Additional two cases of Klinefelter's syndrome with a literatural review was presented Two cases were revealed characteristics of Klinefelter's syndrome such as small testes, hyalinization of seminiferous tubules, azoospermia, increased FSH, normal 17-ketogteroid and positive sex chromatin. One case was associated with left inguinal crytorchidism.

3.Clinical Analysis of Extremity Ulcer during Ten Consecutive Years (1971 - 1980).

From January 1971 to December 1980 the authors observed 165 cases of chronic ulcers of the extrernities in the Department of Dermatology, Chonnam University Hospital. The results obtained can be summarized as follows. 1) During this period, the mean average percent of ulcer patients seen was 0.41 of the yearly total patients. 2) The ages of onset was quite varied with the highest group in the 5th decade. The ratio of men to women was 2.93: 1. 3) The most common predilection site was right lower extremity. 4) Leprosy, especially T-type, was the most comrnon underlying disease.

5.Effects of Obesity on Pregnancy Outcomes.

OBJECTIVE:To compare pregnancy outcomes between obese and nonobese women and to determine the effect of gestational weight gain on pregnancy outcome in obese women. METHODS: A retrospective cohort study was conducted comparing 100 obese and 300 nonobese women who delivered a singleton live birth at Yeungnam university hospital from June 1998 to Dec 1998. Morbid obesity was defined as a body mass index greater than 30. The incidence of selected perinatal and neonatal outcome was assessed for two groups. RESULTS: Morbidly obese patients were more likly to experience pregnancy complications including gestational diabetes mellitus, preeclampsia, placental abruption, fetal distress, meconium aspiration, cesarean delivery & birth trauma. However, these were not affected by gestational weight gain in morbidly obese women. Weight gains more than 12kg were strongly associated with birth of a large for gestational age(LGA) neonate, however, poor weight gain did not appear to incrcase the risk of delivery of a low birth weight neonate. CONCLUSION: To optimize fetal growth, weight gain of 7-12kg for obese women appear to be appropriate. To reduce the risk of delivery of an LGA neonate, the optimal gestational weight gain for obese women should not exceed 12kg.

7.Interlocking Kuntscher Nailing

Kuntscher introduced the interlocking nail as the name of Detensionsnagel at 1968. Klemm, Kaessman, Hempel used it clinically through transverse hole. Authors tried to make the sagittal hole at the dorsum of the Kiintscher nail, which was interlocked- from the slot to the hole by bone screw. The direction of the sagittal hole crossed right angle to the transverse hole. Analysis of 35 cases conducted following conclusion. 1. Transfixing the screw at the sagittal hole of the nail can be done easily when transfixing through the transverse hole is difficult. 2. There was no difference in stability between transverse hole and sagittal hole transfixation. Lateral movement of the nail was disappeared after transfixation of the screw at the sagittal hole. Interlocking system is very similar to Harrington distraction rod system. 3. One case of nail breakage had occured at the transverse hole. Nail breakage easily occurs if the hole is situated near the fracture site. For prevent nail breakage larger nail insertion is desirable. (More than 14mm in femur and more than 12mm in tibia.) 5. Indication of the I-M nailing is widened by screw interlocking: from the subtrochanteric fracture to the supracondylar fracture of the femur. It is also useful in arthrodesis of the knee joint. 6. Addition of the Interlocking in ordinary I-M nailing patient could walk 3 to 10 days after operation and radiologic union achieved 12 to 20 week after opeation.

8.Double - Blind Evaluation of the Effectiveness of Vibramycin to Acne Vulgaris.

In order to evaluate the effectiveness of Vibramycin (Doxycycline) to acne vulgaris, a double-blind study was performed in forty three patients of ance vulgaris. The patients were classified as to the severity of the disease before the treatment according to Wand, et al. and the degree of the effectiveness was evaluated weekly by Clinical improvement. The results obtained were as follows: Among 14 patients who received a 100mg capsule orally per day for four weeks, 12 cases (85.7%) showed excellent improvement, whereas in the placebo group only 4 out of 12 patients (33.3%) showed good responses. Statistical analysis of the data revealed that there was a significant difference (P<0.05) in effectiveness between the Vibramycin group (100mg) and the placebo group, and that at the end of the first week of the treatment there was no significant improvement, but at the end of the third week there was a highly significant improvement, p_value between the first-week response and the third-week response among the patients in administering 100mg of Vibramycin being less than 0.01. It was noted that 24 out of 43 patients(55.8%) had a distint familial history of the disease, suggesting that there might be a predisposing factor involved in the manifestation of acne vulgris. It is concluded from this study that Vibramycin, when administered in dose of 100mg per day for more than three weeks, is effective in improving the acne vulgaris.

9.Serologic Evaluations and The Effect of Dantrium(R) for Preventing Malignant Hyperthermia in Strabismus Children.

Malignant hyperthermia is of particular interest to ophthalmologist because of higher incidence in children and its' potential for occurrence in strabismus and blepharoptosis patients. We analyzed the serologic data including serum CPK and the effectiveness of dantrolene sodium in strabismus children for screening carriers of malignant hyperthermia. The results were as follows; 1. Comprehensive studies including family history, laboratory test, underlying muscular disorder, muscle biopsy for susceptible individuals can help to make the diagnosis accurately. 2. No significant difference on serum CPK, LDH, GOT levels between the strabismus patients and control groups. 3. Dantrolene Sodium is effective agent for preventing malignant hyperthermia. 4. General anesthesia or stress associated with surgery and sex were the factors influencing serum CPK levels(i.e., values were significantly higher in postoperative periods and males).
