1.Study on Repairing Method of Vaginal Cuff in Total Laparoscopic Hysterectomy.

OBJECTIVE: Hysterectomy is one of the most common gynecological operations. The objective of this study is to compare the efficacy of suturing methods of vaginal cuff between laparoscopic and vaginal approach. METHODS: One hundred and sixty one cases of laparoscopic hysterectomy were devided into two groups. In group A(n=67), vaginal cuff was sutured by laparoscopic suture technique and in group B(n=94), vaginal cuff was repaired by vaginal approach. RESULTS: There were no significant differences in uterine weight, hospital stay, pre-and postoperative hemoglobin level and complications including febrile morbidity in two groups. Operation time was significantly shorter in group B(p=0.05). CONCLUSION: Transvaginal repair of vaginal cuff showed shorter duration of operation time than laparoscopic suture technique in total laparoscopichy sterectomy.

3.Presence of E-cadherin in Cultured Amnionic Cells.

OBJECTIVES: Cell adhesion molecules may play a role in integrating amnionic membrane. The objective of this study was to prove E-cadherin mRNA and proteins in cultured human amnionic cells. METHODS: We cultured amnionic cells from 4 women undergoing cesarean section without labor. E-cadherin was determined by immunohistochemistry and Western ligend blotting. To demonstrate E-cadherin mRNA, reverse transcription-polymerase chain reaction was performed. RESULTS: On immunohistochemistry, E-cadherin was abundantly showed on the cytoplasm of the cells. Western ligend blotting showed clear 120 kDa bands on four specimens, and relatively weak band on one specimen. Reverse transcription-polymerase chain reaction showed 432 BP bands. CONCLUSION: We proved E-cadherin and its mRNA by immunohistochemistry, Western ligend blotting and reverse transcription-polymerase chain reaction in cultured human amnionic cells.

4.A Case of Sjogren-Larsson Syndrome.

Sjogren-Larsson Syndrome is a rare disorder chracterized by congenital icthyosis, spastic diplegia or tetraplegia and mental retardation. The inheritance is autosomal recessive, due to deficiency of alcohol dehydrogenase activity of fatty acid matabolism. We have experienced a case of Sjogren-Larsson Syndrome in a 16 month-old male children who have dried thick skin, developmental delay, and spastic diplegia. He also has glistening spot on macula area with irregular high voltage slow wave (2-3Hz) on electroencephalography. From Biopsy finding, Lamella ictyosis with acanthosis and hyperkeratosis in epithelium is shown. He was much improved clinically for skin lesion and developmental status with etretinate supplement. We report the case with brief review of related literatures.

5.Electron Microscopic Study of Cervical Carcinoma Tumor Cells after Chemotherapy.

Our aim was to identify histologic changes in cervical carcinoma tumor cells due to chemotherapy,by electron microscopic examination. Cisplatin and 5-fluorouracil were used between March 1, 1994 and February 28, 1995 on a total of 16 patients. The treatment schedule consisted of 75 mg/m2 cisplatin via iv with hyperhydration on Day 1, in combination with 1000 mg/m2 5-fluorouracil via continuous iv on Day 1 to 5. The treatment was repeated every 4 weeks with a maxinum of 3 courses. Ultrastructurally, chemotherapy induced apoptosis,indicated by condensation of nucleus and cytoplasm,fragmentation of nuclei, and apoptotic bodies. Due to repeat chemotherapy, increased levels of desmosomes were noted and parts of tumor nests were replaced by mature squamous cells. Tumor nests were reduced in size, mitochondria exhibited swelling, the endoplasmic reticulum was dilated, and infiltration of inflammatory cells increased after chemotherapy. These results suggest that chemotherapy induces apoptosis and injury to the cytoplasm and nuclei of tumor cells. However, the exact selection mechanism of the biochemical pathway of cell death is not fuUy understood. Further study of the molecular biologic actions of cell death after chemotherapy is warranted.

6.A Case of Congenital Hyperextension of the Knee.

Congenital dislocation of the knee is a very rare condition and was first described by Chatelaine in 1822. The etiology of this condition is unknown. It is generally subclassified as simple hyperextension, subluxation, and dislocation, depending on the degree of the joint displacement and the severity of disease. There are a large of associated conditions that have been described, the most common of which are congenital dislocation of the hip, club foot, arthrogryposis, and Larsen's syndrome. The mainstay of treatment is early serial rnanipulation and splinting. Operative treatment was indicated whenever conservative treatment did not lead to satisfactory reduction. The authors experienced a case of congenital hyperextension of left knee associated with the calcaneovalgus deformity of both foot. Early closed treatment obtanied a successful reduction and satisfactory knee motion.

