2.A Case of Multiple Cutaneous Leiomyoma.

We present a case of multiple cutaneous leiomyoma in a 45 year old woman. About 20 years ago, multiple skin colored papules developed on right cheek and man4ibular area. About 10 years after the onset of the disease a uterine myoma developed which wae tree,ted by total hysterectomy. The skin lesions were gradually increasing in number and size, Some larger nodular lesions changed to brown in color and became painful after cold exposure. The histopathologic finding showed typical leiomyoma arieing from arrector pilorum muscle.

Retroperitoneal foregut duplication cyst is an extremely rare congenital malformation. Pathologically, this lesion contains both gastric mucosa and respiratory type mucosa; radiologically, it is often challenging to differentiate it from the other cystic neoplasms that present a similar appearance. We report here on a case of retroperitoneal foregut duplication cyst that was lined by both gastric and pseudostratified ciliated columnar epithelium, and it was also accompanied by a pancreatic pseudocyst. Initially, it presented with peripancreatic and intrapancreatic cystic masses in an asymptomatic 30-year-old man, and this man has since undergone surgical resection.

Gastric volvulus can either present as an acute or chronic symptoms according to the degree of gastric rotation and subsequent obstruction. The diagnosis of gastric volvulus is often difficult and is mainly based on imaging studies. We describe a case of mesenteroaxial gastric volvulus associated with paraesophageal hernia, well demonstrated on upper gastrointestinal (UGI) series and coronal reconstructed CT image.

8.The Continuous Monitoring of Mixed Venous Oxygen Saturation for the Estimation of Cardiac Output Changes for Liver Transplantation in Dogs.

BACKGROUND: In the management of liver transplantation, massive blood loss, interruption of venous retum to the heart, sudden infusion of acidic, cold and hyperkalemic blood from the graft, and uses of inotropic and vasoactive agents make cardiac output (CO) change unpredictably. We tried to find convenient method to estimate the change of CO by mixed venous oxygen saturation (SvO2) which can be monitored by pulmonary artery catheterization with fiberoptic oximeter using a spectrophotometer, By adapting the equation of oxygen transport and oxygen consumption(VO2), the equation CO=V/O2(Hbx0.134)x1/(SaO2-SvO2) can be obtained. If we assume the VO2 and hemoglobin concentration is constant and hypoxemia is excluded, CO might be proportional to I/(100-Sv O2). METHOD: For the management of orthotopic liver transplantations in 5 mongrel dogs, we continuously monitored Sv O2 with fiberoptic pulmonary catheter and intermittently measured the CO by thermodilution method according to operative phases. The Pearsons correlation coefficients between CO and mean arterial pressure, CO and S vO. and CO and 1/(100 SvO2) were measured in each dog. RESULT: The correlation coefficients between CO and 1/(100 SvO2) are raging from 0.74 to 0.98 with a corrected mean of 0.847 and significant in all dogs (p<0.05), and the correlation coefficients between CO and SvO2 are ranging from 0.67 to 0.96 with a corrected means of 0.786 and significant in 4 of 5 dogs. CONCLUSIONS: For the management of liver transplantation, the continuous monitoring of mixed venous oxygen saturation by a fiberoptic pulmonary catheter is helpful to detect changes of cardiac output.

9.Volvulus of the splenic flexure of the colon.

The definition of volvulus is an axial twist of a portion of the gastrointestinal tract along its mesentery. The involved bowel is obstructed partially or completely with a variable degree of arterial and venous occlusion. The colon is the most common site for volvulus. The splenic flexure is the least common site of colonic volvulus. We experienced a case of the volvulus of the splenic flexure. It will be the 30th case of the volvulus involving the splenic flexure in the English literature, to our knowledge. A 30-year-old woman was admitted due to abdominal pain and distention with vomiting. An emergency barium study revealed characteristic "bird beak" sign. Surgery was performed resecting the involved colon of splenic flexure. The result was excellent.

10.Perioperative changes of serum leptin levels in women with uterine leiomyoma.

OBJECTIVE: To investigate possible relationship between uterine leiomyoma and serum leptin levels, we compared serum leptin levels between women with uterine leiomyoma and women without leiomyoma, and then investigate the effects of hysterectomy on serum leptin levels. METHODS: Serum leptin levels were measured before (day-1) and after (day+5) surgery for 32 women with uterine leiomyoma and were measured when they visit our health promotion center for 32 normal women. We compared serum leptin levels between two groups, and then we analyzed perioperative changes of serum leptin levels in women with uterine leiomyoma. Pearson correlation test was performed between pre-/postoperative leptin ratio and resected uterine weight to investigate whether the changes of serum leptin levels are directly related with resection of leiomyoma. RESULTS: In women with uterine leiomyoma, serum leptin levels were lower compared with normal group (7.41+/-4.31 ng/mL vs. 17.56+/-8.65 ng/mL) (P<0.05). Postoperative serum leptin levels were decresed than preoperative serum leptin levels (6.07+/-2.98 ng/mL vs. 7.41+/-4.31 ng/mL) (P<0.05). Pearson correlation coefficient between pre-/postoperative leptin ratio and resected uterine weight showed no statistical significance. CONCLUSION: Serum leptin levels were lower in women with uterine leiomyoma. And serum leptin levels were more decreased after hysterectomy. There were significant postoperative decrease in serum leptin levels but the result between pre-/postoperative leptin ratio and resected uterine weight revealed no direct correlation. It seems that lower serum leptin levels maybe due to postoperative stress and neuroendocrinological response rather than myoma itself. In conclusion, further investigation is needed for clinical use of serum leptin levels as a serum marker after conservative treatment of uterine leiomyoma.
