1.A Case of Combined Interstitial and Intrauterine Pregnancy as Fifth Ectopic Pregnancies after In Vitro Fertilization and Embryo Transfer in Bilateral Salpingectomy patient.

Although a higher incidence of ectopic pregnancy has been reported after in vitro fertilization(IVF) and embryo transfer(ET), five ectopic pregnancies in one patient is very rare. We experienced a case of combined interstitial and intrauterine pregnancy as fifth ectopic pregnancies after in vitro fertilization and embryo transfer in one patient with history of bilateral salpingectomy due to previous four tubal pregnancies. Repeated transvaginal ultrasound examinations confirmed heterotopic pregnancy. In IVF-ET, bilateral salpingectomy does not remove the risk of interstitial or cornual pregnancy.

2.Fluorographic Findings of Normal Optic Disc.

One hundred normal optic discs were analysed by fluorographic fundus pictures. Horizontal diameter of optic disc was set as 10. The following results were obtained. 1) The vertical diameter of optic disc was larger than the horizontal one. (10.77 : 10). 2) The ratio of diameters of cenitral retinal vein(CRV) was 1 : 1.51 before and 1 : 1.47 after fluorescein injection. 3) The central retinal artery(CRA) located nasal to the CRV in most cases. 4) In nearly half of cases, physiologic cups located at the nasal portion of optic discs. 5) The average size of physologic cup was 2.90. 6) The cilioretinal artery was detected in 58% of cases in which one cilioretinal artery was found in 42%, two was found in 16%. 7) The CRA made branchings more than retinal veins (7.74 : 5.48) and total sum of diameters of retinal artery was larger than that of retinal vein at the disc margin.

6.Nitric Oxide Synthase Expression and Apoptosis in Psoriasis.

BACKGROUND: Nitric oxide(NO) is generated from L-arginine by NO synthase(NOS). Three types of NOS are currently known:inducible(iNOS), neuronal(nNOS) and endothelial(eNOS). NO has been found to be important in a number of different physiological processes. Of particular relevance to the skin are the roles of NO in vasodilatation, inflammation, immunomodulation and in oxidative damage to cells and tissues. NO exhibits contradictory effects in the regulation of apoptosis. The proapoptotic effects seem to be linked to pathophysiological conditions, where high amounts of NO are produced by iNOS. In contrast, the continuous release of eNOS inhibits apoptosis. Psoriasis is a common chronic skin disease, but the cause of psoriasis is not definitely known until now. OBJECTIVE: To investigate the role of NO in psoriasis pathogenesis, such as inflammatory infiltration, dermal vessels dilatation, apoptosis, we performed this study. METHODS: Ten cases of psoriasis and 5 cases of normal skin for immunohistochemical with antibodies to iNOS, PCNA, bcl-2, p53 and TUNEL stainings, and 5 cases of psoriasis and 2 cases of normal skin for western blot with antibody to iNOS were investigated. RESULTS: The results were summarized as follows.1. Immunohistochemical staining with iNOS showed positive reactions in 9 cases(90 %) of psoriasis, 4 cases among them were strong positive staining, but all cases of normal skin were negative.2. Labelling index of PCNA staining was 24.4+2.5%, 2.4+0.7% in psoriasis, normal skin, respectively.3. All cases of psoriasis and normal skin were negative in p53 staining, but squamous cell carcinoma as positive control was positive.4. Bcl-2 staining showed focal positivity in 5(50%) cases of psoriasis, but diffuse positivity in epidermal basal layer of normal skin.5. TUNEL staining showed positivity in 7(70%) cases of psoriasis, but all of normal skin were negative.6. Western blot with anti-iNOS showed positive 130 kDa band in psoriatic, but not in normal skin tissues. CONCLUSION: The results suggested that NO was considered to play a role in psoriasis pathogenesis, including apoptosis, dermal vessels dilatation and inflammatory infiltration.

8.One Case of Intra-Orbital Hemangio-endothelioma.

Here, we report one case of intra-orbital hemangio-endothelioma. The patient was a Korean male aged 37. The most outstanding complaint was exophthalmos of the left eye. His left eye began to protrude insidiously without pain since 4 years ago. A soft movable mass was palpated on the medial portion of the upper orbital margin and the ocular motility was limited to upward, but ocular deviation was not present. Pathologic diagnosis was intra-orbital hemangio-endothelioma, sclerotic, borderline malignancy. This tumor was well encapsulated and any evidence of involving bone and soft tissue was not found. The size of was 2cm X 2.5cm X 3.5cm. Extirpation was performed by means of superior orbital approach. Tumor mass was removed entirely with finger dissection. The vision was not changed but the cosmetic result was satisfactory.

9.A Case of Ewing's Sarcoma from Temporal Bone.

A rare case of primary Ewing's sarcoma arising from left temporal bone is presented. This case showed child first sized tender scalp mass in left temporal area is 17 year old boy. With CT scan of brain, authors identified intracranial and extracranial extension of soft tissue mass associated with temporal bone change. Authors performed total resection of mass. Histologically, Ewing's sarcoma was confirmed.

10.Intravenous immunoglobulin for patients with repeated failure of in vitro fertilization-embryo transfer.

OBJECTIVE: The implantation failure after embryo-transfer (ET) is a major continuing problem in in vitro fertilization (IVF). This study was undertaken to determine the effectiveness of intravenous immunoglobulin for treatment of individuals experiencing repeated unexplained in vitro fertilization-embryo transfer (IVF-ET) failure. METHODS: A total of nine consecutive infertile patients who failed to become pregnant after previous IVF-ET replacing at least three or more normal developed embryos each were included in our study. During the subsequent new IVF-ET cycle, each women received intravenous immunoglobulin 500mg/kg before the embryo transfer. RESULTS: Only one implantation occurred. There were no remarkable side effects. A specific effect of intravenous immunoglobulin for patients with repeated IVF-ET failure could not be demonstrated. CONCLUSION: High-dose intravenous immunoglobulin may not be useful for patients with repeated failure of embryo transfer.
