3.Changes of the Renal Arteries Accordding to Various Embolic Materials.

The transarterial embolization has been widely used to control bleeding. It has a variety of clinical utility; to reduce bleeding on the surgical field, to reduce the size of malignant tumor as a preopearative treatment, to treat arteriovenous malformation or arterial aneurysm as a curative method and to promote life qua' ity of patient with diffuse or multiple hepatocellular carcinoma as a palliative treatment, etc. With the advance of modem technology, various embolic materials have been also developed. IIowever, it has not- been -fully investigated of histopathologic changes of the embolized organs according to the embolic materials used. This study was undertaken to investigate the histopathologic changes of embolized renal artery in rabbit by various embolic materials, according to each embolic material and to time passed by after embolization. Of the 5 arteries embolized by ethylene vinyl alcohol copolymer(EVAL), one showed abscess formation in embolized kidney. The other 4 allowed to perform further pathologic study: within a week after embolization there was no any specific change in vessels, however, r,unimal endothelial hypertrophy was observed following 2 weeks of embolization. Of the 8 renal arteries embolized by N-buthyl-2-cyanoacrylate(Histoacryl), 4 showed total occlusion of the main renal arteries as well as renal infarction, which reflects the strong adhesiveness of Histoacryl to vascular wall. 4.Prognostic Significance of Heat Shock Protein 70 Expression in Bladder Tumor.

Heat Shock Protein (HSP) is a genetic product reacting on stress. HSP is increased by physiological or environmental stress and expressed at gastrointestinal tumors such as stomach cancer, pancreatic cancer and large intestinal cancer, and at other various tumors such as lymphoma and breast cancer. The role of HSP is to interrupt the process of apoptosis interfering with formation of tumors, and weaken function of tumor control beyond that of immune surveillance. In case of causing the normal p53 to be mutated, it leads to morphological change of p53 protein and combine with HSP. But, it has not been clarified yet. We intend to examine the meaning of HSP 70 in bladder tumor by investigating the relations among HSP expression and tumor stage, tumor grade, P-gp (glycoprotein) expression as a product of multi-drug resistant gene, and p53 expression in 59 cases of bladder tumor. 1) There were HSP expressions of 8 cases (22%) among 36 superficial bladder tumors and of 9 cases (39%) among 23 invasive bladder tumors. 2) HSP positive reactions were observed in 1 case (8%) of 13 Grade I, and 6 cases (29%) of 21 Grade II and 10 cases (40%) of 25 Grade III. 3) Positive reactions of HSP were showed in 10 cases (40%) among 25 P-gp expression, and in 7 cases (21%) among 34 P-gp non-expression. 4) The p53 proteins were expressed in 12 cases (29%) among 42 ones of HSP non-expression and in 8 cases (47%) among 17 ones of HSP expression. 5) Positive reactions of HSP were showed in 9 cases (23%) among 39 ones of p53 non-expression, and in 8 cases (40%) among 20 ones of p53 expression. 6) 5 patients of 6 with negative expression of HSP and strong positive expression of p53 had poorly differentiated transitional cells, in which one of the patients accompanied with lung metastasis. In view of above study, HSP expression has no correlation with P-gp and stage in bladder tumor, but it has probable pertaining to tumor grade and p53. As it were, tumor cellular differentiation and p53 expression have weak correlations with HSP 70 expression. Meanwhile, judging from poor differentiation, in most cases of HSP non-expressed but p53 strongly expressed, HSP is insufficient to be a prognostic factor of bladder tumor independently, however, in case of using it, as supplementary one, concurrently with p53, it would be valuable prognostic factor in bladder tumor.

5.A Clinical Survey of Cesarean Hysterectomy.

