical treatment included excision of redundant skin and pretarsal orbicularis muscle without tarsal fixation and simple skin suture technique. RESULTS: The mean age of the group was 7.2+/-6.8 years. There were 45 males and 70 females. Following the average follow up period of 18.4+/-4.5 months (minimum was 12 months), 212 eyes (96.4%) showed satisfactory result in functional and cosmetic aspect with of 8 eyes (3.6%) showed undercorrection early postoperativey but none underwent reoperation due to recurrences. CONCLUSIONS: This operative technique resulted in a good cosmetic and functional result.

6.Efficacy of Endoscopic Variceal Ligation on Bleeding Esophageal Varices.

Endoscopic injection sclerotheraphy(EIS) was an effective method for treatment of bleeding esophageal varices. However, it might be associated with some undesirable complications. Endoscopic variceal ligation(EVL) is a recently developed method for control of active variceal bleeding and eradicating esophageal varices with similar efficacy and minimal risk of complications compare to EIS. We performed EVL in 40 patients who had recently bled from esophageal varices. Total 198 variceal ligations were performed during 64 separate EVL session. Control rate of acute bleeding was 90%(36 of 40patients) and 4 patients died after EVL because of failure of bleeding control. Rebleeding following initiation of EVL occured in 5 patients-three patients were successfully controlled by EVL, one patient was controlled but eradication was impossible and one patient died. Varices were eradicated or reduced to grade I in 31(86.1%) of 36 survivors by 1-9 ligation(mean 5.0) in l-4 EVL sessions(mean 1.6). After EVL, there were mild complications-mild substernal discomfort in 4 patients, mild dysphagia in 2 patients and fever in 2 patients. These results suggest that EVL is a safe and effective method for treatment of bleeding and eradication of esophageal varices with less complication.

9.Endoscopic Submucosal Dissection of an Inverted Early Gastric Cancer-Forming False Gastric Diverticulum.

Endoscopic submucosal dissection (ESD) is a standard treatment for early gastric cancer (EGC) that does not have any risk of lymph node or distant metastases. Here, we report a case of EGC resembling a diverticulum. Diverticular formation makes it difficult for endoscopists to determine the depth of invasion and to subsequently perform ESD. Because the false diverticulum does not have a muscular layer, this lesion can be treated with ESD. Our case was successfully treated with ESD. After ESD, the EGC was confined to the submucosal layer without vertical and lateral margin involvement. This is the first case in which ESD was successfully performed for a case of EGC that coexisted with a false gastric diverticulum. An additional, larger study is needed to determine the efficacy of ESD in various types of EGC, such as a false gastric diverticulum.

10.Effect of Iron - Uptake Mechanisms of Staphylococcus Aureus on the Growth in Human Body Fluids.

BACKGROUND: We could establish a streptonigrin-resistant strain called SR-1 strain from Staphylococcus aureus ATCC 6538 as a parental strain and characterize SR-1 strain as defective in the iron-uptake mechanisms including production of siderophores and expression of transferrin-binding protein on the cell wall. We performed this study to elucidate effect of the iron-uptake mechanisms on the growth in human body fluids. METHODS: Growth kinetics of SR-1 strain were compared with those of the parental strain and the increase of unsaturated iron-binding capacity (UIBC) was measured. Siderophore production and expression of transferrin-binding protein were detected by CAS diffusion assay and ligand-blot method probed with human transferrin conjugated horseradish peroxidase, respectively, as the strains were cultivated in normal pooled sera, ascitic fluid and pleural effusion. RESULTS: Siderophores activity in the body fluids could not be detected by the CAS diffusion assay. The parental strain expressed the transferrin-binding protein on the cell wall during the growth in ascites and pleural effusion except the sera whereas SR-1 strain did not. Growth kinetics showed that SR-1 strain grew sluggish compared to the parental strain. The peak of increase of UIBC of the parental strain was observed at the mid-exponential growth phase and the increase of UIBC of SR-1 strain was either lower than that of the parental strain or not changed. CONCLUSION: The iron-uptake mechanisms of S. aureus, especially expression of transferrin-binding protein, play a significant role in growing in the body fluids.