7.Familial Lichen Planus.

Familial lichen planus is an uncommon disease, and there are 104 cases reported in the English literature. Several theories regarding the etiopathogenesis of lichen planus have been proposed. However, immunological mechanisms and genetic susceptibility have been emphasized the most. Several reports showed HLA analysis in lichen planus patients, and recently 2 cases have revealed an increased frequency of DR1 antigen in patients with lichen planus compared to a control group. We observed two brothers with typical skin lesions of lichen planus of almost simultaneous onset. We confirmed lichen planus by skin biopsies in both brothers and analyzed HLA typing. We also observed the occurrence of DR1 antigen in both of them.

9.Urologic Complications in 125 Renal Transplantations and Comparison between Intravesical and Extravesical Uretero Neocystostomy.

We evaluated the complications of intravesical and extravesical ureteroneocystostomy in 125 renal transplantations performed in the Department of Urology, Catholic Medical College between March 1969 and July 1982. The following results were obtained: 1. Of 125 cases of the recipient 105 cases were male and 20 cases were female. 2. Intravesical ureteroneocystostomy was performed by Politano-Leadbetter method in 66 cases and extravesical ureteroneocystostomy by the similar ones reported by MacKinnon in 59 cases. 3. Our incidence of urological complications in the 125 renal transplantations was 9.7% (12 cases). 4. Postoperative complications developed in 11 of the 66 who underwent intravesical ureteroneocystostomy and in 1 of the 59 who underwent extravesical ureteroneocystostomy. 5. The details of the complications were as follows: 6 were ureteral obstruction, 4 ureteral leakage, 1 bleeding from anastomosis site and 1 was urine leakage from anastomosis site. 6. Operative methods for urological complications were ureteroureterostomy in ureteral obstruction 161 and ureteral leakage (3), ureteroneocystostomy in ureteral leakage (1), simple suture in bleeding (1) and urine leakage (1) from anastomosis site. 7. This method of extravesical ureteroneocystostomy in renal transplantation was easy and fast and has a low complication.

10.Comparative Analysis in Methods of Operation of Urethral Stricture and Long Term Follow-up.

Urethral stricture is one of the most serious disease in Urology and must be treated vigorously, but the best method of treatment is not established. Our aim is to evaluate a policy of treatment which had developed in favor of urethroplasty for all but minor stricture. A total of 123 patients who had suffered from urethral stricture and undergone urethroplasty in the Department of Urology. Catholic Medical College, were studied during the period from January, 1970 to December, 1982. Of 123 patients,112 primary operations were interlocking, resection and end-to-end anastomosis, Turner-Warwick urethroplasty and transpubic urethroplasty. The results were as follows: 1. In age distribution of the 123 cases of urethral stricture, age of 80 cases ranged from 20 to 49 years. 2. The most common cause of urethral stricture was traumatic urethral injury(traffic accident, industrial accident and staddle injury)comprising 117(95.1%)our of 123cases and the remainders were inflammatory in 3(2.4%),iatrogenic in 3(2.4%). 3. In 41 cases(36.6%)repair was done by interlocking,57(50.9%)by resection and end-to-end anastomosis,3 (2.7%)by Turner-warwick urethroplasty,1(0.9%)by transpubic urethroplasty, 10(8.9%)by suprapubic cystostomy alone. 4. Of the 41 cases of interlocking,6(14.6%) were developed stricture and they required a secondary operation due to persistent stricture in spite of urethral dilatation. 5. Of the 57 cases of resection and end-to-end anastomosis, 5(8.8%)were developed stricture and they required a secondary operation. 6. The methods of secondary operation were resection and end-to-end anastomosis in 6, Turner-Warwick urethroplasty in 2, transpubic urethroplasty in 2.9 of the 10 had good results but 1 required a tertiary operation. 7. In 11 cases transferred from other hospital, methods of secondary operation were resection and end-to- end anastomosis in 8,Turner-Warwick urethroplasty in 2, transpubic urethroplasty in 1 but 3 of the 11 cases required a tertiary operation(Turner-Warwick urethroplasty.) 8. Impotence was developed in 17(13.8%) and the incidence of impotence associated with pelvic bone fracture was 11 cases(64.7%) In total 123 urethral stricture patients,55cases have been followed up by interview and questionnaire from 2 to 10 years. 68 were lost to follow-up. 2 of the 55 vases died of unrelated disease within the period of follow-up. In long term follow-up survey of 53 cases, 7(13.2%) were complicated of urethral stricture and impotence in 9 (17.0%).
