1.Primary Malignant Melanoma arising in Mucosa of the Palatine Tonsil: A case report.

Maligant melanomas of the oral or nasal cavity, and the vulvovaginal area are relatively common among the melanomas of non-ocular mucosa. But, primary malignant melanoma arising in the mucosa of the palatine tonsil is rare. We present a case of primary malignant melanoma arising in the mucosa of the palatine tonsil. A 36-year-old male was admitted for evaluation of a recurrent sore throat. Tonsillectomy was performed on the basis of clinical suspicion of chronic tonsillitis. Grossly, the left tonsil was focally dark. Microscopically, the tonsillar mucosa was diffusely infiltrated with tumor cells. Tumor cells revealed numerous melanin pigments. Intraepithelial nests of tumor cells were noted, but pagetoid spread of tumor cells was not found. Tumor cells were positive for S-100 protein and HMB45 stain. There was no evidence of melanoma in the skin or eye.

2.Clinical Study on Relation Between Herpes Zoster and Underlying Diseases.

This study was designed to observe more closely about clinical course, complication, predilection site of skin eruption and associated diseases with herpes zoster. Total 122 cases of herpes zoster were prospectively observed from April 1986 to July 1987. The results obtained were as follows : 1. The ineidence of herpes zoster versus the total number of outpatients was 20.6%. There was no difference in sex ratio of male(52.5%) to female(47.5%). 2. Herpes zoster was observed most frequently in the 50-59 year age group (25.4%). 3. There were no marked monthly or seasonal difference in herpes zoster without an explosive outbreak or an epidemic cycle. 4, We found that 68 cases(55.7%) were associated with underlying diseases : the most common disease was hypertension(23 cases), the next one was malignsncy(10 cases), namely, lung cancer, Hodgkin's lymphoma, other malignant lymphoma, leukemia and mediastinal mass, and the other diseases were tuberculosis of intenal organs, diabetes mellitus, hepatic diseases, cardiac diseases, chronic pulmonary diseases, cord injury, gastric ulcer, brain damage, burn, renal diseases, rheumatoid arthritis, and measles. 5. It was suggested that the distributuon of skin lesion were related to dermatome. The incidence of skin lesion was distributuon in the following region in order : T5-T8(25.4%), C6-T4(24.6%), Cl-C5(20.5%), Ll-S3(13.9%), Vl-V3 (12.3%), and T9-T12(3.3%), there was involvement of the entire thoracic dermatome(C6-T12) in 65 cases(53.3%). A great many cases of malignancy, chronic pulmonary disease, cardiac disease, tuberculosis and gastric ulcer were most frequently distributed in the thoracie dermatome, but the involvement of ophthalmic dermatome was found only in the patients with hypertension. 6. The most common complication was postherpetic neuralgia for 31 cases(25.4%) with the highest frequency in 60's(32.3%). Other complications were ophthalmic complication, Ramsay-Hunt syndrome, secondary bacterial infection, moter nerve involvement, difficulty of urination, and recurrent herpes zoster.

6.A Case of Sebaceous Gland Carcinoma of the Eyelid with Huge Mass.

We report a case of sebaceous gland carcinoma occuring in a 55 year-old woman, which showed huge ulcerated tumor mass measuring 6X7 cm in size and orbital invasion on left lower eyelid without any evidence of metastases. She was treated with wide excision of the lesion and exenteration of the eyeball, and then grafted with skin and muscle. We obtained good result with surgical treatment until 8 months observation after surgical operation.

8.Two Case of Scleredema.

We report two cases of scleredema, which appeared as symmetrically nonpitting wooden-like indurated edema on the nape, shoulder, and upper and mid portion of back in a 49-year-old woman and a 72-year-old man, associated with long standing diabetes mellitus for the past 5 and 14 years respectively. The scleredema showed no tendency to resolve. Acid mucopolysaccharides were demonstrated histochemically between collagen bundles in the dermis, especially in the mid and lower dermis.

9.A Malignant Melanoma Showing Excellent Effect with Combination of DTIC Chemotherapy and BCG Immunotherapy.

We report a case of malignant melanorna, which showed dramatic effect by combination therapy of DTIC and BCG immunotherpy. The case was s 56-year-old female patient who had an asymptomatic dark brown colored walunt sized protruding tumor with ulceration of the medial aspect of lower one third of right leg for 1 year and palpable masses over the right popliteal fossa and inguinal area. Histologic fingings showed cellular mass composed of epithelioid cells with atypical nucleus, occasionally mitotic figure and numerous melanin granules scattered in the epidermo-dermal junction and papillary or reticular dermis. The mass was diagnosed nodular malignant melanoma which showed Clark level W with metastasis in 4th thoracic vertebra and lymph node. After surgical remove of the primary lesion, chemo-immunotherapy with combined DTIC and BCG was tried, and all the lesion were completely dissppeared. No evidence of recurrence either in primsry focus or other sites was observed up to one year and eight months.

10.Heart Transplantation in a Patient with Complex Congenital Heart Disease, Physiologic Single Lung, and Severe Pulmonary Hypertension

Here we report a case of successful heart transplantation (HT) in a patient with high risk on HT due to her complex congenital heart disease and its consequent complications; physiologic single lung and severe pulmonary arterial hypertension. HT was successfully performed in our patient using meticulous perioperative management techniques, such as control of collateral vessels, appropriate donor selection, pulmonary vasodilator therapy, post-transplant extracorporeal membranous oxygenation support, and atrial septostomy for right ventricular unloading.
