The development of a safe method of transfer of the greater toe for thumb reconstruction, using dorsalis pedis artery and the great saphenous vein, suggested the possibility of one stage microvascular transfer of the second toe for reconstruction of surgery. Based upon our part experience with microsurgical experiments, limb transplantation and toe-to-thumb transplantation since 1975, we accomplished the second toe-to-index transplantation done in Korea on October 24, 1979, and succeeded in one stage second toe-to-index transplantation in two cases. Following results were obtained. I. Free second toe-to index transplantation on making a index is the most excellent method of index reconstruction with results in minimum functional and cosmetic impairment. 2. Technically, skin incision, isolation of blood vessels and flexor tendon in preparation for transplantation of second toe is more difficult than that of greater toe. 3. The specialized microsurgical training with skillful technique is essential for success.

To determine the difference in cephalometric measurements between centric relation and centric occlusion, 60 patient-20 patients for each malocclusion group-were examined with the cephalograms in centric relation and in centric occlusion. The results are as follows: 1. There are differences in cephalometric measurements with the mandible in the two different position, and some measurements are statistically significant. 2. No clinically useful prediction may be made from cephalometric radiographs concerning the amount of mandibular deflection from centric relation to centric occlusion. 3. If, the large differences in the cephalometric measurements with the mandible in the two different position, the analysis with the cephalogram in centric occlusion is needed to be compensated with the other clinical approach.

The purpose of this study was to quantify the biologic effects of the tensile forces from helical springs across the maxillary incisors on the periodontal tissues of rats. 39 Sprague-Dawlely rats were divided into a control group(3 rats) and three experimental groups(36 rats)-group 1, pressured with a light force(50-75g), group 2, with a heavy force(250-300g) and group 3, with a heavy force(250-300g) plus laser irradiation. Autoradiographic and histopathologic observations were performed in 12, 24, 48 and 96 hours after force application. The result were as follow: 1. Hyalinized zone of periodontal ligament began to appear at pressure side in 12 hours in group 2 and group 3 ; all decreased in 96 hours except in group 2. 2. Alveolar bone resorption began to appear in 12 hours in group 2 and group 3 ; Group 2 showed more resorption than group 1 and no difference to group 3. 3. Tearing of periodontal ligament and vascular dilatation began to appear at tension side in 12 hours in all groups ; Group 2 showed more changes than group 1 and no difference to group 3 ; Decrease began to appear in 96 hours. 4. New bone formation began to appear at tension side in 12 hours and increased more and more; No differences were shown of groups 5. New capillary proliferation began to appear at pressure side in 12 hours; The changes were the greatest in group 3, group 2, group 1, in that order. 6. Positive reaction of cells to [3H]-thymidine was the greatest in 24 hours of all groups and decreased with times ; Group 2 showed more reaction than group 1 and no difference to group 3.

Chordomas are maligaant tumors which arise in the remnants of the embryonic notochord, and exhibit a definite predilection for the extremes of the spinal axis, the great majority arising either in the basioccipital or in the sacrococcy geal regions. Particularly, complete surgical removal is almost impossible, therefore, local recurrence after the surgical removal of this tumor is common or rule. This report describes a csse of chordoma arising sacrococcygeal region treated with block resection.

The authors performed the interfascicular nerve grafting in the gap of injured nerves using the microsurgical technique to obtain the accurate coaptation of the fascicles. The authors experienced the 33 cases(26 patients) of the interfascicular nerve grafting in the median(11), ulnar(9), radial(2), common peroneal(3) and digital(8) nerves at the Department of Orthopaedic Surgery, College of Medicine, Kyung Hee University from Jan. 1980 to June 1984. The patients were followed up from 12 to 26 months (average 20 1/2 months). The following results were obtained; 1. The shorter the length of the donor nerve and the younger the age of the patients are, the better the recovery of the nerve function is. 2. The medial and lateral cutaneous nerves of forearm as donor nerve are suitable as well as the sural and superficial radial nerves. 3. The end result of each patient was evaluated by Seddon's classification. The excellent motor recovery was 72.0% (18/25) and the excellent sensory recovery was 81.8% (27/33). 4. The interfascicular nerve grafting is an excellent method in treating the widely separated,severed peripheral nerve injury.

It is difficult to treat the children with the growing deformity of long bone due toepiphysealloss or absence, because resulting deformith and discrepancy of limb length is progressive. There were many conventional treating methods including the lengthening and shortening, corrective osteotomy, and epiphysiodesis. But these procedures result in inadequate results, and there are limitation in treatment of these children. We tried the free vascularized epiphyseal transplantation using the proximal fibular epiphysis in 3 patients with epiphyseal injury or loss due to old tauma, wide excision of multiple exostosis and congenital clubhand. The duration of follow-up was from 20 months to 57 months, and the lengthening of the transplantedepiphyses was evaluated by the scanogram inregular interval. As final radiologic check, each transplanted epiphyseal growth was 1cm for 24months in case 1, 0.9 cm for 10 months in case 2, 1 cm for 24 months in case 3 (av. 1.3cm for 24 months) and the adjacent joint function was good. There was no gross deformity.

BACKGROUND: Recently, childhood obesity has increased and became a major health concern in Korea. The aim of this study is to measure the prevalence of childhood obesity in rural city and to explore the risk factors of obesity including obesity of parents. METHODS: We made 13 convenience samples of elementary school and attached kindergarten located in Asan-city, ChungNam, in 2001. We surveyed children's height, weight, and risk factors of childhood obesity with a self-recorded questionnaire answered by parents. Children's obesity was evaluated by ideal body weight which is defined as the 50th percentile of weight for Korean children of the same height and sex in 1998. The criteria of parents' obesity was over 25 of BMI. RESULTS: The subjects were 1,558 children among 1870 respondents. The prevalence of childhood obesity was 9.4% and that of male children was 11.0% and that of female children was 7.9%. The older in both male and female, the higher the obesity prevalence was. Mother's age were significantly higher in obese children. Birth weight and family income were also significantly higher in obese children. There was a higher family history of obesity in obese children. Risk factors associated with childhood obesity were gender, age of child, mother's age, birth weight, number of siblings, family income, and family history of obesity. CONCLUSION: The prevalence of childhood obesity in Asan-city was 9.4%. Children whose father, mother, or parents were obese tended to be obese.

Castleman's disease is a rare disorder characterized by tumorous masses that may develop in the lymph node tissue throughout the body. Most common location is mediastinum, but it can also affect retroperitoneum, neck, pelvis, and/or axilla. It may exceptionally affect extranodal sites like striated muscle, thoracic wall, lungs, skull, larynx, and/or vulva. The presentation is varied and diagnosis is difficult. There are two main types of Castleman's disease: hyaline-vascular type and plasma cell type. The hyaline vascular type accounts for approximately 90% of the cases. Most individuals exhibit no symptoms of this form of the disorder or they may develop non-cancerous growths in the lymph nodes. The plasma cell type is often associated with fever, weight loss, skin rash, early destruction of red blood cells, leading to unusually low levels of circulating red blood cells (hemolytic anemia), and/ or abnormally increased amounts of certain immune factors in the blood (hypergammaglobulinemia). We here report a case of Castleman's disease of ileal mesentery in 30-years old female patient. Abdominal mass, 4.7x3.6 cm in size, was completely removed from ileal mesentery without complication, and confirmed histologically mesenteric Castleman's disease of the mixed type.
