1.Radiologic Changes after Lumbar Discectomy

Open discectomy is one of usual treatment for herniated intervertebral disc. We may expect the reduction of height of intervertebral disc due to decreased volume by partial resection of herniated disc. But the effect of decreased height of intervertebral disc space to posterior joint and spinal canal has not been identified, and the correlation between the amount of reduced height and the changes of range of motion has been undetermined. Thus, we analyzed radiologic changes of disc height and segmental motion by the flexion-extension view in 20 patients who had undergone discetomy for prolapsed intervertebral disc from June 1989 to January 1991 who could be followed up for 3 years or more. As a result, lumbar discetomy associates with 1) significant decrement of disc height (average 14%), 2) decreased disc motion of involved segment without hypermobility (average 34%), 3) hypermobility of adjacent segment, 4) and posterior facet overriding consistent with recurred low back pain.

2.A Case of Erythrodermic Form of Mycosis Fungoides.

We report a case of an erythrodermic form of mycosis fungoides in a 68-year-old male, who showed generalized erythroderma with scales for 3 months. Lymphadenopathies in the inguinal and neck areas were present. Histopathological findings showed epidermotropism, perivascular atypical lymphocyte infilteration in the upper dermis, and dermatopathic lymphadenopathy in the inguinal lymph node. T cell markers were positively stained in the immunohistochemical study. These clinicopathological features were consistent with an erythrodermic form of mycosis fungoides(TNM IIl). The patient was treated with PUVA therapy and low dose chemotherapy.

5.Relationship between Clinical Symptoms and P300 in Schizophrenia.

OBJECTIVE: This research was performed to evaluate the relation clinical symptoms and P300 in patients with schizophrenia. METHODS: 25 patients with schizophenia by DSM-IV in Soonchunhyang Chunan University hospital were participated. Clinical symptoms were checked by BPRS (Brief Psychiatric Rating Scale) and PANSS (Positive and Negative Syndrome Scale). Auditory event related potentials P300 components were obtained with "Oddball paradigm". RESULTS: P300 in T 3 was smaller and more delayed than T 4 in patients with schizophrenia. Especially, P300 on T 3 were correlated with BPRS scores and negative symptoms scores of PANSS, which were also correlated with P300 on Fz. CONCLUSION: The results of this study suggest that P300 on frontal and left temporal lobe reflect clinical symptoms in patients with schizophrenia.

Background:
During the coronavirus disease 2019 (COVID-19) pandemic, health care workers (HCWs) have faced multiple physical and psychological challenges while carrying out their duties. In this study, we examined the experiences of specific groups of HCWs during the pandemic.
Methods:
From 18 November to 30 December 2020, we conducted a qualitative study using semi-structured, face-to-face interviews with four groups of 14 HCWs in three cities (Seoul, Daegu, and Gwangju) in South Korea. The HCWs who participated in the focus groups included physicians, nurses, medical practitioners, and cleaning staff who directly or indirectly cared for patients during the COVID-19 epidemic. Interviews were transcribed verbatim and analyzed using the consensual qualitative research approach.
Results:
Our qualitative data analysis revealed four main domains: work-related struggles, personal life-related struggles, psychological stress, and health-related struggles. Health care providers were challenged by working in critical situations and were overwhelmed by heavy workloads, fear of infection, lifestyle changes, and psychological and physical struggles.
Conclusion
Our findings could serve as a foundation for establishing health care systems and policies that help HCWs cope with occupational stress, thus increasing their ability to adapt to the ongoing COVID-19 pandemic.

8.Serum Fluoride Level in Normal Adult Women and Changes in Serum Fluoride Level after Disodium Monofluorophosphate Administration.

