1.Effects of Hand Reflexology on Physiological Emotional Responses and Immunity in the Patients with Chronic illness; Chronic renal failure patients and Cancer patients.

PURPOSE: The purpose of this study was to explore the effects of hand reflexology on the physiological?emotional responses and immunity of the patients with chronic illness. This study looked specifically at patients with chronic renal failure(CRF) and cancer patients. METHOD: This study was designed as a quasi-experimental nonequivalent control group pre and post test. Subjects were 54 patients who received dialysis and chemotherapy in one hospital. Thirty-one patients were assigned to the experimental group and 23 to the control group. The hand reflexology was applied to both hands of the experiment group for ten minutes each time, 5 times during three days. For data collection, physiological lab levels, immune cells of blood and questionnaires for emotional responses were measured before and after the program. RESULT: BT of the experiment group was decreased significantly on both of the 1st and the 5th application. PR and BP were decreased significantly on the 1st times, but not 5th times. Hb levels of the experimental group were significantly increased. And emotional responses, vigor and mood scores of the experiment group were significantly increased. B cell and CD19 were increased significantly on the experiment group. Suppressor T cell and NK cell showed significant decrease after the program, but no significant differences between the groups. CONCLUSION: We have found that the hand reflexology helps the chronic patients to improve physiological emotional responses and the immune reaction. Through this result, the hand reflexology is effective as a intervention of psychoneuroimmunologic function.

OBJECTIVEThe evaluation of curative effect of CRF by Bushen Jianpi Huoxue Paidu and combination western medicine.
METHODGather articles mainly of VIP Information and Wanfang Data of the decade to cure CRF by Bushen Jianpi Huoxue Paidu and combination western medicine with random controlled way, and geostatistical analyse with RevMan4. 2 downloading from Cochrane Collaboration.
RESULTThe control group was 3.82 (95% CI, 2.72-5.38), in neurofunction 2 al impairment. The serum creatinine SMD = -0.72 (95% CI, -1.08-(-)0.35). Urea Nitrogen WMD = -3.32 (95% CI, -4.34-(-)2.29), P < 0.05.
CONCLUSIONIn addition to the routine treatment, the method of Bushen Jianpi Huoxue Paidu can be used by using some Chinese drugs to enhance the clinical effect.

Small cell lung cancer is primarily treated with chemotherapy. For patients with end-stage renal disease (ESRD), systemic chemotherapy is often challenging since renal excretion of chemotherapeutic agents might be decreased due to impaired renal function, leading to increased toxicity. No consensus is made so far regarding appropriate dosage and combination of chemotherapeutic agents for patients on hemodialysis. We report two cases of chemotherapy without significant toxicity in small cell lung cancer patients who were on hemodialysis for ESRD.

According to the systematic review of text and opinion(SrTO) developed by the Joanna Briggs Institute(JBI), this study integrated the evidence of the medical cases, papers, medical experience, and other related research of famous experts and schools on the treatment of chronic renal failure in the field of traditional Chinese medicine(TCM) nephropathy, analyzed the academic characteristics and influencing factors from the aspects of the principle, method, recipe, and medicines, and explored the internal logic and law of inheritance and innovation in TCM nephropathy. A total of 36 famous TCM experts and 4 schools were included. The work areas are concentrated in North and South China. 94.0% of the famous TCM experts have experienced master-disciple teaching. 27.8% have studied TCM through family succession. 38.9% have been taught by multiple famous teachers. And 5.6% have not experienced master-disciple teaching. In terms of principle, method, recipe, and medicines, many innovations have been made, with 30.6% of famous TCM experts proposing new theories, 11.1% advocating staged treatment of chronic renal failure, and 47.2% summarizing the treatment methods. The formation of the academic thoughts of contemporary famous TCM experts and schools on the treatment of chronic renal failure is closely related to the learning mode, era, and work area. Contemporary famous TCM experts and schools have the academic characteristics of combining classical and clinical theories and paying more attention to TCM with western medicine supplemented. The field of TCM nephropathy has the characteristics of simultaneous inheritance and innovation. On the basis of inheriting the classical theories of TCM, it absorbs modern medical theories, combines clinical diagnosis with the syndrome and treatment law of contemporary diseases, and makes innovations in principle, method, recipe, and medicines, which has significant clinical efficacy in the treatment of chronic renal failure.

OBJECTIVETo review the clinical and laboratory study on the rhubarb in the treatment of Chronic Renal Failure (CRF).
METHODDocuments of clinical and mechanism study on chinese medicine rhubarb including the simple rhubarb and the rhubarb complex prescription in which rhubarb is the main effective ingredient in recent 10 years were searched and summarized.
RESULTClinical application involving dosage forms, prescriptions, usage and effect of the rhubarb and its prescriptions in the treatment of the CRF and their laboratory study progress wre summarized.
CONCLUSIONRhubarb and all kinds of its complex prescriptions have an assured effect on CRF and their mechanism includes many ways such as ameliorating azotemia, preventing nephritic compensatory hypertrophy and high metabolism situation, and so on. So the rhubarb can be used as an ideal medicine in treating CRF in a long period of time and will have a very brilliant prospect.

7.Efficacy observation of Guan's quadruple therapy for kidney disease on the treatment of chronic renal failure.

