1.A Case of Erythema Elevatum Diutinum.

A case oof erythema elevatum diutinum in 30-year old male was described. Reviewing literatures, this disease is very rare and is the first case to be reported in Korea. There were persistant elevated hard brownish red nodulo-infiltrated plaques which were symmetrically distributed especia]ly over the shins, elbows, knees, ear lobes, buttocks, and dorsal surfaces of the hands, feet, fingers, and toes, accompanied by occasional mild itching. These skin eruption were started from the erythematous papulo-vesicular macules which developed on the left shin and wrist about 7 years previously. He has also had a long-standing encapsulated pleural effusion with marked pleural thickening in left lower chest which was diagnosed 3 years ago. The serum IgG was noted to be increased. Histological examination of skin lesions revea]ed leucocytoclastic angiitis and sevrer perivascular fibrosis in older lesions.

2.bcl-2 and MIB-1 Expression in Basal Cell Carcinoma.

BACKGROUND: bcl-2 is a newly characterized proto-oncogen that has been shown to suppress programmed cell death(apoptosis), which is involved in tumorigenesis, and its expression has been demonstrated within tumor cells in a variety of neoplasms. OBJECTIVE: We examined whether there are differences in bcl-2 expression in basal cell carcinoma and several epidermal keratinocytic tumors. In addition, we evaluated bcl-2 expression according to histological types of basal cell carcinoma as well as the interaction of expression of bcl-2 and MIB-1 protein in BCC subtypes. METHODS: Routine paraffin sections of formalin-fixed sixty tissues(20 BCC, 10 Bowen disease, 10 keratoacanthoma, and 20 SCC) were labelled with anti-bcl-2 monoclonal antibody and MIB-1 using a labelled streptavidin-biotin complex. RESULTS: 1. Twenty cases of basal cell carcinoma were examined and all expressed cytoplasmic bcl-2. Three out of 20 cases of squamous cell carcinoma were focally positive. None of the 10 Bowen disease cases and the 10 keratoacanthoma cases expressed bcl-2. 2. In BCC, bcl-2 immunoreactivity was great in the superficical subtypes which had an indolent growth variant, moderate in the circumscribed types, and weak in the infiltrative types which had aggressive growth variants. 3. Evaluation of the distribution of bcl-2 immunoreactive categories and MIB-1 grades revealed a negative correlation tendency, but no statistical significance. CONCLUSION: Our results suggest the presence of a protection mechanism from apoptosis mediated by bcl-2 protein involved in the neoplastic growth mechanism of BCC. In addition, the observed findings in the expression pattern of bcl-2 and MIB-1 in the BCC subtypes may be due to interaction between bcl-2 and other apoptotic-related oncogens.

6.Animal Experiments of Cutaneous Depigmentation by 4-Isoprophlcatechol and Hydroquinone.

The depigmenting activity of 4-isopropylcatechol and hydroquinone, and the behaviour of salicylic acid affecting to the activity were studied. To the hydrophilic ointment base, 4-isopropylcatechol and hydroquinone were incorperated respectively in concentrations of l% and 3%, and to these creams, salicylic acid was also added in concentration of 3% and 5% respectively. These creams were applied topically to the epilated skins of the back of black guinea pigs once a day for 3 weeks. Followings were the results: 1) The depigmenting activity was found to be more potent in 4-isopropylcatechol than in hydroquinone. 2) The depigmenting activity of hydroquinone was found to be remarkably accelerated by the addition of salicylic acid. 3) The skins of guinea pigs were found to be mildly irritated by the application of both 4-isopropylcatechol and hydroquinone.

7.Pedal Indirect Lymphangiography.

PURPOSE: Recently, indirect lymphangiography has been developed as a relatively good and noninvasive imaging modality of the lymphatic system at extremities. But the disadvantage of the indirect lymphangiography is a low contrast ratio between the surrounding tissues and the contrast media in lymphatic vessels, because dimeric nonionic contrast media is water soluble and diluted in the proximal leg lymphatic vessels. We could have relatively better image than previously published images for the leg lymphatic system, when we injected contrast media with adequate high pressure in intradermal space of the interdigital areas at the foot dotsum. So, we would like to report the results. MATERIALS AND METHODS: We could study all 9 lymphedemas(primary :6, secondary: 3) from April 1990 to May 1993 on outpatient base. They were diagnosed as lymphedema clinically and radiologically. Ten ml of dimeric nonionic aget, iotrolan(Isovist 300 ) was injected into intradermal space with five 30-gauge needles The injection speed was more than 0.2mi/min. We have done one side pedal lymphangiogram in 30 minutes. The evaluation of the anterior superficial lymphatics was accordig to the criteria of the Weissleder(2). RESULTS: The results were as follows:1. All lymphatic vessels from foot to inguinal area could be visualized. 2. Two or three inferior inguinal lymph nodes could be visualized about 42%. 3. The most common abnormal finding of the lymphedma was the neovascularization of the lymphatics on indirect pedal lymphangiogram. CONCLUSION: If we use adequate technique relatively high pressure injection, correct intradermal needle insertion, adequate soft tissue exposure technique indirect lymphangiography is considered to be a safe and noninvasive imaging modality for the evaluation of the lymphedema of lower extremity lymphatics including inferior inguinal lymph nodes.

8.The Treatment of Infected Nonunited Fractures of Long Bone

Many difficult problems are encountered in the treatrnent of infected nonunion of long bone. To solve these difficult fractures, authors adopted an active approach-thorough debridement of the infected and necrotic soft tissue and bone, rigid internal fixation with autogenous cancellous bone graft and opening of the wound to ensure continuous drainage until union of the fracture. Authors treated 28 cases of infected nonunions with this method from Mar. 1980 to Feb. 1989, and the results are as follows ;1. Femur(54%) and tibia(43%) were most frequently involved sites. 2. Most of the cases(86%) were treated by rigid internal fixation and cancellous bone graft. 3. In most cases(64%), infection was controlled spontaneously by the time of bony union, but in 5 cases which showed persistent drainage, implant removal controlled the problem, and residual soft tissue and bony defect were solved by Papineau's technique or musculocutaneous flap. 4. Revision was required in 5 cases(18%) because of insercure fixation. 5. Union could be obtained in average 4.8 months, and infection could be controlled in all except 1 case.

9.A Comparative Clinical analysis of Arthroscopic Meniscectomy and Arthroscopic Meniscus Repair

There have been controversies on the rationale, surgicl indication, technique and results of meniscus repair.. Authors comparatively studied 100 cases of meniscus injury in 88 patients (arthroscopic total menisectomy: 16 cases, arthroscopic partial menisectomy: 68 cases, arthroscopic meniscus suturing: 16 cases) from February 1985 to March 1992. The results were as follows: 1. Male (61.4%) was much more affected than female. 2. The third decade group was most commonly affected and the most common causes was sports injury (43.2%) 3. The lateral meniscus was more injured (71%), than the medial (29%) 4. In arthroscopic finding, longitudinal tear (37%) was the most common. 5. According to the Tapper and Hoover's criteria, satisfactory result was 56.3% in arthroscopic meniscus repair group.
