2.Placental Site Nodules & Plaques: A clinicopathologic analysis of 14 cases.

Placental site nodules and plaques have been recently described to designated single or multiple, well-circumscribed, rounded lesions at the placental site, composed of viable or degenerating intermediate trophoblastic cells and extensive hyalinization between the cells. We described clinicopathologic findings of 14 cases of placental site nodules and plaques. The age of 14 patients ranged from 25 to 39(average 33) years and all of them had been pregnant in the past. Ten of them presented with vaginal spotting, which was preceded by recent pregnancy in only 3 cases. Three patients presented with secondary infertility and one with secondary infertility and vaginal spotting. Urine pregnancy tests were negative in all 14 cases at the time of presentation. Ultrasonographic examination disclosed abnormalities in only 3 cases and the remaining cases were normal. Hysterosalpingography was performed in 3 patients who presented with 2 degrees infertility and revealed moderate to severe intrauterine adhesions. Microscopically, chronic endometritis of varying degrees evidenced by plasma cells and eosinophiles were present in all cases and these were more prominent in the vicinity of the lesions. It is presumed that the placental site nodules and plaques are not sloughed at the time of menstruation and it may cause chronic endometritis or intrauterine adhesions at any time after previous delivery.

3.A Case of Gronblad-Strandberg Syndrome.

Pseudoxanthoma elasticum associated with angioid streaks has been called rthe Gronblad-Strandberg Syndrome. A 30-year-old female patient showed circumscribed, cream-colored, crepelike, redundant plaques on neck, axillae, periumbilical area, and inguinal regions. The histopathologic findings of the skin taken from the axilla revealed extensive basophilic degeneration of the elastic tissue and calcium deposition in the middle and lower dermis. Both fundi showed irregular angioid streaks radiating from the optic disk on ophthalmoscopy, and hyperfluorescence of angioid streaks was noted with fluorescein angiography.

7.Two Cases of Eyelid Sebaceous Carcinoma.

Sebaceous carcinoma is a particularly common malignant tumor of the eyelid. Therefore, this lesion is important to ophthalmologist. Recently, there appears to have been an increased incidence or recognition of sebaceous carcinomas of the eyelid, which had been previously believed to be very rare. Frequently mistaken for a recurrent chalazion, invasion by sebaceous carcinoma is frequently enhanced by repeat curettage and forms of therapy that obscure the true diagnosis. This tumor is difficult to completely resect due to intraepithelial widespread, so its prognosis is poor. The authors experienced two cases of sebaceous carcinoma of the eyelid. One was a 57 year old female patient who was treated with surgical resection and the other was a 71 year old male with cervical lymph node metastasis. He was treated with surgical resection, but he rejected radiotherapy.

8.Pathologic Prognostic Indicators in Recurrence and Progression of Superficial Bladder Tumor.

PURPOSE: The identification of factors present at the time of initial evaluation may be important to predict the prognosis in patients with superficial bladder carcinoma. Attempts were made to identify predictive factors of recurrence and progression. MATERIALS AND METHODS: This study included twenty seven patients with superficial bladder tumor(pTa-pTl) who had no recurrence within 5 years. A comparative non-randomized study was performed of a control group of twenty six patients with same superficial bladder tumor who had recurrence within 1 year after transurethral resection of bladder tumor(TURBT) was done. Between the former and the latter patients cumulative and multivariate analyses were performed to estabilish predictive factors of pathological recurrence and progression. The resected tumors were immunohistochemically stained with anti-lamin rabbit serum to evaluate the presence or absence of basement membrane breakage, the vascular and/or lymphatic invasion, and to count number of neovascularizasion. RESULTS: In superficial bladder carcinoma, invasion of basement membrane, grade, invasion of lymphatics and vessels, and neoangiogenesis were significant factors affecting recurrence and progression, and these factors acted independently. CONCLUSIONS: These factors used to predict recurrence and progression of superficial bladder tumor were thought to be the important risk factors. These results suggested that close follow-up and aggressive treatment such as BCG instillation should be considered for the treatment of superficial bladder cancer with unfavourable prognostic factors.

10.Modulation of Apoptosis of Polymorphonuclear leukocyte by Plasma and Peritoneal fluid from Patients with Endometriosis.

OBJECTIVE: The effects of plasma and peritoneal fluid from endometriosis patients on apoptosis of neutrophils were investigated. METHODS: Neutrophils and plasma were obtained from women with endometriosis (n=20) and healthy control donors (n=20). Peritoneal fluid from endometriosis patients (n=10) and the controls (n=5) were collected prior to laparoscopic operation. Apoptotic changes of the cells were evaluated by morphological changes using Giemsa staining. DNA electrophoretic analysis were used to confirm apoptosis. RESULTS: Compared to the plasma of healthy controls, the addition of 10% plasma from patients with endometriosis to culture medium reduced the percentage of apoptotic cells from 65.3+/-6.6 % to 27.2+/-4.6% in cultures of neutrophils maintained in vitro at 24 h. The addition of 10% peritoneal fluid obtained from patients with endometriosis to culture medium also inhibited the apoptosis of neutrophils compared with control group (10.5+/-4.3% vs. 45.3+/-4.8%). 10 ng of interleukin-8 (IL-8) in the presence of plasma from controls and endometriosis patients further delayed the spontaneous apoptosis of neutrophils. Neutralizing IL-8 antibody abrogated the delay of apoptosis of neutrophils induced by peritoneal fluid but not in the plasma of endometriosis patients. Neutrophils obtained from patients with endometriosis showed delayed apoptosis in the absence and in the presence of plasma or peritoneal fluid from either source and this delay was not abrogated by neutralizing IL-8 antibody. CONCLUSION: These findings show that IL-8 is one of the neutrophil survival factors in the peritoneal fluid of endometriosis patients, which contains both pro-apoptotic and anti-apoptotic cytokines. However, our results indicate that an unidentified survival factor is present in the plasma of patients with endometriosis, and also suggest that the impairment of innate immunity, such as the delayed apoptosis of neutrophils may be related with pathogenesis of endometriosis.