OBJECTIVETo compare the differences in the clinical efficacy between Guan's quadruple therapy for kidney disease and the conventional western medication in the treatment of chronic renal failure (CRF).
METHODSEighty patients were randomized into an observation group and a control group, 40 cases in each one. In the observation group, besides the physician instruction of low protein and low phosphorus diet in the two groups, Guan's quadruple therapy for kidney disease was applied, including (1) the isolated herbal moxibustion at Shenshu (BL 23) and Pishu (BL 21), or Ganshu (BL 18), Zusanli (ST 36) and Guanyuan (CV 4) alternatively; (2) acupuncture at the auricular points such as shen (CO10), jiaogan (AHR6), Shenshangxian (TG2), fei (CO14) and pizhixia (AT4); (3) injection of mixture of astragalus injection and lidocaine injection at Feishu (BL 13), Pishu (BL 21), Shenshu (BL 23) and the others; (4) modified Shenshuai Yihao decoction according to syndrome differentiation. In the control group, the conventional western medication was used. After 6 months of treatment, the differences were observed between the two groups in the clinical efficacy, serum creatinine (Scr), blood urea nitrogen (BUN), endogenous creatinine (Ccr) and 24 h urine protein quantitation (UPQ).
RESULTSIn the observation group, after treatment, the remarkably effective rate was 50.0% (20/40) and the total effective rate was 82.5% (33/40), which were superior to 25.0% (10/40) and 45.0% (18/40) in the control group (both P < 0.05). After treatment, the levels of Scr, BUN and 24 h UPQ were reduced apparently (all P < 0.05), and the level of Ccr was increased apparently in the two groups (both P < 0.05). After treatment, the levels of Scr, BUN and 24 h UPQ were reduced much more apparently in the observation group as compared with those in the control group (all P < 0.05).
CONCLUSIONGuan's quadruple therapy for kidney disease achieves a better efficacy on CRF compared with the conventional western medication. This therapy improves renal functions, relieves clinical symptoms and physical signs and benefits the life quality of patients.

8.Effects of statins on delaying progression of chronic kidney disease: a meta-analysis.

OBJECTIVEWhether statins can slow down the progression of chronic kidney disease (CKD) remains controversial. We performed a meta-analysis to evaluate the effects of statin therapy on disease progression in adult patients with CKD who did not require dialysis therapy.
METHODSWe searched the electronic databases for relevant randomized controlled trials (RCTs) published by February 2015. Random-effects meta-analysis of RCTs was used to pool the renal outcomes of the patients.
RESULTSTwenty-eight studies (30 RCTs) involving a total of 45 688 participants were included in the analysis. Compared with the control groups, statins produced no effects in preventing end-stage renal disease (ESRD) [relative risks (RR) 0.98, 95% confidence intervals (CI): 0.91-1.05] and in reducing the risk of doubling of the serum creatinine level (RR 1.43, 95% CI: 0.26-7.79). Statin therapy was associated with a lowered risk of estimated glomerular filtration rate (eGFR) reduction by 25% or more (RR 0.91, 95% CI: 0.83-0.99) and delayed the reduction of eGFR [standardized mean differences (SMD) 0.04, 95% CI: 0.02-0.07]. In subgroup analyses, the benefit of statins on changes in eGFR was statistically significant in patients with moderate CKD (SMD 0.09, 95% CI 0.04-0.13). Among different statins, atorvastatin was associated with a beneficial effect on kidney function (SMD 0.10, 95% CI 0.03-0.17). Patients who received high-intensity statin therapy showed significant changes in eGFR (SMD 0.12, 95% CI: 0.02-0.21).
CONCLUSIONStatin therapies may not prevent ESRD or doubling of serum creatinine level, but can improve GFR or delay the reduction of GFR in CKD patients. The therapeutic effects are associated with the patients' baseline eGFR levels, statin types and therapy intensity.

10.Acute renal failure in acute pyelonephritis.

BACKGROUND: If acute renal failure develops in patients with acute pyelonephritis, it is most commonly due to hypovolemia, sepsis, drug therapy or urinary obstruction. But there have been reported many cases about patients of acute renal failure derived from acute pyelonephritis itself without any predisposing factor. To find out the predisposing related factors, we analysed the clinical patterns of patients of acute pyelonephritis with acute renal failure compared to that of patients of acute pyelonephritis without acute renal failure. METHODS: From January 1996 to December 2000, the authors identified 172 patients older than 16 years of age who admitted to Korea University Hospital for acute pyelonephritis. Among them, patients whose serum creatinine level had been less than 1.5mg/dL before admission and who did not have any of chronic renal failure, diabetic nephropathy or hypertensive nephropathy were recruited. According to the level of serum creatinine at admission, the patients were divided into two groups. If ones serum creatinine level at admission was less than 1.5mg/dL, he or she included to control group, if more than 1.5 mg/dL to acute renal failure group. And we compared their clinical features and laboratory values. RESULTS: The patients with acute pyelonephritis complicated to renal failure showed the tendency of older age, more dehydration, more inflammation, more frequent abnormal findings in abdominal ultrasonography and more frequency of chronic systemic disease than those without renal failure. Acute renal failure due to acute pyelonephritis might be recovered rapidly by general supportive care like fluid replacement and adequate antimicrobial therapy. CONCLUSION: Acute pyelonephritis should be considered rare cause of acute renal failure. Especially in case of old age, severe dehydration, severe inflammation and comorbidity with chronic systemic disease involved in kidney, it would be helpful to observe the clinical course closely.
