The mechanism of development of pulmonary edema associated with -sympathomimetic agent is still not fully understood. 2-sympathomimetic drugs such as ritodrine, terbutaline, and fenoterol are widely used for the treatment of premature uterine contraction. We describe a case of pulmonary edema that occurred suddenly during Cesarean section in a previously healthy parturient, associated with ritodrine pretreatment for 5 days in an attempt to arrest preterm labor. Pulmonary edema was completely treated with mechanical ventilation and diuretic therapy.

3.Study on the frequency distribution of the metacercarial density of Clonorchis sinensis in fish host, Pseudorasbora parva.

Since the pattern of the frequency distribution of a parasite within a host showed an overdispersed pattern, various statistical models such as Poisson, negative binomial and lognormal distributions have been applied on the population dynamics in host-parasite relations. The observed data on the number of metacercariae of Clonorchis sinensis in a suitable intermediate host, Pseudorasbora parva which were collected from an endemic area, Juk-Rim River, Kim-Hae Goon, South Kyong-Sang Do were applied on the statistical models. The results obtained are as follows: By the calculation of the raw data, 258(94.9 percent) out of 272 fish showed positive to the metacercarial infection of C. sinensis. The mean number of metacercaria in a fish was 335.1. The standard deviation was 250.6 and the mode was observed between 250 and 299. 4.A mathematical approach to the mode of transmission of clonorchiasis in the inhabitants of Nak-dong and Han River basin.

To understand the mode of transmission in clonorchiasis, a survey was made in Kim-hae Goon, South Kyong-sang Do (=Province). The mathematical analysis of the age prevalence was done on the egg positive rates. And another analysis for the comparison was also made to the cited data from two areas, North Kyong-sang Do and Ko-yang Goon, Kyong-gi Do. Some catalytic models of H. Muench (1959) were applied to the observed age prevalence. Because the both parameters, such as force of infection(a) and loss of positivity(b) were considered to be constant for a long period in the surveyed area, the two stage catalytic model by Muench was chosen to the analysis. In the surveyed area, Kim-hae Goon where the egg positive rates were 56.2 percent and 61.2 percent (by Kim, 1974), the constant values of 'a' were found to be 0.051 and 0.089 respectively. In other words, the force of infection was 51, 89 per 1,000 susceptibles. The values of 'b' were found to be 0.006 and 0.005. This means that the rates of disappearance from egg positive cases to negative were 6 and 5 per annum per l,000 positive cases in above area. Therefore, the two catalytic curves were expressed by the following equations, y = 1.133 {e(-0.006t) - e(-0.051t)} and y = 1.047 {e(-0.005t) - e(-0.089t)} respectively. In the cases of North Kyong-sang Do and Ko-yang Goon, Kyong-gi Do where the egg positive rates of clonorchis shown as 27.7 percent and 15.2 percent by Shin (1964) and Kim (l974), the curves were expressed by y = 1.769 {e(-0.010t) - e(-0.034t)} and y = 2.857 {e(-0.020t) - e(-0.027t)} respectively. From the above mathematical analyses by age prevalence in clonorchiasis, it was considered that the mode of transmission of clonorchiasis in the surveyed area, Kim-hae Goon presented more rapid pattern than those of North Kyong-sang Do and Ko-yang Goon, Kyong-gi Do.

6.Bladder Mucosal Graft for Severe Hypospadias Repair.

Bladder mucosa has been reported as an excellent tissue for construction of a neourethra. We used bladder mucosa in 3 cases when there was inadequate foreskin to create a neourethra. Two cases were penoscrotal hypospadias with chordee which were repaired in one stage operation and one case was scrotal hypospadias with severe chordee which was repaired in two stage operation. The. sizes of harvested bladder mucosa were 4x 1.5cm, 10 x 3.5cm and 15 x 3cm respectively. Two minor and one major complications occurred. The minor complications were urethral stricture in one case which was managed with urethral dilation and penile swelling after self-voiding in one case which required urinary diversion for several days. In one case severe problem with entire neourethra stricture necessitated internal urethrotomy and intermittent urethral dilatation. In all cases sufficient graft tissue could be obtained and there were no urethrocutaneous fistulas. Thus, the use of bladder mucosal graft in hypospadias repair is feasible in cases in which previous repairs have railed or in which sufficient foreskin for neourethral reconstruction is lacking.

7.Treatment Options for Lower Ureteral Calculi: Expectancy or Endourologic manipulations.