Cesarean hysterectomy remains a necessary procedure for life saving during intractable obstetric hemorrhage. The procedure itself is usually associated with considerable perioperative morbidity, especially when performed under emergency conditions. In this study, the outcomes of 15 cases of cesarean hysterectomy performed at Catholic university of Taegu-Hyosung hospital for 5 years from January, 1993 to December, 1997 were reviewed. There were 22,251 deliveries during this period, cesarean hysterectomy was performed in 13 of 5,753 cesarean section(0.22%) and in 2 of 16,698 vaginal deliveries(0.12%), so more frequently after cesarean section than vaginal deliveries. The age of patients varied from 23 to 39 years old. The higher the age of patients, the higher incidence of cesarean hysterectomy was noted. The most common indication of emergency cesarean hysterectomy was in uterine atony(53.3%), followed by placental disorders(33.3%), uterine rupture(6.7%), and uterine myoma with pregnancy (6.7%). All patients who had cesarean hysterectomy received transfusion from 3 pints to 13 pints. In aspect of fetal outcomes, live birth were 12 cases(80%), one case was fetal death in utero(6.6%), 2 infants were dead during the early neonatal period(13.3%) due to prematurity and RDS (respiratory distress syndrome), so the perinatal mortality rate was 20.0%. The postoperative complications were wound disruption, hematoma, febrile morbidity and disseminated intravascular coagulopathy.

8.Isolated avuision fracture of the tibial attachment of the posterior cruciate ligament

Five patients with isolated avulsion fracture of the tibial attachment of the posterior cruciate ligament were treated in our hospital from September, 1975 to May, 1978. Among the five patients. one was treated conservatively and the others by surgical repair using catgut suture, cancellous bone screw fixation and staple fixation. Four patients were followed from 4 to 10 months poatoperatively. Satisfactory results were obtained in four patients (one was loat in follow up).

9.A Clinical Study of the Surgical Treatment of the Spinal Injuries

The number of spinal injuries are now increasing with the high speed of automobile and industrial development. It is a general trend to treat the spinal injuries more actively in recent years, Among 135 spinal injury patients who were admitted to this hospital from August 1,1972 to August 31,1978, twenty six cases of spine fractures and fracture-dislocations who was treated with surgical measures were evaluated and analysed. Short summary of the followings observed is as follows: 1. Out of 26 patients, there were 22 male and 4 female cases. The ratio between male and female was 5.5:1. The majority (16 cases, 62%) was found in the age group of 20 to 40 years. 55.4% of the cases was caused by traffic accident. 2. The most common site of the lesion was cervical spine (9 cases, 35%) and the most common mechanism of injury was flexion and rotation (10 cases, 38%). 3. Our surgical measures were mainly the early anatomical reduction and anterior and posterior spinal fusion followed by external immobilization. 4. Among 26 cases, 34% had complete paralysis and 31%, incomplete paralysis. Complete or partial recovery was observed in 22% of the completely paralysed cases and in 87% of the imcompletely paralysed cases. 5. The initial recovery of spinal cord injury was observed from 1 to 25 days postoperatively (average 15 days).

10.Treatment of Acromioclavicular Joint Dislocation by Dewar and Barrington Procedure

There are many procedures described for the treatment of acromioclavicular joint dislocation. Most of them, however, have elther technical difficulties or many complications following the operation. Dewar and Barrington procedure was described originally for the old dislocation of the acromio-clavicular joint. However, we have had applied it for the fresh dislocation of the acromioclavicular joint as well as for old one. So far, we have treated 18 cases of fresh dislocations of the acromioclavicular joint together with 3 case of old dislocations from March 1976 to June 1979. The brief summary of the observatlons made are as follows: 1. Among 21 cases, all were grade III by Allman. The age distribution was from 19 to 65 years old. The most common cause of the injury was a traffic accident and the most frequent mechanism of injury was a direct blow. 2. All were treated with Dewar and Barrington procedure and 11 excellent, 7 good, 2 fair and one poor cases were observed in the end result. 3. The impression was that Dewar and Barrington procedure was a excellent procedure for the treatment of fresh acromioclavicular joint dislocation as well as old one.
