3.The Role of Computerized Tomography in the Diagnosis of Bladder Rapture.

BACKGROUND: Computerized topography(CT) is the method of choice for establishing patients with abdominal and/or pelvic trauma. However, the sensitivity of CT far detecting bladder rupture has been questioned. We investigated the roles of CT as the initial evaluation of abdominal and pelvic trauma in diagnosis of bladder rupture. METHODS: We reviewed the medical records and radiographs of 53 patients with bladder rupture for last 5 year. And among them, all radiographs of 30 patients who underwent both CT and retrograde cystography were evaluated independently by two urologists who had no knowledge of the patients' final diagnosis. RESULTS: Among 30 patients, all of patients were correctly diagnosed by retrograde cystography, but the CT diagnosis was correct in only 23 patients(76.7%), who were 17 patients(85.0%) with intraperitoneal rupture and 6 patients(60%) with extraperitoneal rupture. And of the 7 patients who were negative by the CT, all showed the sign of inadequate bladder distension. CONCLUSION: We suggest that CT, if properly performed with adequate bladder filling, is as sensitive for detection of bladder injuries as conventional cystography. Especially, in trauma patients with hematuria and suspected other organ injury, CT-cystography with retrograde filing may be as accurate as conventional cystography and obviate the need for an additional plain film cystography,

5.Percutaneous transluminal angioplasty in renal artery stenosis by Takayasu's arteritis

Authors want to report that PTA was an out-standing therapeutic method in treatment of renovascular hypertension by renal artery stenosis due to Takaysu's arteritis in an 18-years-old Korean female. Abdominal aortogram and bilateral renal arteriogram showed mild smooth narrowing of the mid abdominal aorta of 4cm length from origin of renal arteries, complete occlusion of left renal artery from it's origin with a few small collateral arteries and severe stenosis of proximal right renal artery. PTA of right renal artery was performed with Gruntzign Balloon dialtation catheter, and the patient's blood pressure was down to 120/80 from 220/130,stable for 8 months follow-up examination. Takayasu's arteritis is one of the msot important cause of renovascular hypertension in Orenals. Multiple treatement modalities, such as steroid administration or variable surgical intervention, were attempted and limited response was presented. But PTA is a simple, cost-effective, repeated,and relatively complicaiton-free method in treatment of renovascular hypertension due ot Takayasu's arteritis, with an outstanding result, as authors case.

6.Clinical Evaluation of Patients with Unilateral Non-functioning Kidney on 99mTc-DMSA scan.

The authors report 16 cases of mediastinal fine-needle aspiration cytology from Jan. 1985 to Mar. 1988 at the Seoul National University Hospital. Among them, diagnostic material were obtained in fifteen cases, establishing the dia- gnosis of 7 thymomas, 2 germinomas, 2 neurogenic tumosr, 1 lymphoma, and 3 meastatic carcinomas. The 9 cytologic diagnoses could be confirmed by histologic examination in 8 patients and by another cytologic method in one patient, allowing concordance rate of 77%.

8.Familial Lichen Planus.

Familial lichen planus is an uncommon disease, and there are 104 cases reported in the English literature. Several theories regarding the etiopathogenesis of lichen planus have been proposed. However, immunological mechanisms and genetic susceptibility have been emphasized the most. Several reports showed HLA analysis in lichen planus patients, and recently 2 cases have revealed an increased frequency of DR1 antigen in patients with lichen planus compared to a control group. We observed two brothers with typical skin lesions of lichen planus of almost simultaneous onset. We confirmed lichen planus by skin biopsies in both brothers and analyzed HLA typing. We also observed the occurrence of DR1 antigen in both of them.

9.Some Problems in Surgery of the Posterior Fossa Lesions.

The management of the patient undergoing posterior fossa surgery meets with various difficulties during and after the surgery due to the anatomical characteristics of the posterior fossa. Sitting position has been used popularly for its advantage of proper exposure of the posterior fossa lesions, but this position presents disastrous complications such as hypotension and air embolism not infrequently. Hydrocephalus, frequently complicating the posterior fossa lesions, also presents many problems during and after surgery. Postoperative brain swelling, CSF leakage and infection are other important problems of the posterior fossa surgery. The authors obtained following results with analysis of surgical results in 71 posterior fossa lesions. 1) The posterior fossa lesions are frequently present hydrocephalus and signs of increased intracranial pressure due to blockade of CSF flow. Their proper management such as steroid, diuretics and preoperative shunt made the operative manipulation easier and improved the postoperative course. 2) The major intraperative problems were air embolism and postural hypotension which were mainly related to th4 sitting position. To avoid these complications, the author propose lateral position for very old patients and those with cardiopulmonary disease or in surgery for CP angle lesion and cerebellar hemispheric lesion which can be exposed adequately with this position. 3) It way easy to detect and manage possible postoperative problems such as brain swelling, hematoma and persistent hydrocephalus with aid of CT scan and their serious outcome could be reduced.