To determine the optimal therapeutic approach for lower ureteral calculi at the hospitals which have not possessed ESWL, we reviewed 85 cases initially treated with expectancy for more than 7 days. Of 85 cases, 37 were treated with expectancy only and the remaining 48 cases received the endourologic manipulations such as stone basket and ureteroscopic removal. A total cumulative spontaneous passage rate at 3 weeks was about 52.9% and the possibility of spontaneous passage of remained stones at 3 weeks was not significant. The spontaneous passage rate was higher in the cases with the stones of small size (less than 6mm), smooth-round shape and without urinary tract obstruction than the other cases. 26 cases were treated with the stone basket and stone extraction rate was only 42.3%. The stone extraction rate was higher in the cases with the stones of smaller size (less than 4mm), irregular-speculated shape and with short duration of previous expectancy than the other cases. A total of 32 cases received the ureteroscopic manipulation and the success rate was 87.5%. Thus, we recommend initial expectancy for up to 3 weeks in the lower ureteral calculi smaller than 10 mm. And after this period, the positive and selective endourological manipulation considering the stone size, shape and expectant duration may be desirable in respect of treatment efficacy.

8.The Occult Osseous Lesions on Magnetic Resonance Imaging Associated with Acute Anterior Cruciate Ligament Tears

Magnetic resonance imaging(MRI) is accepted as a useful tool for the diagnosis of injury of the knee recently. The anterior cruciate ligament tear is accompanied by the damage of the osseous and soft tissue of the knee. The occult osseous lesion is found on magnetic resonance imaging as bone density change that is not found on the simple radiograph. From Jan. 1992 to Apr. 1994, magnetic resonance imaging was performed on 44 patients with acute anterior cruciate ligament tears. MRI was done within 6 weeks of the index anterior cruciate ligament injury. The criteria of decreased signal intensity on the Tl weighted image and increased signal intensity on the T2 weighted image was applied for the incidence assessment of the occult osseous lesion. After arthroscopy with or without ACL reconstruction, follow-up simple radiograph was checked every 3 months. Occult osseous lesions were documented in 32(73%) of the 44 patients. Of the 32 patients with bony lesion, 81% had lesions of the lateral compartment. Lateral femoral condyle was involved in 15 cases(46.9%), and lateral tibial plateau in 22 cases (68.8%). Follow-up X-ray revealed no joint deperession in all of the 32 patients with the occult osseous lesion. This result suggests that there is a high correlation between the occult osseous lesion of lateral compartment of the knee on magnetic resonance imaging and anterior cruciate ligament tear. The presence of the occult osseous lesion in the patient with acute anterior cruciate ligament tear did not affect rehabilitation and weight bearing.

9.Effect of Transforming Growth Factor-beta1 on Expressions of Epidermal Growth Factor and Transforming Growth Factor-alpha in DU145 Androgen-Independent Prostate Cancer Cells.

PURPOSE: This study was designed to identify the possible mechanism of insensitivity of DU145 prostate cancer cells to the transforming growth factor (TGF)-beta1-mediated growth inhibition. MATERIALS AND METHODS: DU145 cells were treated with 4, 40, 100 pM TGF-beta1 for 3, 6, 9 days. Initially we performed the growth assay. After that, we analysed the change of cell cycle using fluorescence flow cytometry. At each time point, Western blot analysis with cell pellets was performed to investigate the change of expressions of epidermal growth factor(EGF), TGF-alpha, EGF receptor(EGFR) and TGF receptorII(TbetaR-II) proteins. RESULTS: The growth rate of TGF-beta1-treated cells was initially suppressed, but over time returned to control level. Flow cytometric analysis revealed that TGF-beta1-treated cells showed an increase in apoptotic/G1 phases, and concurrent decrease in S, G2/M phases until 6days. On day 9, however, TGF-beta1-treated cells showed a persistent increase of apoptotic unclei in spite of recovery of apoptotic/G1, S and G2/M phases. Western blot analysis showed that the intensity of EGF or TGF-alpha band decreased in dose-sependent manner on day 6. However, the intensity of each band increased up to the control level on day 9. the expression of EGFR or TbetaR-II protein did not change after treatment of TGF-beta1. CONCLUSIONS: these results suggest that EGF and TGF-alpha sould mediate in part the escape fron the inhibitory effect of TGF-beta1 in DU145 cells.

10.Early Experience of Laparoscopic Bladder Neck Suspension for Female Genuine Stress Urinary Incontinence.

For the treatment of genuine stress urinary incontinence(GSUI), various surgical approaches have been developed since early in this century. Laparoscopic bladder neck suspension has recently been described as an alternative method to the open surgical correction of GSUI. We had treated 16 patients of GSUI with laparoscopic Burch procedure from May to November 1995. Mean age of the patients was 42.5 years(range:28-77years) and mean parity was 3.7. There was no serious operative complications such as bladder rupture or bleeding needed transfusion. The duration of follow up was 6.2 months(range:3-9 months) in average. Incontinence was completely disappeared in 11 patients(68.7%) and significantly improved in 5 patients (31.3%), and no patient was recurred during follow up periods. Postoperatively, there was no significant complication, except two cases of urinary tract infection. Early results of the small number of women studied suggest that this new technique can provide successful outcomes in properly selected patients.