BACKGROUND: Since the morning fluoride level of 10 uM is recommended for adults patients being treated for osteoporosis so far, measurement of serum fluoride level is important to detect abnormally high levels or to detect levels below the therapeutic windows. Aims of this study are to determine the normal range of serum ionic fluoride levels in Korean female adults (from 5th to 7th decade), and to evaluate the in vivo fluoride pharmacokinetics of monofluorophosphate in Korean adults. METHODS: Serum level of fluoride was measured from blood samples of 72 female subjects (age 43-69years) using an ion selective electrode. For pharrnacokinetics of monofluorophosphate-calcium (MFP-Ca), 6 subjects (age 27~45 years) were included to be withdrawn the blood hourly for the first S hours and the blood was withdrawn at 24 hours after a single dose of MFP-Ca. RESULTS: Mean level of serum fluoride was 1.64+-0.12uM in 5th, 6th, 7th decades adults, and there was no difference of serum fluoride levels among age groups. Peak serum fluoride level exhibited 5.02+-0.67pM, and returned to basal level on 24 hours after a single dose of MFP-Ca. CONCLUSION: This study shows that mean serutn fluoride of Korean female adults (from 5th to 7th decade) is not different from that of other reports, and a single dose of MFP-Ca does not cause serum fluoride levels above the recommended therapeutic windows of 5-10uM for 24 hours.

9.Comparison of Appendicitis Between Children and Adults.

BACKGROUND: Appendicitis is one of the most common causes of abdominal emergency with a life time risk of 6-8%. Children are unable to give an accurate history, and since acute nonspecific abdominal pain is more common than appendicitis at this age, diagnosis and management of appendicitis are often delayed. This results in more frequent complications and difficult management. In order to evaluate whether pediatric appendicitis should be thought as more urgent and serious because of the delay in its diagnosis, we compared the clinical characteristics and outcomes of appendicitis in children with those of adults. METHODS: We retrospectively reviewed 106 adults and 43 children who were pathologically verified with appendicitis after an appendectomy. RESULTS: There was no difference in the duration of symptoms. Advanced appendicitis was more frequent in children (60.5% vs. 53.7%). In children, the symptom durations of advanced appendicitis were similar to those that of early appendicitis, but were significantly longer in adults. Children showed no increase in cumulative rate of advanced appendicitis in relation with prolonged symptom duration whereas adults showed a steady increase after 72 hours of onset. The complication rate was higher in advanced cases in both, but children had a lower complication rate than adults. The postoperative hospital stay was significantly longer in advanced cases in both, but children had a relatively shorter hospital stay. CONCLUSIONS: Pediatric appendicitis was more prone to early perforation with a relatively higher perforation rate. However, children had a lower morbidity rate and a shorter admission stay. Therefore, appendicitis in children is not necessarily more serious and urgent than in adults.

10.Serial Changes in Plasma Cytokine Levels in Hemorrhagic Fever with Renal Syndrome.

Hemorrhagic fever with renal syndrome (HFRS) in Korea is caused by 2 serotypes of hantavirus, Hantaan and Seoul. Even though the number of peripheral CD8' T-lymphocytes is markedly elevated in the acute phase of HFRS, the pathogenetic role of this phenomena is not clear. To elucidate the role of cell-mediated immune response in HFRS, we measured concentrations of cytokines, such as IL-1, IL-2, IL-4, IL-6, IL-10 and IFN-r in plasma samples obtained from 15 patients (15 men, 20-24 years old) at various phase of illness. Cytokines were measured by ELISA in plasma samples. The plasma concentration of IL-10 was elevated in 13/15 patients. The median maximum value, during days 4-8, was 23 ng/L (range, 0-70.2). The maximum values were significantly higher(p=0.005) than the baseline. IL-1 and IL-6 was detected in 6 patients. Other cytokine responses (IL-2, IL-4, IFN-r ) were negligible. There was no significant correlation between maximum levels of IL-10 and maximum serum concentration of creatinine (r=0.118, p=0.675), and lowest plate#let count (r=-0.088, p=0.765). Kinetics of IL-10 were quite similar in HFRS patients with maximum creatinine values >3 mg/dL and in those with values <3 mg/dL. In conclusion, plasma IL-10 levels, but not IL-2, IL-4 or INF-r were elevated in acute phase of HFRS, and it suggests that the systemic immunosuppressive activities of IL-10 might have some pathogenetic role in HFRS.
