3.A Case of Subcutaneous Panniculitic T-cell Lymphoma in a Child.

We report a case of subcutaneous panniculitic T-cell lymphoma (SPTCL) which occurred in a 10-year-old Korean girl. Her disease presented as multiple erythematous subcutaneous nodules on the right cheek, left chest, abdomen, left flank, both calves, and left shin with systemic symptoms. She had a protracted course of multiple erythematous subcutaneous nodules for 2 months often with spiking fever. Histopathologic findings for the subcutaneous nodules revealed lobular panniculitis-like findings composed of atypical small, bland lymphocytes and histiocytes. Characteristically, atypical lymphocytes rimmed individual fat cells in a lace-like pattern and some histiocytes occasionally phagocytosed WBCs. Bone marrow findings revealed increased phagocytic histiocytes with engulfed hematopoietic cell. The immunophenotypic studies showed CD45RO (UCHLl)+, CD20-, CD4-, CD8+ and CD56+ (focal), lysozyme+, CD45 (LCA)+ and EBV-. She received three cycles of high-dose cyclophosphamide, adriamycin, vincristine, prednisolone (CHOP) and methotrexate, intrathecal methotrexate and one cycle of fludarabine, mitoxantrone and dexamethasone (FND) chemotherapy. She died of acute renal failure during multiple chemotherapy.

4.Actinomycosis on left submandibular area: a case report.

Actinomycosis is a chronic suppurative and granulomatous bacterial infection characterized by contiguous spread, abscess formation and sinus tract formation. There are four clinical forms according to the lesional site, as 1) cervicofacial, 2) thoracic, 3) abdominal, and 4) disseminated form. Recently, we experienced a case of 54 year-old patient with left mandibular actinomycosis. The pathognomonic findings of actinomycosis is sulfur granule with multiple filaments in Gram-stain and the treatment of actinomycosis is surgical excision of mass or sinus tract with massive antibiotics (esp. Penicillin) therapy for 6 to 12 months.

5.MR imaging of intracranial lipoma.

Five cases of intracranial lipoma diagnosed by MR are presented. MR imaging was performed on a 0.2T permanent unit using T1 weighted, proton density-weighted, and T2 weighted spin echo sequences. In two patients, gadolinium-enhanced T1 weighted image was also obtained. The lipomas were located dorsolaterally to the splenium of the corpus callosum (n=1), inferior to the splenium (n=2), in quadrigeminal plate(n=1) and in the presumed corpus callosum area in the case of agenesis of corpus call?um (n=1). The size and shape of the lipomas were variable. No contrast enhancement was seen in post contrast study. Sagittal T1 weighted image appeared to be the most useful imaging plane for the demonstration of the relationship between the lipoma ad the adjacent normal structures. The Homogenous signal intensity paralleling the fat signal and the characteristic location of the lesion are considered to be helpful in the differential diagnosis from dermoid cyst or teratoma.

8.Evaluation of intracranial hemorrhage in neonates and infants using real time sonography

The real time high resolution mechanical neurosonographic sector scanner is a convenient and useful instrumentfor detection of intracranial lesions in premature infants as well as low birth weight. From this experience,authors report with 104 neonates and infants who had been suspected intracranial hemorrhage and had been detectedand graded by sonography. The results were as followed: 1. The incidence of intracranial hemorrhage is 20 of 46(43.5%) neonates who had been studied before 3 days of age. But 7 of 16(43.7%) neonates showed ICH between the 4-7days of age. 2. There was no difference of prevalence in sex ratio in neonates with ICH. 3. There was closerelationship between ICH and gestational weeks. 15 of 27 (55.6%) neonates who were born under 32 gestational weeksshowed ICH was graded as 3 or 4 in 7 of 10. 4. 10 of 16(62.5%) neonate who were born under 1,500 gram body weighthad ICH within first 4 days. The severity of ICH was graded as 3 or 4 in 7 of 10. 5. The mode of deliveryinfluenced neonatal ICH incase of difficulty delivery such as forcep(4/4, 100%), vacuum (7/10, 70%), breech (3/3,100%), and C/S(13/33, 39.4%). 6. Incidence of ICH in neonates with pathologica lung conditions (29/40, 72.5%) washigher than ICH with normal chest findins(15/64, 23.4%). 7. For frequent follow up study of brain, neurosonographywas very helpul and economic in case of ICH, and could find congenital glioblastoma multiformed(1), and low gradeastrocytoma(1) incidentally.

9.Detection of Glaucomatous Visual Field Defect by Screening Program of Humphrey Field Analyzer.

We have experienced automated and computerized perimetry using a Humphrey Field Analyzer in our Glaucoma Services. To evaluate its clinical efficacy, we report the results of the Armaly central field screening test in comparison with those of central 30-2 threshold test of the Humphrey Field Analyzer. One hundred fortythree eyes of 83 cases of normotensives, ocular hypertensives and glaucoma patients with open-angle or narrow-angle were enrolled in this study. Fiftyfour out of 61 patients (88.5%) and 73 out of 81 eyes (90.1 %) with visual field defect were detected by the screening test of the Humphrey Field Analyzer. Eight out of 81 eyes (9.9%) with visual field defect detected by the screening test were confirmed as false positive. All points of false positive were solitary, which tended to occur more frequently on superior vusual field. False negative of 8 eyes (12.9%) detected by the screening test were confirmed by the threshold test. The point of false negative was more frequent on superior field and showed to locate more peripherally from the fixation point. There was no significant difference in either central sensitivity or age of the false ring.

10.Clinical Effects of Topical Betaxolol Hydrochloride(Betoptic(R)) in Glaucoma Patients.

To evaluate the clinical usefulness of a newly developed beta-one selective blocker. Betaxolol hydrochloride(Betoptice(R)), seven patients(14 eyes) with primary open-angle glaucoma who were instilled twice a day with a drop of 0.5% Betoptic were enrolled in this study. Intraocular pressure(IOP), systolic and diastolic blood pressure, pulse rate and pupil size were measured with the evaluation on the local adverse reaction before and after the Betoptic treatment of 1, 2, 4, 8 and 12 weeks. The following results were obtained: 1. Significant reduction of IOP was noted at all intervals examined after the treatment of 0.5% Betoptic and it had the additive effect on pilocarpine. 2. The systemic side reaction of Betoptic on either pulse rate or blood pressure was not significant and pupil size was not changed by Betoptic. 3. The local transient and mild stinging sensation with tearing was followed after the instillation of 0.5% Betoptic eye solution.
