2.Complications in the Treatment of Comminuted Fracture and Nonunion by Ilizarov Procedure

We reviewed the records of thirty-four patients who had been managed with the Ilizarov technique for the comminuted intra- and extra-articular fractures and nonunion of the long bones with or with-out bone defect. The goal of this study is to analysis the clinical complications of the Ilizarov technique, and to find out the methods solving them. 1. This study comprises 34 patients who were treated by Ilizarov technique at the Chonbuk National University Hospital from March, 1992 to June, 1993. 2. Among 34 cases, 22 were comminuted intra- and extra-articular fractures, and 12 were nonunion of the long bones. 3. The complications were divided intd problem, obstacle and complication; problem in 16 cases, obstacle in 11 cases and complication in 6 cases. 4. We conclude that the Ilizarov technique is a useful method in management of the severe commi- nuted fractures and the nonunion of the long bones, but we have to consider the complication sincerely and endeavor to reduce them.

3.Case report for Blepharospasm requiring careful considerations in the application of the blepharoplasty.

With the aging process, the senile lids are manifested by the excessive skin, decreased height of palpebral fissures with interference of the vision. Sometimes these patients often complained the blepharitis on the lateral canthal area and phtophobia. But there are many diseases to differentiate from senile lids such as dermatochalasia, blepharochalasia, hypertrophy of orbicularis oculi, herniation of orbital fat, myasthenia gravis, and blepharoptosis. Blepharospasm must also be differentiated because of sysmptoms similar to the above described senile lids. Essential blepharospam is an idiopathic progressively debilitating desease which involve bilateral spasm of the orbicularis oculi. So just only an conventional blepharoplasty could not get an improvement of symptoms and some times it makes the symptoms worsen. This case report is about blepharospasm patients visiting for blepharoplasty. And the patients got relatively the satisfied results with the injection of botulinum A toxin, aided by neurologist, instead of blepharoplasty. In conslutions, The blepharospasm in required to detect in preoperative evaluation for blepharoplasty. Once the blepharospasm is diagnosed, it is better to cooperate with neurologist and to do the blepharoplasty.

4.A Case of Acute Posterior Multlfocal Placoid Pigment Epithellopathy.

Acute posterior multifocal placoid pigment epitheliopathy(APMPPE), first described in 1968 by Gass, is chracterized by rapid loss of central vision secondary to multifocal, yellow-white placoid lesions at the level of the pigment epithelium and choroid and significant visual improvement after spontaneous resolution of the active lesions within several weeks or months. Fluorescein angiography shows chracteristically that the chtoidal fluorescence is not visible at the site of acute lesion in the early arterial and arteriovenous phases and become hyperfluorescent due to staining of the lesions in the late venous phases. We experienced a case of acute multifocal placoid pigment epitheliopathy in 26 years old female patient occurring in both eyes.

5.Clinical and Histopathologic Features of Pernio.

BACKGROUND: Pernio or chilblain is an abnormal reaction to damp cold air, resulting in pruritic erythematous swellings on acral skin. The histologic features are subepidermal edema, diffuse fluffy edema of endothelial cell, and superficial or superficial and deep perivascular lymphocytic infiltrates. Many studies with a large sample size have been conducted in western countries, but there were no collective studies of pernio that have been reported in Korea. OBJECTIVE: The purpose of this study was to clarify the clinical and histopathologic features of pernio in Korea. METHODS: We reviewed the clinical and histopathologic findings in ten patients with pernio who visited the Department of Dermatology, Kosin Medical College Gaspel Hospital during the 10 year period from 1988 to 1997. RESULTS: The results were as follows.1. The age distribution was in the range of 6-50 year old. The prevalance was heighest in the third and fourth decade. Sex ratio was 1:4 (2 males :8 females).The duration of symptoms ranged from 4 days to 30 years. All patients developed skin lesions on the acral area. Two male patients developed skin lesions on the hand associated with cold exposure and outdoor works. All patient had itching and burning papules and/or plaques, and one had a painful hard nodule. Laboratory studies were all negative and within normal ranges.2. Histopathologic features revealed subepidermal edema, diffuse fluffy edema of endothelial cell, and superficial or superficial and deep perivascular lymphocytic infiltrates in all specimens. 3. Treatment includes prophylatic measures against cold and nifedipine(calcium channel inhibiting drug). In more severe cases, pentoxifylline might be helpful. CONCLUSION: Our study suggests that pernio commonly occurs in twenty to forty year old females during fall and winter. Upon exposure to cold temperatures, the acral skin develops pruritic erythematous papules and plaques. If these lesions persisit for a long time or recur, they may progress to vesicle and ulceration and possibly develop vasculitis which does not respond to therapy well. Thus, a thorough understanding of clinical and histopathologic features of pernio is vital in making a diagnosis, a treatment plan and determining the prognosis.

6.Factors Influencing Willingness for Human Papilloma Virus (HPV) Vaccination in Female Students at One University.

BACKGROUND: This study was done to identify factors that influence the willingness to be vaccinated with the human papilloma virus (HPV) vaccine in female college students eligible for a catch-up vaccination program. METHODS: This cross-sectional study included 572 female students aged 18-26 years attending one selected university in the Gyeongbuk Province. From March 2 to 6, 2009, they completed a self-administered questionnaire on their HPV vaccination status. Thereafter, unvaccinated participants were educated about the vaccination program, and each participant's willingness to be vaccinated was assessed. RESULTS: Of the 572 participants, 18 (3.1%) were already vaccinated with the HPV vaccine. The number of participants advised to be vaccinated was 102 (17.8%). Of the 554 unvaccinated participants, 305 (55.1%) were willing to be vaccinated after being educated about the vaccination program. Of the remaining 249 unvaccinated participants, 116 (48.7%) refused the vaccine due to its high cost. Advice by family members was related significantly to the willingness to be vaccinated (odds ratio, 2.37; 95% confidence interval, 1.09-5.14). CONCLUSIONS: The results of this study suggest that the willingness to be vaccinated with the HPV vaccine could be increased in female college students if they are advised to do so by their family members after being educated about the vaccination program.

8.Castleman's Disease in Pelvic Cavity.

Castlemans disease is a relatively rare disease of differential diagnostic interest in patients with lymphadenopathy. The etilogy and pathogenesis are still not elucidated. One case of Castlemsns disease of the pelvis is reported. Tumoral lesion was located in the lymph nodes of the right broad ligament. Histologically, the lymph nodes demonstrated the the hyaline-vascular type of Castleman's disease which had a favorable prognosis. This unusal presentation made Castlemans disease difficult to diagnose preoperatively. We describe a patient with Castleman disease of hyaline-vascular type localized to the pelvic cavity.
