2.A Case of Suprasellar Germinoma Metastasized to the Epidural Space of the Lumar Spinal Canal: Report of a Case.

We have experienced a case of suprasellar germinoma metastasized to the lumbar spinal canal. A 22-year-old man was admitted to St. Mary's Hospital May, 1975 with complaints of low back pain, progressive weakness of both lower extremities and voiding difficulty for about one month. About 7 months ago he received craniotomy and irradiation therapy for suprasellar germinoma. Neurological examination revealed foot drop on the left side with sensory change and atonic neurogenic bladder. Pantopaque myelogram showed a total block at the level of the 3 rd lumbar vertebra suggestive an epidural mass compressing dural sac posteriolaterally. Total laminectomy of lumbar first to fifth vertebrae disclosed a firm, rubbery hard and vascular mass in the epidural space, which was not adherent to the dura. Histological examination of the tumor revealed typical germinoma. Pathogenesis of geminoma and possible mode of epidural metastasis are discussed.

3.A Clinical Survey of Cesarean Hysterectomy.

Cesarean hysterectomy remains a necessary procedure for life saving during intractable obstetric hemorrhage. The procedure itself is usually associated with considerable perioperative morbidity, especially when performed under emergency conditions. In this study, the outcomes of 15 cases of cesarean hysterectomy performed at Catholic university of Taegu-Hyosung hospital for 5 years from January, 1993 to December, 1997 were reviewed. There were 22,251 deliveries during this period, cesarean hysterectomy was performed in 13 of 5,753 cesarean section(0.22%) and in 2 of 16,698 vaginal deliveries(0.12%), so more frequently after cesarean section than vaginal deliveries. The age of patients varied from 23 to 39 years old. The higher the age of patients, the higher incidence of cesarean hysterectomy was noted. The most common indication of emergency cesarean hysterectomy was in uterine atony(53.3%), followed by placental disorders(33.3%), uterine rupture(6.7%), and uterine myoma with pregnancy (6.7%). All patients who had cesarean hysterectomy received transfusion from 3 pints to 13 pints. In aspect of fetal outcomes, live birth were 12 cases(80%), one case was fetal death in utero(6.6%), 2 infants were dead during the early neonatal period(13.3%) due to prematurity and RDS (respiratory distress syndrome), so the perinatal mortality rate was 20.0%. The postoperative complications were wound disruption, hematoma, febrile morbidity and disseminated intravascular coagulopathy.

5.Arrhythmogenic Right Ventricular Dysplasia/Cardiomyopathy: Report of an autopsy case.

A 35-year-old man was admitted with a 20 day history of generalized edema and muscular weakness of the lower extremities. He was alert with a pale puffy face and an ejection murmur was heard at the cardiac apex. The electrocardiogram disclosed low voltage, first degree atrioventricular block, and a right bundle branch block. During the hospitalization an intractable diastolic hypotension developed, which measured 0 mmHg at the lowest point. At that time the echocardiogram revealed a dilated, akinetic right ventricle. Eventually a multiorgan failure developed and an autopsy following his death presented a fibrofatty replacement of the right ventricular myocardium. This might be a case of an arrhythmogenic right ventricular dysplasia/cardiomyopathy, which is usually characterized clinically by a ventricular tachycardia and may cause a sudden death in young adults.

6.Gas-Forming Brain Abscess due to Peptostreptococcus.

A gas forming brain abscess that was resistant to treatment with antibiotics, blycerol, and steroids is reported. A CT scan showed a frontal epidural gas shadow which progressed in 3 weeks to massive frontal cerebritis with gas within the abscess, ventricle, and basal cisterns. An anaerobic culture of CSF revealed peptostreptococus, which was been increasing recently as a causative organism in brain abscess. We feel that the primary management of brain abscess should be surgical excision followed by appropriate antibiotic therapy.

9.The Effect of Methimazole on the Thyroglobulin Synthesis in Cultured Porcine Thyroid Cells

The thioureylene drugs, propylthiouracil and methylmercaptoimidazol(MMI), exert their antithyroid effect primarily through inhibition of thyroid peroxidase-catalyzed iodination of thyroglobulin. Recently the interest about the effect to the thyroglobulin synthesis of these drugs have been increasing. So we studied the MMI effect to the thyroglobulin synthesis in cultured porcine thyroid cells. Porcine thyroid cells were isolated by sequential trypsinization in the presence of EGTA, seeded at high density(1X10^6 cells/cm^2) and cultured. One week later, MMI was added in different concentrations(0, 0.2, 1, 5mM) with TSH only or with 4H(b-TSH, Insulin, Transferrin, Hydrocortisone) or without hormone. Medias were collected after 24 hours and compared the amount of thyroglobulin secreted. And also pulse-labeling were performed with S^35 cysteine/methionine(1-2uCi/well) for 30, 60, 90min at the same conditions.There was no significant change in the amount of the secreted thyroglobulin by MMI, and there was no significant change in the pulse-labeled interacellular thyroglobulin by MMI. And also there was no significant change in the secretion of TSH-stimulated thyroglobulin by MMI. So we conclude that MMI has no effect on the thyroglobulin synthesis in cultured porcine thyroid cells and also MMI has no effect on the TSH-stimulated thyroglobulin synthesis in cultured porcine thyroid cell.

10.The Effect of Iodine on the thyroglobulin Synthesis in Cultured Porcine Thyroid Cells

The amount of thyroglobulin synthesized from thyroid cells and stored in colloid space in very important in thyroid hormone synthesis. The thyroglobulin synthesis is mainly regulated by TSH secreted from the pituitary gland. But recently there were some reports about the possibility that iodine regulated the thyroid protein synthesis. So our studied were conducted to determined whether iodine could have inhibitory effect on thyroglobulin synthesis and methimazole could abolish the inhibitory effect of idoine.Porcine thyroid cells were isolated by sequential trypsinization in the presence of EGTA, seeded at high density(1X10^6 cells/cm^2) and cultured. One week later, Nal was added in different concentrations(10^-7, 10^-6, 10^-5, 10^-4M). 24hour medias were collected and checked the amount of thyroglobulin secreted. And also pulse-labeling were performed with[^35S] cysteine/methionine(1-2 uCi/well) for 1 hour at the same conditions. We used 3mM methimazole and 10^-4M NaI to observe the blocking effect of methimazol in iodine.The extracellular thyroglobulin secretion was significantly decreased by iodine in dose dependent manner(82.4%, 80.7%, 76.8% and 73.1% of control). And also intracellular thyroglobulin synthesis was significantly decreased by iodide in dose dependent manner(100.5%, 83.4%, 82.3% and 79.4% of control). The inhibitory effect of iodide was abolished by methimazole(74.7% to 101.3% of control). These data indicate that high iodide inhibit the thyroglobulin synthesis and secretion from the thyroid cells, and furthermore autoregulation by iodide may include thyroglobulin synthesis. And also this effect is dependent on the generation of an organic form of iodine because methimazole abolish the inhibitory effect of iodide.
